Regulations pertaining to the Historic Preservation Commission are
found in Chapter 21 (Zoning), South Bend Municipal Code and Chapter
26 of the St. Joseph County Code.
The issuance of this certificate does NOT in any manner, release
the recipient from the responsibility of complying with the requirements
of the zoning ordinances, building codes, safety codes, ADA or other
requirements of the City of South Bend, the County of St. Joseph, the
State of Indiana, or the United States Federal Government.
This certificate is good for one year from the date of issuance and
is effective from the date entered herein. Plans are on file and open for
public inspection at the office of the Historic Preservation Commission of
South Bend and St. Joseph County, 227 West Jefferson Blvd., Suite 1400
S, South Bend, Indiana, during normal business hours.

Hi Adam,
Attached is the site plan and the COA application. We've mailed the original so you have one to scale in case the
attachment doesn't print to scale.
I believe all the other pictures you need are contained with this email thread. We are also removing and replacing a
basement door located on the back of the house. Pictures below.
It is a steel 6‐panel door and we are replacing it with another just like it. The existing door doesn't function and we are
replacing the frame as well to match the existing.
We will make sure that a barrier 2x4 remains outside the sliding patio door until completion of the deck for safety.
Let me know if you need anything else. I'll call and try to make a payment
