Dear Mr. Alvarado:
At its August 10, 2021 meeting, the Board of Public Works approved the Final Change
Order for this project, for an increase of $10,999.03. The final contract amount is $50,395.03.
In addition, the Project Completion Affidavit for this project was approved in the amount
of $50,395.03. Copies of the Change Order and Project Completion Affidavit are enclosed for
your records.

SUBJECT: Change Order and Closeout Paperwork
120-051R Charles Martin Parking Lot

The enclosed documentation contains necessary paperwork for closeout of the subject
project, including a Project Completion Affidavit and a Waiver of Lien. The amount of the
change order exceeds the 20% limit as set by Indiana State Code. The change order (final) is
explained below.
During the milling of the parking lot, it was discovered that several ‘soft’ areas of asphalt existed
under the parking lot, essentially having been worn down to fine graded soil beneath. Extra
asphalt was required to bridge over or fill these softer areas. While a quantity of approximately
20 tons was included in the original contract to account for areas like this, the total over-run was
40 tons, or roughly one extra truckload of asphalt.
Also during the milling operation, it was discovered the eastern drywell, outside the rear
entrance to the building, had failed and required full replacement. This required a new precast
concrete structure, and a separate pipe crew for the contractor to install.
Neither of the adjustments required to the contract could have been reasonably foreseen.

Zach,
Our Change Order price for replacing the drywell at the Charles Martin Parking Lot Project No. 120-
051R is $8,220.00 (1 LSUM). That price includes removal of the existing drywell, and installation of
one new 1,200 Gallon Drywell. We have anticipated re-using the existing frame and grate casting for
the new drywell and have therefore not included the cost of a new frame and grate in this price.
Please also note that any tie-ins to equalizer and/or downspout drainage pipes that may flow to the
existing drywell have not been included in this price as we were not able to see any such pipes
during our field inspection of the existing drywell and therefore did not anticipate needing to
perform any such work. As we discussed over the phone, we have scheduling availability to install
this drywell tomorrow on Saturday 6/19/2021 provided that we have the City’s approval of this
additional cost to the project and the City’s authorization to work on this specific Saturday. Please
reply back to this e-mail at your earliest convenience to either approve or deny this change order
pricing and scheduling request.
