At its July 27, 2021 meeting, the Board of Public Works approved the rejection of your
quote for the above referenced project as non-responsive due to incorrect MWBE Utilization
Form MWBE -1.0. The project will be re-quoted at a later date, with updated form usage
clarified.

Upon review of the quote package, I recommend rejecting the quote as non-responsive due to
the incorrect MWBE Utilization Plan Form MWBE – 1.0. The forms attached to the quote
solicitated were incorrect or outdated and did not include two pages, one for an MBE goal and
one for a WBE goal. Corrected forms will be attached to a follow up solicitation in response to
rejection of this quote.
