AMENDMENT TO OWNER-ENGINEER AGREEMENT Amendment No. __2___ 1. Background Data: a. Effective Date of Owner-Engineer Agreement: b. Owner: Board of Public Works, City of South Bend, Indiana c. Engineer: Black & Veatch Corporation d. Project: Wastewater Treatment Plant Electrical Improvements Design and Construction Phase Services 2. Description of Modifications: a. The Scope of Services currently authorized to be performed by Engineer in accordance with the Agreement and previous amendments, if any, is modified as follows: Task 202 – Construction Administration Engineer will assist the Owner with assistance in review of supporting documentation for an additional 10 change order requests from the Contractor for a total of 15 including engineering assistance to ensure a sound resolution for the Contractor’s out-of-scope cable installation change. . Task 203 – Construction Progress Meetings Engineer participated in various teleconference meetings with the Owner and Contractor to coordinate work and answer questions pertaining to SCADA, VFDs, generators and two-week outage advance notice. Task 208 – Project Management and Administration Engineer provided project supervision, direction and coordination with the Owner’s management and staff for an additional 14 months for a total of 32 months. Project administration and management also includes budget and schedule control, coordination of activities and monthly invoicing. b.
