PROJECT DESCRIPTION A. The project includes the furnishing of all equipment, material, insurance, and labor for the cleaning of storm sewers, pond maintenance and associated work near Laurel Woods subdivision within the City of South Bend, IN. II. PREVAILING SPECIFCIATIONS AND CONSTRUCTION STANDARDS AND SPECIFICATIONS A. The City of South Bend's PREVAILING SPECIFICATIONS, most recent version, and CONSTRUCTION STANDARDS AND SPECIFICATIONS, most recent version, are to be used on this project. B. Each Quote provider is specifically instructed to become completely familiar with the most recent version of the PREVAILING SPECIFICATIONS and the CONSTRUCTION STANDARDS AND SPECIFICATIONS prior to submitting a quote. C. Wherever the PREVAILING SPECIFICATIONS refer to “State Specifications,” it shall mean the 2020 INDOT Standard Specifications for the letting effective after September 1, 2019. D. These SPECIAL PROVISIONS will list only "Additions" or "Deletions" to the PREVAILING SPECIFICATIONS and are to be used only in conjunction with the PREVAILING SPECIFICATIONS. E. In the event of conflict between the SPECIAL PROVISIONS and the PREVAILING SPECIFICATIONS, the SPECIAL PROVISIONS will govern. III. TERM “OR EQUAL” A. Prevailing Specifications: None B. Additions 1. Where the term "or equal" is used in these specifications, the Quote provider deviating from specified item shall file with his/her/its Quote a letter fully explaining and justifying his/her/its proposed article or equal. The City of South Bend shall be the sole judge in determining if the “or equal” offered meets the specification. IV. TAX EXEMPT A. Prevailing Specifications: None B. Additions 1. Materials and properties purchased under contract with the Owner that becomes a permanent part of the structure or facilities constructed are not subject to the Indiana Gross Retail Tax (Sales Tax). The exemption number will be furnished to the Contractor upon award. V. INDEMNIFICATION A. Prevailing Specifications: None B. Additions 1. Contractor agrees to indemnify, defend and hold harmless the City of South Bend, its agents, officers and employees, from all costs, losses, claims and suits, including court costs, attorney fees, and other expenses, arising from or out of the negligent performance of this Contract by Contractor, or because of arising out of any defect in the goods, materials or equipment supplied by the Quote provider. All Contractors and subcontractors doing business with the City of South Bend shall present a Certificate of Insurance showing coverage in the following minimum amount: a. General Liability: Premises-Completed Operations or Products, Bodily Injury and Property Damage Combined Single Limit - $5,000,000. b. There shall be no exclusion for explosion, collapse or underground hazard. c. Workmen’s Compensation: Statutory State of Indiana Employer's Liability - $100,000. d. Auto Liability: Bodily Injury and Property Damage Combined Single limit - $1,000,000. e. The City of South Bend shall be named as additional insured on the Certificate of Insurance. VII. CONTROL OF WORK A. Prevailing Specifications: 2020, INDOT Standard Specifications Section 105 B. Additions: 1. The complete responsibility for this project lies with the Director of Public Works of the City of South Bend, Indiana acting through his authorized representatives. 2. Construction Engineering - The Contractor shall provide all the necessary, qualified personnel, equipment and supplies to perform all work required under this item. There will be no direct payment for this item. 3. The contractor is responsible to maintain the site which includes but is not limited to; dust control, site security, erosion control, and protecting adjacent properties. 4. Work hours for the Project shall be from 7:00 a.m. through 6:00 p.m., Monday through Friday. No work shall be permitted on weekends, Holidays, or after hours unless approved by the City of South Bend Department of Public Works. 5. Access to all properties, driveways and Public streets shall be maintained at all times throughout the Work. VIII. LEGAL RELATIONS A. Prevailing Specifications: 2020, INDOT Standard Specifications Section 107 B. Additions: 1. The Owner, where mentioned in these documents, is the City of South Bend. The Engineer, where mentioned in these documents, is McCormick Engineering, LLC. 2. The Contractor shall apply for and obtain any and all required permits for the work from local, state, and federal agencies and shall comply with permit requirements, including the St. Joseph County / City of South Bend Building Department. 3. If the Contractor awarded this contract is not a resident of Indiana, within thirty days, the Contractor shall provide the Owner with proof that the Contractor is duly licensed, qualified and registered with the Secretary of State of Indiana to engage in business within the State of Indiana. 4. The Contractor shall comply with all provisions of Indiana Code 5-16-13, as amended. The Owner reserves the right to immediately terminate the public work project awarded should the Contractor be found to be in violation of any provision of Indiana Code 5-16-13 and such violation shall result in the forfeiture of Contractor’s performance bond to the Owner. Version 2/14/2020 Page - 5 Public Works Quote IX. PROSECUTION AND PROGRESS A. Prevailing Specifications: 2020, INDOT Standard Specifications Sec. 108 B. Additions: 1. The City, Engineer, and Contractor will hold a pre-construction meeting following award of the contract. The date of the Notice to Proceed (NTP) will be agreed at that meeting. 2. The project will have a completion date of 45 calendar days from the mutually agreed NTP. The Contractor shall not propose a date for the NTP that would result in a completion date past October 25, 2021. 3. The contract time will start when the Notice to Proceed is delivered and signed. 4. Contractor shall provide a schedule to the Owner prior to beginning any work on the site. 5. No work shall begin on the site prior to receipt of the NTP by the Contractor. X. CHANGE OF CONTRACT TIME A. Prevailing Specifications: 2020, INDOT Standard Specifications Section 108 B. Additions 1. The Contract Time may only be changed by Change Order. Any Claim for an extension in the Contract Time shall be based on written notice delivered to the Department of Public Works within seven (7) calendar days of the occurrence of the event giving rise to the claim. Notice of the extent of the claim with supporting data shall be delivered within fourteen (14) calendar days after such occurrence unless an official of the Public Works Department allows an additional period of time to ascertain more accurate data. The Contract Time will be extended in an amount equal to time lost to delays beyond the control of the Contractor if a claim is made in accordance with this provision. Such delays shall include acts of neglect by the Public Works employees, or to fires, flood, labor disputes, epidemics, abnormal weather conditions, governmental procedures, or acts of God. 2. Unless otherwise provided, the Contract time is based upon normal weather conditions. An extension is granted for weather conditions significantly more severe than normal if the Contractor demonstrates to the satisfaction of the City that the delay in the progress of the work was due to such weather. The basis to define normal weather will be the data compiled by the United States Department of Commerce, National Oceanic and Atmospheric Administration (NOAA). 3. No extension of time will be granted if the Contractor, by his/her/its own action or inaction, including fault or negligence of Contractor’s subcontractors, caused the delay, or for which any remedies are provided under any other provision of this agreement. 4. The grant of an extension of time under this Section in no way constitutes a waiver by the City of any rights or remedies existing under this contract at law or in equity. XI. DEFAULT AND TERMINATION A. Prevailing Specifications: 2020, INDOT Standard Specifications Section 108 B. Additions: 1. Events of Default shall include Contractor’s failure to perform any of its obligations under this contract including failure to commence work at the time specified, failure to perform the work in accordance with these specifications, unauthorized discontinuation of the work, failure to carry out the work in a manner acceptable to the City, failure to observe Federal, State, or local laws or regulations, and failure to comply with any other term of this contract. 2. If an Event of Default occurs, the City shall provide Contractor written notice and may permit Contractor ten (10) calendar days after the date of the notice to cure the default. If the default is not cured within the ten (10) day cure period, the City may at any time thereafter terminate this contract in which case the termination shall be final and effective. 3. Upon an Event of Default, the City may invoke the following remedies in addition to those remedies provided under separate provisions of this contract, the right of set-off against any payments due or to become due to the Contractor against the retainage, the right to take over and complete the Work. If the City notifies Contractor that City is invoking its right to complete the Work, all rights that the Contractor has in order under Contractor’s subcontracts are assigned to the City, subject to the City’s right to take assignment of all or only selected subcontracts at the City’s discretion. The sole Version 2/14/2020 Page - 6 Public Works Quote obligation accepted by the City under such subcontracts is to pay for Work satisfactorily performed after the date of the assignment. In the event a conditional assignment has not been executed, the Contractor shall execute or cause to be executed any assignment, agreement, or other document that may be necessary in the sole opinion of legal counsel to the City’s Board of Public Works to evidence compliance with this provision. The Contractor shall promptly deliver such documents upon the City’s request. In the case of such assignment, unless otherwise agreed in writing, The Contractor remains liable to subcontractors for any payment already involved, and for any claim, suit or cause of action based upon or resulting from any error, omission, negligence or other breach of contract by the Contractor, its officers, employees, or agents arising prior to the date of assignment to the City. XII. LIQUIDATED DAMAGES A. Prevailing Specifications: 2020, INDOT Standard Specifications Section 108 B. Additions: 1. The contractor shall proceed with the work at such rate of progress to insure full completion within the Contract Time. It is expressly understood and agreed, by and between the Contractor and the Owner, that the Contract Time for completion of the work described herein is a reasonable time, taking into consideration the average climatic and economic conditions and other factors prevailing in the locality of the work, and excludes the time for unavoidable delays which were beyond the control and without the fault of the Contractor. 2. If the Contractor shall fail to complete the work within the Contract Time, or extension of time granted by the Owner, then the Contractor will pay to the Owner the amount for liquidated damages a sum of five hundred dollars ($500.00) for each calendar day that the Contractor shall remain in default after the time of completion stipulated in the Contract Documents. 3. The Contractor shall not be charged with liquidated damages or any excess cost when the delay in completion of the work is due to the following and the Contractor has promptly given written notice of such delay to the Owner and Engineer/Architect. a. To any preference, priority, or allocation order duly issued by the Owner. b. To unforeseeable causes beyond the control and without the fault or negligence of the Contractor, including but not restricted to acts of God, acts of public enemy, acts of the Owner, acts of another Contractor in the performance of a Contract with the Owner, fires, floods, epidemics, quarantine restrictions, strikes, freight embargoes, and abnormal and unforeseeable weather. XIII. RETAINAGE AND FINAL PAYMENT A. Prevailing Specifications: 2020, INDOT Standard Specifications Section 109 B. Additions: 1. Payments will be made every thirty (30) calendar days 2. Consistent with provisions of IC 36-1-12-14, the Board of Public Works shall retain a percentage of payments throughout the duration of the project. 3. Before final payment and retainage are released, the Contractor must satisfy the following: a. All parts and labor meet requirements stated in the specifications. b. Provide copies of test reports or cut sheets on all materials supplied. c. Provide As-Built drawings in accordance with the City of South Bend Prevailing Specifications for Public Works. d. One (1) copy of the City of South Bend Completion Affidavit and one (1) copy of a Final Waiver of Lien. Version 2/14/2020 Page - 7 Public Works Quote XIV. WARRANTY A. Installation and safety requirements XV. OTHER UTILITIES A. Prevailing Specifications: None B. Additions: 1. The Contractor shall verify the locations of all utilities by contacting Indiana811 by calling 811 (in Indiana) or 1-800-382-5544 (outside of Indiana), or via www.indiana811.org, at least two (2) working days, not counting Saturdays, Sundays or federal and state holidays before proceeding with construction. It shall also be the Contractor’s responsibility to contact any other utility that is not contacted by Indiana811 and verify the utility locations. 2. The Contractor shall be responsible for working with the other utilities, i.e., gas, electric, telephone, etc. in order to assure that all utilities that need to be replaced or relocated can be done with a minimum disturbance to service. The Contractor shall also be responsible for coordinating schedules with the various utilities such that they can proceed with their relocation work as efficiently as possible. 3. If the odor of natural gas is detected in a work area at any time during the course of work, the Contractor shall immediately notify NIPSCO at 1-800-634-3524. The Contractor shall also immediately notify the residents of adjacent properties. The Contractor shall advise the residents to evacuate their homes immediately if the odor is present within the dwelling. 4. Restoration of sprinkler systems damaged by the Contractor's operations shall be repaired by the Contractor at no additional cost to the City or the Owner of the system. XVI. MAINTENANCE OF TRAFFIC A. Prevailing Specifications: 2020, INDOT Standard Specifications Section 105, 801, and City Construction Specification 1-8.1 B. Additions: 1. Maintenance of traffic during construction shall conform to the “Indiana Manual on Uniform Traffic Control Devices” and the City of South Bend Design and Construction Standards. 2. The attached “Traffic Closure Request” form is to be used for any lane restrictions or closures and required to be filled out and sent to the Department of Public Works. 3. The Contractor shall arrange and prosecute the work specified for this contract in such a manner that traffic on existing streets is unrestricted throughout the Project. The Engineer shall approve the method of traffic control. No construction equipment, vehicles, materials, supplies or temporary facilities shall be left unattended in the right-of-way of any street or left parked overnight without proper marking and lighting. 4. A maximum of one lane shall be closed during daylight work hours, with appropriate traffic control measures as described above. Two-way traffic must be maintained when Contractor personnel are not active or present at the site. 5. After the award of the contract and before beginning the work, the Contractor shall submit his proposed schedule of operations for the review of the Engineer. The schedule of operations as reviewed by the Engineer shall be maintained at all times. 6. This Pay Item shall include all labor, materials for the temporary installation and removal of all road closure assemblies, barricades and other signs as needed to provide adequate traffic control. 7. Work to be performed shall include furnishing all labor, services, materials, insurance and equipment to clean storm sewers, perform pond maintenance and associated work near Laurel Woods subdivision within the City of South Bend. 2. The Contractor shall preserve and protect all surrounding property, structures, tenants, visitors and their property from damage caused by the Contractor’s operations. XVIII. PLANS A. Prevailing Specifications: City of South Bend CONSTRUCTION STANDARDS AND SPECIFICATIONS B. Additions: 1. The plans consist of 13 Sheets. 2. The work shall conform to the plans. 3. The drawings are schematic in nature. 4. The CONTRACTOR is responsible for estimating dimensions and quantities of materials. 5. In the event that the Special Provisions and the Plans conflict, the Special Provisions shall govern. XIX. M/WBE-DBE GOALS A. Prevailing Specifications: City of South Bend Ordinance No. 10693-19, Minority and Women Business Enterprise Inclusion Program Plan B. Additions: 1. The M/WBE Goals for this Project are: a. MBE 1.4%. b. WBE 4.3%. 2. Complete FORM MWBE-1.0 MWBE Utilization Plan and submit with Contractor’s Quote for Public Work. 3. Complete FORM MWBE-2.0 Evidence of Good Faith Efforts and submit with Contractor’s Quote for Public Work. 4. Complete FORM MWBE-2.1 MWBE Contacted and submit with Contractor’s Quote for Public Work. XX. ELECTRONIC DATA A. Prevailing Specifications: City of South Bend Design and Construction Standards B. Additions: 1. Unless otherwise stated in the Supplementary Conditions, the data furnished by City or Engineer to Contractor that may be relied upon are limited to the printed copies (also known as hard copies). Files in electronic media format of text, data, graphics, or other types are furnished only for the convenience of the receiving party. Any conclusion or information obtained or derived from such electronic files will be at the Contractor’s sole risk. If there is a discrepancy between the electronic files and the hard copies, the hard copies govern. 2. When transferring documents in electronic media format, neither the City nor the Engineer make any representations as to long term compatibility, usability, or readability of documents resulting from the use of software application packages, operating systems, or computer hardware differing from those used in the data creation. Version 2/14/2020 Page - 9 Public Works Quote XXI. SCOPE OF WORK A. Prevailing Specifications: 2020, INDOT Standard Specifications Section 104.02 B. Additions: 1. The Bidder shall verify existing site conditions prior to Bid/Quote submission to preclude a stoppage of work due to “Differing Site Conditions”. 2. The Contractor shall take special care to protect existing facilities adjacent to the limits of the active work areas shown on the Plans. Any scope required to repair existing curb, sidewalk, pavement, or other items not indicated for removal on the Plans that are damaged by the Contractor shall be repaired by the Contractor but shall be considered incidental to the work and not paid for directly. XXII. PRECONSTRUCTION CONFERENCE A. Prevailing Specifications: None B. Additions: 1. After quote award and prior to the initiation of work at the site, the Contractor and Owner shall hold a Preconstruction Conference. 2. In addition to any other documentation required by the Owner, the selected Contractor shall submit the following information to the City prior to the Preconstruction Conference: a. Prior to the start of any Work under this project, Contractor shall make available to Owner all equipment that is to be utilized in the execution of this contract. Owner will hold a preconstruction conference at which the sequence of work, methods, inspection, and monitoring requirements and debris disposal shall be discussed. b. Designated personnel shall be field certified NASSCO (National Association of Sewer Service Companies) technicians certified by passing the three-day Pipeline Assessment and Certification Program. Contractor shall submit documentation of certifications of all staff to be assigned to the project. c. A description of the method used to prevent passage of debris to downstream sewers. d. A description of the method of disposal of sewer pipe cleaning debris. e. Equipment storage location for off-hours and weekends. f. Construction dewatering process, equipment and plan g. Documentation on any chemicals anticipated to be used on the project, including at a minimum, material safety data sheets. h. Phone numbers of the personnel used for emergency and off-hours inquiries or calls. XXIII. MOBILIZATION AND DEMOBILIZATION A. CLEARING OPERATIONS A. Prevailing Specifications: 2020, INDOT Standard Specifications Section 201 B. Additions: 1. “Clearing and Grubbing” shall consist of clearing, grubbing, removing and disposing of all vegetation, debris and other man-made materials required to perform the work indicated in the Plans and Contract Documents, but not specifically paid for otherwise. 2. This Pay Item shall include the removal of all trees and grinding of stumps within the perimeter of the existing fenced pond area which are not otherwise designated on the Plans to be preserved. 3. Disposal of debris resulting from clearing or removal operations shall be at an off-site area, provided by the Contractor and approved by the Owner. No debris shall be left overnight. The Contractor shall take diligent care to protect the public, the workers, streets, sidewalks, and adjacent public and private property from the hazards of any tree removal and stump removal operations. 5. Payment will be made under: Pay Item Pay Unit Clearing and Grubbing LS XXV. EARTHWORK A. Prevailing Specifications: 2020, INDOT Standard Specification Section 203, City Std. Spec. Section 7 B. Additions: 1. This Pay Item shall include the earthwork needed to excavate, haul and dispose of all material not being removed under some other Pay Item which is encountered within the limits of the work to complete the Project to the proposed finished grades indicated on the Plans. 2. Excavation and disposal of silt and debris deposited in the work area shall not be measured directly but shall be included in the unclassified excavation Pay Item. 3. All material excavated within the perimeter of the fence surrounding the existing pond shall be considered unclassified excavation, without consideration of material consistency or water content. 4. Proposed grades shown on the Plans within the fenced pond area are to be verified by the Contractor as finished grades after the sewer cleaning operation and topsoil placement is complete. The fenced pond area may be used as a temporary area of debris collection from sewer cleaning operations, but the addition of water or debris to the pond area shall not entitle the Contractor to additional payment for earthwork operations. 5. Payment will be made under: Pay Item Pay Unit Excavation, Unclassified LS XXVI. STORMWATER MANAGEMENT A. Prevailing Specifications: 2020, INDOT Standard Specifications Section 205, City Std. Spec. Section 7, City Std Drawings 7-1 and 7-7. B. Additions: 1. Erosion Control Blanket (ECB) shall be provided at the locations shown on the Plans after the finished grade has been topsoiled and seeded. 2. ECB shall be North American Green (NAG) SC-150, or approved equivalent. 3. ECB shall be securely anchored to the ground using staples or Engineer approved equivalent method at intervals as recommended by the manufacturer. 4. Cost of all labor and materials, including required ECB anchoring mechanisms shall be included in the cost of the respective Pay Items. 5. Geotextile fabric below the Temporary Construction Entrance shall not be measured or paid for directly but shall be included in the cost of the No. 2 Stone. 6. A 10-foot width shall be allowed for the project. 7. Prevailing Specifications: 603 B. Additions: 1. This Pay Item shall include all labor and materials to remove portions of existing chain link fence section and posts to provide access to existing pond area to be excavated and the resetting and restoration of the fence along the current alignment. 2. Existing fence section is approximately six-foot of chain link fabric, with three additional strands of barbed wire above the fabric. 3. Replacement fabric and posts shall match the existing fence material as close as practical. Barbed wire sections removed for project access shall be neatly terminated at the nearest existing post outside of the limits to be reset and not replaced within the limits of fence to be reset. 4. Payment will be made under: Pay Item Pay Unit Fence, Chain Link, Reset LFT XXVIII. SEEDING A. Prevailing Specifications: 2020, INDOT Standard Specifications Section 621, 914.01, 914.03, 914.04 B. Additions: 1. Fertilizer shall be applied at a rate of 800 pounds per acre. Fertilizer shall not be paid for separately, but shall be considered incidental to the seeding operations on the Project. 2. Permanent seeding shall be INDOT seed mixture, Type U. 3. Topsoil shall be placed below the areas to be seeded and shall consist of natural loam, sandy loam, silty loam, silty clay loam, or clay loam humus-bearing soils adapted to the sustenance of plant life and such topsoil shall be neither excessively acidic nor excessively alkaline in accordance with INDOT Standard Specification 914.01. Topsoil shall be screened such that 100 percent of the topsoil passes the one 1” sieve and 95 percent passes the No. 10 (2.00 mm) sieve. Topsoil shall be a minimum of 6” deep. Topsoil shall not be compacted by heavy equipment. 4. Topsoil obtained from within the limits of construction shall not be paid for directly but shall be considered incidental to the project. Topsoil obtained from outside the project limits which has been tested and documented shall be paid for on a volumetric basis. 5. Payment will be made under: Pay Item Pay Unit Permanent Seeding, Type U SYS Topsoil CYS XXIX. CASTING REPLACEMENT A. Prevailing Specifications: 2020, INDOT Standard Specifications Section 720 B. Additions: 1. This Pay Item shall include the removal and disposal of existing castings shown on the Plans to be replaced, as well as the materials and labor for the new replacement casting. 2. Identifying marks were not observed on the existing casting. The approximate equivalent grate casting may be R-2561-A, by Neenah Foundry. 3. The Contractor shall verify the dimensions of the existing casting prior to removal, ordering or fabrication of the replacement casting. The new casting shall be of equal or greater materials than the existing casting and shall be of the same existing dimensions confirmed by the Contractor. 4. The Contractor shall submit a Shop Drawing of the proposed replacement casting for the project. 5. The Owner does not object to the reuse of the existing frame, if a suitable replacement grate can be utilized that conforms to the dimensions of the existing frame. Otherwise, the Contractor shall Version 2/14/2020 Page - 12 Public Works Quote replace both the frame and grate as part of the casting replacement, including incidental work to remove and replace the existing frame. 6. Payment will be made under: Pay Item Pay Unit Casting, Replace EACH XXX. STORM DRAIN CLEANING AND INSPECTION A. Prevailing Specifications: None B. Additions: 1. The work shall include all labor, tools, equipment, and related items as may be required for the complete cleaning or cleaning and internal inspection and television monitoring (including bypass plumbing, drain dewatering and associated work) of the storm drain and inlet/manhole structures and removing and disposing of all deposits cleaned from the drains. 2. All storm drains shall be thoroughly cleaned and allow for unrestricted inspection by closed circuit television. All storm drains will be cleared prior to television inspection. If the television inspection shows drains were improperly cleaned, the Engineer will direct the appropriate cleaning and re- cleaning. Re-cleaning shall be done at no additional cost. 3. Equipment a. Cleaning shall be accomplished by means of water jetting or other approved methods. The equipment utilized shall be capable of removing all sand, dirt, rocks and other debris, including potential roots, from the drain line to allow internal inspection of all internal surfaces. The equipment used shall suit the conditions and size of the sewer to be cleaned. The equipment used shall include the use of a movable dam type and be constructed in such a way that a portion of the dam may be collapsed at any time during the cleaning operation to protect against flooding of the sewer. The movable dam shall be equal in diameter to the pipe being cleaned and shall provide a flexible scraper around the outer periphery to insure removal of grease. If sewer cleaning balls or other equipment, which cannot be collapsed, is used, special precautions to prevent flooding of the sewers and public or private property shall be taken. b. The camera and recording equipment shall be specifically designed for continuous viewing and recording of detailed images of the interior wall of pipes and transitions of the specified size ranges. The equipment shall include sufficient lighting to view the entire periphery of the pipe. The equipment shall have appropriate attachments to maintain a position in the center of the pipe and an electronic counter to continuously record the location of the equipment in the pipe. A copy of the video inspection shall be submitted to the Owner. c. Cleaning shall be performed within the seventy-two (72) hour period immediately before closed circuit television inspection. 4. Sewer Cleaning Operation a. The Contractor shall thoroughly clean all sewer segments designated for cleaning in the project plans. Cleaning includes the removal of all debris in the sewer including, but not limited to, items such as silt, sand, tree roots, rocks, bricks, pieces of pipe, buildups on the pipe walls, grit, sludge, and slime and all other foreign material in the sewer pipe. A clean pipe segment is generally defined as a pipe that has approximately 95 percent of the debris removed for the full length of pipe. The Contractor shall provide adequate documentation that the pipe segments have been thoroughly cleaned. The Owner will make the final decision as to whether a pipe has been thoroughly cleaned according to these Specifications. b. The Contractor shall clean the sewers by moving debris within the pipe from the upstream manhole to downstream manhole in a given sewer segment. Debris from the cleaning operation shall be collected at the downstream manhole with the use of the vacuum unit on the cleaning equipment. The Contractor shall not pass debris to the next downstream sewer segment. Under no circumstances shall the Contractor allow debris to flow downstream into the receiving channel at the outlet of the sewer system. c. If cleaning of an entire section of sewer from manhole to manhole cannot be successfully performed from upstream manhole; the equipment shall be set up on the opposing end and the sewer shall be cleaned again from this “reverse set-up” configuration. 5. Water. Version 2/14/2020 Page - 13 Public Works Quote a. Any and all Owner water used by Contractor shall be from a metered supply with an approved backflow device to protect the water supply. Water used to clean the sewer lines may be obtained from South Bend Water Works. The Contractor should contact South Bend Water Works to determine if there will be any charges for using their water. If there are charges from South Bend Water Works, the Contractor shall pay all costs associated with coordinating and obtaining water for the sewer cleaning operations. All metered water supply shall be paid for to OWNER through the regular billing system. b. It shall be Contractor’s responsibility to obtain approval from the South Bend Water Works to use Owner’s fire hydrants. c. Contractor shall be responsible for preventing contamination of the potable water system. d. Contractor when drawing water from any public hydrant shall use a backflow preventer and/or an eighteen (18) inch air gap, or other City-approved back-flow device. e. Contractor shall remove the water meter(s)/piping etc. from all fire hydrants at the end of each working day. f. Any costs of water shall be considered incidental to the Contract. 6. Disposal of Materials a. All materials removed from the sewer lines during cleaning operations shall be trapped and removed from the system at the downstream manhole of the section being cleaned. Passing of debris from upstream manhole section to beyond downstream manhole section will not be allowed. All materials shall be disposed of in compliance with all applicable laws and regulations. b. A suitable dam or weir shall be placed in the downstream manhole to trap all such materials. Passing material from manhole section to manhole section, which could cause line stoppages, accumulations of sand in wet wells, or damage pumping equipment, shall not be permitted. c. All solids or semi-solids resulting from the cleaning operations shall be removed from the site at the end of each workday, hauled to and disposed of in a properly licensed sanitary landfill. Contractor will pay all landfill-tipping fees, if applicable, as an incidental cost to the Work. d. Under no circumstances shall sewage or solids removed in the cleaning process be dumped onto streets or into ditches, catch basins, storm drains, sanitary sewer manholes, cleanouts, or dumps. e. The Contractor shall keep haul route and work area (s) neat and clean and reasonably free of odor, and shall bear all responsibility for the cleanup of any spill which occurs during the transport of cleaning/surface preparation by-products and the cleanup of any such material which is authorized by or pursuant to this Contract and in accord with applicable law and regulations. The Contractor shall immediately clean up any such spill, or waste. If the Contractor fails to clean up such spill, or waste immediately, the Owner shall have the right to clean up or arrange for its cleanup and shall charge to the Contractor all costs, including administrative costs and overhead, incurred by the Owner in connection with such cleanup. The Owner shall also charge to the Contractor any costs incurred or penalties imposed on the City as a result of any spill, dump or discard. Under no circumstances is this material to be discharged into the waterways or any place other than where authorized to do so by the appropriate authority. f. The general requirements for vehicles hauling such waste material are as follows: Transport vehicles must be of type(s) approved for this application by the jurisdictions involved. General requirements are that the vehicles have watertight bodies, that they be properly equipped and fitted with seals and covers to prohibit material spillage or drainage, and that they be cleaned as often as is necessary to prevent deposit of material on roadways. Vehicles must be loaded within legal weight limits and operated safely within all traffic speed regulations. g. The routes used by the Contractor for the conveyance of this material on a regular basis shall be subject to approval by the governing authority having jurisdiction over such routes. 7. Sewer Inspection and Televising a. The Contractor shall furnish all equipment for video recording. b. All inspections shall follow the Pipeline Assessment and Certification Program (PACP) protocol, as established by the National Association of Sewer Service Companies (NASSCO). c. The inspection will be done one (1) reach at a time. The reach being inspected will be suitably isolated from the remainder of the sewer system by restricting all upstream and groundwater flows to allow maximum exposure of the pipe being inspected. Version 2/14/2020 Page - 14 Public Works Quote d. Should the Contractor’s internal inspection equipment become immobilized within a storm drain, said equipment shall be removed from the line. If excavation is required to retrieve the Contractor’s equipment, the excavation shall be accomplished by the Contractor at his expense. Following removal of the equipment, the Contractor shall restore the storm drain and the site to conditions consistent with new pipe work in accordance with City specifications at no additional cost to the Owner. e. The Contractor shall provide the City with an exported file with the television data on an external hard drive with a USB 2.0 connection, at the preference of the Owner. All hard drives submitted shall become property of the City. Each video shall be named according to the “Pipeline ID” format. The video shall document in a clear and continuous manner the condition of the sewer segment from the upstream manhole to the downstream manhole. No interruptions or missing segments shall be allowed unless noted in writing by the Engineer. The City will reject any video that has foggy or static conditions on it. f. The date of the television inspection and the distance that the camera has traveled through a particular storm drain reach shall be continuously displayed on the recording. All recordings obtained during the work shall be turned over to the Engineer and shall become the property of the Owner. All costs for recordings shall be included in the Unit Prices for the Work. If the recording is not complete or the quality is not satisfactory, the storm drain shall be reinspected at the Contractor’s expense. g. The television video supplied by the Contractor shall be compatible with existing replay equipment owned by the City. h. Recordings and inspection reports shall be submitted to the City Engineer within seven (7) days of inspection. 8. Coordination with Televising Operations a. The Contractor is allowed to operate televising equipment simultaneously with the cleaning equipment to check on the progress of the current cleaning operation in a sewer segment. However, in these situations, the use of televising equipment shall be incidental to the sewer cleaning operation and will not be measured or paid for. 9. Acceptance of Cleaning Operation a. Acceptance of storm sewer line cleaning shall be made upon the successful completion of the television inspection and shall be to the satisfaction of the Engineer. If television inspection shows the cleaning to be unsatisfactory, the Contractor shall be required to reclean and reinspect the storm sewer line until the cleaning is shown to be satisfactory. If there are areas where television inspection is not able to be performed, the Engineer may require the Contractor to pull a double squeegee (with each squeegee the same diameter as the storm sewer) through each manhole section as evidence of adequate cleaning. b. In addition, on all storm sewer lines which have sags or dips, to an extent that the television camera lens becomes submerged for three (3) or more lineal feet during the television inspection, the Contractor shall pull double squeegee and/or sponges through the line in order to remove the water from the dips or sags. Water removal through squeegees and/or sponges shall be performed until the television camera lens will no longer be submerged. This requirement may be waived by the Engineer if the water in which the camera lens is submerged, is clear enough to allow the identification of pipe defects, cracks, holes and location of service taps. c. The basis for non-acceptance by the Engineer shall be any evidence of poor cleaning observed on the video tape recording that prevents the proper internal inspection of the drain reach. No additional payment shall be made for the re-cleaning of drain reaches to an acceptable condition regardless of the number of times re-cleaning may be required. All measurements for payment shall be based upon the linear feet of pipe cleaning and television inspection. 10. Damaged Pipe Sections a. The Owner anticipates the possibility that some sewers will have broken or collapsed sections that prevent cleaning and televising equipment access. When these sections are encountered, the Owner will make a decision whether to delete the entire section from the Contract or obtain quotes for a dig repair with the current or other Contractor. b. If the decision involves a dig repair, the sewer cleaning crew may clean and televise another sewer segment indicated on the plans. Once the dig repair has been completed, the sewer cleaning Contractor will return to the repaired sewer section and complete the cleaning and televising operations. Version 2/14/2020 Page - 15 Public Works Quote c. The Owner has the authority to delete a sewer segment from the project if the sewer is in such condition that normal cleaning would damage the pipe. d. The Contractor shall only use equipment correctly sized for the diameter of the sewer segment being cleaned, and shall have on the project an adequate number of consumable parts with the appropriate diameters to minimize disruptions related to replacement of worn or broken equipment. 11. Manhole & Inlet Cleaning a. All manholes and inlets encountered shall be thoroughly cleaned of all foreign materials, obstacles, and buildups. This shall include, but is not limited, to removing grit, sand, silt, leaves, debris, rocks, bricks, sticks, and sludge from the manhole or inlet. b. The intent of the project is to clean the manholes and inlets simultaneously with the storm sewer cleaning and televising operation. c. This work is considered incidental to the cleaning of the storm sewers. It will not be paid for on a separate basis. 12. Storm Drain Dewatering a. When sewer flow depth is greater than 25 percent, flow depth must be decreased by plugging and/or bypass pumping. Plugs shall be designed to pass any desired portion of sewage flow. If bypass pumping is required for sewer cleaning operations, Contractor shall provide all necessary equipment, manpower, and expertise. Contractor is responsible for all damage to public or private property resulting from these operations. Bypass pumping shall require proper filtration prior to discharge release. b. For systems with submerged or partially submerged outfall(s) the Contractor shall plug the system, where and as needed, to allow the cleaning or cleaning and Television Inspection to proceed. c. During the television inspection process, every effort shall be exerted to obtain a full view of the pipe interior. For instances where the camera lens becomes submerged or where a large portion of the pipe shall contain water, and these conditions persist for significant portions of the sewer being inspected, the Contractor shall dewater the pipe. d. Dewatering can be accomplished with a pump and discharge hose or by the nozzle of a hydraulic high-pressure jet machine. If the jet machine is used, it shall precede the television camera through the sewer pipe. The nozzle of the jet machine shall work in conjunction with the television camera’s motion and be positioned so that several feet of pipe length can be viewed by the camera. The dewatering procedure shall move standing or ponded water through the storm drain to a point within the storm drain downstream of the reach being inspected. e. Due to anticipated high ground water elevations, it is anticipated that the groundwater water levels at the existing pond area will need to be depressed in addition to the normal dewatering measures associated with sewer cleaning outlined above. f. Dewatering of pipes are considered part of the Construction Dewatering Pay Item. 13. Project Cleanup a. The Contractor shall leave all cleaning and televising sites neat and clean. The Contractor shall not allow trash and debris to accumulate during the cleaning and televising period and shall remove all trash and debris before final acceptance will be granted. b. Unless otherwise approved, the Contractor will not be allowed to store equipment or materials or debris on the City right-of-way, City property, or on private property. c. Above ground surfaces along the line of the maintenance work shall be restored to conditions equal to or better than those found at the start of the work. d. Project cleanup shall not be measured for payment. Cleanup shall be considered incidental and included in the respective contract unit prices. 14. Payment a. Contractor’s compensation for sewer cleaning will be calculated together with storm sewer inspection and televising by multiplying the number of linear feet of storm sewer cleaned and televised times the unit price per linear foot for Clean Storm Sewer and Televise Storm Sewer and shall be based on diameter of the pipe as indicated in the quote. All of the Contractor’s costs for labor, equipment, materials and incidentals necessary for this work shall be considered included by the Contractor in said proposed unit prices. The cost of the sewer televising work shall include all information supplied to the City such as external hard drives, hardcopy reports, electronic files, time spent converting data formats and other Version 2/14/2020 Page - 16 Public Works Quote reporting information shall be included in the cost of Sewer Televising and no additional compensation will be allowed. b. Payment will be made under: Pay Item Pay Unit Clean Storm Sewer, 12”-24” Dia LFT Televise Storm Sewer, 12”-24” Dia LFT XXXI. CONSTRUCTION DEWATERING A. Prevailing Specifications: None B. Additions: 1. The work consists of providing all labor, equipment, including pumps, and materials necessary to dewater the site to maintain suitable working conditions. 2. The Contractor shall design, install, and maintain a construction dewatering system with adequate pumping capacity to lower and maintain a maximum free water surface elevation of 693.0 ft across the existing fenced pond area, on the same vertical datum benchmarks shown on the Plans. This measurement shall be the standard measurement for suitable Construction Dewatering conditions at the existing detention pond area. 3. The equipment used to dewater the pond area shall include the use of a movable dam downstream of the existing detention pond and shall be constructed in such a way that a portion of the dam may be collapsed at any time during the cleaning operation to protect against flooding of the sewer. If sewer cleaning balls or other equipment, which cannot be collapsed, is used, special precautions to prevent flooding of the sewers and public or private property shall be taken. The use of a suitable temporary dam within the storm sewer downstream of the existing pond is intended to prevent backflow of groundwater and Kiefer Ditch into the pond area during dewatering operations. 4. Construction dewatering measures shall be in place and active during all sewer cleaning and video operations, and during all earthwork operations within the existing pond area. 5. Adequate construction dewatering operations shall be in place during the project punch list walkthrough with the Owner. 6. By submitting a Bid/Quote, the Contractor is certifying that high groundwater elevations, on the order of elevation 695.0 feet, are an anticipated component of the project which have been considered in the Construction Dewatering component of the Bid/Quote. As shown on the Plans, this groundwater elevation may be above the crown of certain pipes within the Work area and above the ground surface within the existing fenced pond area. 7. All work required for maintaining the project work site in suitable working conditions is included in this Pay Item. 8. Dewatering measures for pipe cleaning operations, such as removal of water and debris from storm lines is to be included in this Pay Item. 9. The acceptable outlet for the Construction Dewatering process from the pond is shown on the Plans. The outlet for temporary pipe dewatering operations is the back to the pond or directly to a vacuum truck operated by the Contractor. 10. The dewatering Plan to be submitted by the Contractor shall include and indicate the use of water filtering and stabilized outlets as part of this Pay Item. Direct release of dewatering discharge lines without the use of a filter is prohibited. 11. Release of discharge from the Construction Dewatering process at the acceptable outlet shall be accomplished by the Contractor at a rate that limits erosion by the use of a stabilized outlet, such as temporary rock filter berm downstream of the filter bags. 12. The Contractor shall construct a rock filter berm near the outlet, as a secondary containment Best Management Practice. The approximate height of the rock filter berm shall be approximately one-foot high by four feet wide by 20 feet long. 13. The Contractor shall place the filter bags in a location where it can be removed efficiently without causing damage or losing sediment but no closer than 20 feet from the edge of pavement along Auten Road. 15. Filter bags are to be replaced when they become laden with sediment, or exhibit leakage or other failure. 16. The Owner has performed geotechnical soil borings within the general vicinity of the Project for purposes other than the current work. 17. Any geotechnical soil boring results provided on the Plans, and the associated geotechnical report, are provided to the Contractor for information only as a general indication groundwater levels at those boring locations and the types of soils anticipated to be encountered in the area during Construction Dewatering activities. 18. It is anticipated that a gravity pump system will be installed by the Contractor. Other pumping configurations, such as a wellpoint system, are allowed on the project. The Contractor is notified that the soil boring logs indicate the presence of silt and clay layers at depths that might impact the design and implementation of the dewatering system. 19. The Contractor is encouraged to perform independent field investigations of the existing conditions at the pond area to assess the method and level of effort associated with Construction Dewatering prior to providing a quote or bid. The Contractor shall coordinate with the Owner to access the existing pond area if such independent field investigations are desired. 20. The new storm sewer pipe is anticipated to be established at a maximum depth of about 5 feet below the existing ground surface. Other details of the storm sewer such as the pipe diameter and outlet structure are unknown. Given the type of project, we anticipate that the sewer will be constructed using cut and cover techniques. In addition, dry wells are being considered for infiltration of storm water collected from the drainage area. No details of dry wells are known at this time. In general, the subsurface soil profile consisted of granular-type soil to a depth of about 5½ to 11 feet below the ground surface and underlain by silt to a depth of about 9 to 22 feet where sandy lean clay was encountered to the terminal depth of the borings. Given that the storm sewer pipe and associated structures are not anticipated to increase the stresses in the soil above that from the existing structure and overburden, bearing capacity is not a concern. However, to provide adequate support for the proposed sewer elements and maintain line and grade, proper subgrade preparation is critical. For proper preparation of the subgrade, we recommend a program of compaction once the subgrade has been established. Given the presence of existing utilities below the subgrade, we recommend that extreme caution be used in the compaction process in these areas. If dry wells are considered, it is likely the depth of the dry well will be limited to the rather high groundwater level and the presence of more cohesive soil and silt, both of which will restrict infiltration. Once the sewer elements have been set, we recommend that the backfill be placed in an engineering manner in thin lifts and compaction test control. We also anticipated that the road will be re-established at the approximate same line and grade as the existing with a minimum acceptable pavement section. A detailed discussion of design parameters and construction considerations is included in subsequent sections of this report. Weaver Consultants Group 2 2 PROJECT INFORMATION 2.1 Project Description and Location We understand that the city of South Bend is considering the construction of a new storm water drain beginning near the intersection of Lindenwood Drive West and Laurel Wood Drive North and ending at Laurel Drive and Auten Road in South Bend, Indiana. The location of the project is shown on a Site Location Plan (Figure 1) in the Appendices. Other details of the storm sewer such as the pipe diameter and outlet structure are unknown. Given the type of project, we anticipate that the sewer will be constructed using cut and cover techniques. We also understand that dry wells are being considered to manage the collected runoff. Details such as size, location and dep th of the dry wells is unknown. For design of the proposed storm sewer, a subsurface exploration program consisting of four test borings was performed. The number, depth and general locations of the soil borings were chosen by Weaver Consultants Group (WCG) based on the project plan provided by McCormick Engineering, LLC. The boring plan was to perform the borings to a depth of 15 feet below the existing ground surface. However, due to the subsurface conditions, two of the four boring were extended to a deep as 25 feet below the ground surface (bgs). The borings were located, in the field, by a representative of WCG referencing existing site features. The approximate locations of the borings are provided on a Boring Location Plan (Figure 2) in the Appendices. 2.2 Site Description The road and ground surface along the planned alignment are relatively flat. A ditch which increases in depth from east to west is located along the south side of Auten Road. At the time of our field activities, there was no water in the ditch. Based on the utility location markings provided by others, numerous below grade utilities including a high-pressure gas main, fiber optic lines, water mains, and others exist along the alignment. Weaver Consultants Group 3 3 SUBSURFACE CONDITIONS 3.1 Subsurface Conditions Our interpretation of the subsurface conditions is based four test borings. The following discussion is general; for more specific information, refer to the boring logs presented in the boring logs in Appendix A. Surface conditions: The surface conditions along the storm sewer alignment consisted of a variety of conditions including asphaltic paved roads and grass covered areas. At three of the boring locations topsoil depths ranged from 6 to 10 inches. At the location of Boring B -3, road gravel was encountered to a depth of about 8 inches. Subsurface Conditions: In general, below the surficial materials, the subsurface soil profile consisted of granular-type soil to a depth of about 5½ to 11 feet bgs and underlain by silt to a depth of about 9 to 22 feet where sandy lean clay was encountered to the terminal depth of the borings. The upper approximately 11 feet of the granular soil was fill consisting of clayey sand with varying amounts of gravel (SC). Below the fill was naturally occurring fine sand (SP). At a depth of about 13 feet, an approximately 5-foot thick stratum of sandy clay (CL) was encountered. The underlying clay stratum was a poorly-graded sand with a trace of gravel (SP) to the maximum depth explored. The underlying soil layers in the borings are described in more detail below. • Granular soils (SP, SP-SM, SP-SC, SC) – Below the surface materials, granular soils consisting of fine to coarse sand with a trace to little gravel (SP), fine to coarse sand with silt (SP-SM), clayey fine to coarse sand (SC), and fine to coarse sand with clay (SP-SC) were encountered to depths ranging from 5½ to 11½ feet bgs. The relative density of the granular soil was very loose to medium dense with Standard Penetration Test (SPT) N- values ranging from 0 to 14 blows per foot (bpf), generally decreasing with depth. • Silt (ML), and Clayey Silt (CL-ML) – Below the granular soils, silt (ML) was encountered to the terminal depth of the borings or 9 to 22 feet bgs. The silt was typically no t cohesive and contained random amounts of gravel. The relative density of the silt was loose to medium dense with Standard Penetration Test (SPT) N-values ranging from 5 and 13 bpf, typically in the 5 to 8 bpf range. At locations within the silt stratum, thin layers of clayey silt (CL-ML) was encountered. Where encountered, the consistency of the clayey silt was very stiff with a hand penetrometer reading of 2.7, and a moisture content of about 29 percent. • Sandy Clay (CL) – At the deeper boring locations, a stratum of sandy clay with a trace of gravel was encountered to the terminal depth of the borings. The consistency of the clay Weaver Consultants Group 4 was stiff to very stiff based on hand penetrometer readings ranging from 1.4 to 3 tons per square feet (tsf), and moisture contents ranging from 10 to 13 percent. 3.2 Groundwater Conditions Groundwater level observations noted during and after completion of the drilling operations are recorded on the boring logs. Based on these observations, groundwater was encountered at depths about 5 to 5½ feet bgs. Given the presence of near surface granular soil underlain by less permeable silt, it is our opinion that the groundwater is perched on the silt stratum. Sand deposits are relatively pervious, and water can drain into the borehole somewhat quickly and come to an equilibrium position. However, fluctuations in the water table should be anticipated througho ut the year with variations in precipitation and other environmental or physical factors. Seasonal fluctuations in the groundwater level should be expected due to variations in precipitation, evaporation and surface water runoff. Weaver Consultants Group 5 4 DESIGN RECOMMENDATIONS 4.1 Basis Our recommendations for the proposed storm sewer are based on data presented in this report, which includes four test borings along the proposed alignment. Subsurface variations can exist at a site which may not be indicated by such a dispersed and limited boring program, particularly given the presence of existing below-grade utilities and expected associated fill soil. If such variations or unexpected conditions are encountered during construction, or if the project information discussed herein is incorrect or changed, we should be informed immediately since the validity of our recommendations may be affected. 4.2 Storm Sewer Recommendations Prior to excavating trenches for piping in those areas where cut-and-cover techniques will be used, we recommend that all underground utilities within the project limits be relocated or temporarily removed where necessary. Following relocation/removal of utilities, excavation for the piping may then proceed. Based on our observations of the subsurface conditions, we anticipate that excavations for the system will typically expose very loose to medium dense granular-type soils. Considering the granular nature of the soils, it is our opinion that the excavated granular soils may be re-used as backfill. Granular soils (i.e., soils containing less than 12 percent fines) which meet the requirements discussed in Section 5.3 are considered conducive for pipe support and fill placement. To reduce the risk of settlement of the backfill, we recommend that the bedding material and soils used for structural backfill surrounding the pipe be mechanically compacted to a minimum of 95 percent of the standard Proctor density (ASTM D 698). Where the storm sewer is beneath roadways, we recommend the fill be compacted to 98 percent of Standard Proctor density. Additionally, periodic field density tests performed by WCG are recommended during fill placement to determine the adequacy of compactive effort. Considering we anticipate that granular-type soil will be encountered at the pipe inverts, a separate bedding material may not be required. The borings did note the occasional presence of cobbles near the probable sewer depths. Cobbles should be relocated away from the installed sewer as to not create hard points on the pipe. Granular backfill placed around the pipe in uniform layers, not exceeding 6 in. in loose lift thickness, is recommended. We also recommend that the backfill be placed and hand compacted in layers simultaneously on each side of the pipe. In our opinion, compaction equipment should operate parallel to the axis of the pipe and over-compaction may cause movement of the pipe prior to being seated. Therefore, caution should be exercised during this initial phase of pipe placement. Note that the pipe manufacturer may provide backfill requirements/procedures pertaining to their particular pipe. If this is the case, then the manufacturer's specifications should be adopted. Weaver Consultants Group 6 4.3 Structure Foundation Considerations In general, the soil conditions, as previously described, appear to be adequate for the support of manhole or outlet structures. An exception to this is in the vicinity of Boring B-1 where the granular soils beneath a depth of about 5 feet are very loose. For structures in this area, we recommend the very loose soil be removed to a depth of 2 feet below the structure bottom and replace with compacted coarse aggregate meeting the general gradation requirements for Indiana Department of Transportation (INDOT) No. 8 coarse aggregate. Based on these conditions, we recommend that the base be proportioned using a net allowable soil bearing pressure of 2,000 psf. In our opinion, the subgrade should be compacted to densify the soils loosened by the excavation process to minimize the risk of excessive settlement. Since groundwater is likely to be below the base of the structures, the effects of buoyancy are not likely to be a consideration. 4.4 Dry Wells We understand that dry wells are being considered to provide infiltration of the collected runoff into the below grade stratum. Groundwater was generally encountered at a depth of about 5 feet below the ground surface. The soil profile above the groundwater was somewhat mixed with fine to coarse sand and fine to coarse sand with silt. These soils are generally conducive to infiltration of water. Below the water table the soil profile was even more mixed with some locations having sand (SP), silty sand (SM) and clayey sand (SC) underlain by silt (ML). The silty sand, clayey sand, and silt are far less permeable than the sand soils and infiltration will within these zones will be slow and limited. In addition to the infiltration issue, excavation below the groundwater level will require dewatering and shoring to prevent soil collapse of the excavation sidewalls. Two soil particle size distribution tests were performed on representative samples of the sand (SP) type soil. Results of these test were used to estimate the soil infiltration rate. The estimated infiltration rates based on the Hazen Formula are presented in Table 1. Table 1 – Summary of Infiltration Estimates Location Depth (ft.) Infiltration Rate (in./hr.) Weaver Consultants Group 7 4.5 Pavement Considerations The pavement subgrade is anticipated to consist of “B” Borrow type soil or crushed stone placed to improve the subgrade conditions or structure backfill placed during backfill of the sewer. Provided the subgrade is prepared in accordance with the recommendations provided herein, these conditions are anticipated to be suitable for support of the pavement. We recommend the following parameters be considered for pavement design, if necessary. As we anticipate a minimum of 2 feet of soil will require improvement, we have recommended a Type I subgrade treatment in accordance with INDOT Standard Specifications Section 207.04. Table 2 below provides a summary of the pavement design parameters. Weaver Consultants Group 8 5 CONSTRUCTION CONSIDERATIONS 5.1 Site Preparation The construction of the sewer is substantially an excavation type project. No placement of fill for support of the structure is anticipated for the project. Site preparation is essentially excavation to the sewer invert. Once the new structure subgrade has been established, we recommend the subgrade soils be densified via several passes of a high-energy vibratory compactor. Where the subgrade crosses existing utilities, consideration of the use of light-weight compactor so as not to damage the utility. Where backfill is required around the new pipe and structures, we recommend that it consist of granular structural fill. The structural fill should meet the requirements of Section 5.3 and be placed in accordance with Section 5.4. 5.2 Utility/Storm Water Structure Excavations Excavations should be sloped, shielded or shored in accordance with the current Occupational Safety and Health Administration (OSHA) requirements (see Section 5.7). Once the necessary subgrade level is reached, no addition subgrade preparation is recommended. 5.3 Structural Fill Structural fill, defined as any fill which will support structural loads, should be free of organic material, have a plasticity index of less than 25 percent, a maximum particle size of no more than 3 in., and a maximum dry density in excess of 100 pounds per cubic foot [pcf, as determined by the standard Proctor compaction test (ASTM D698)]. In addition, structural fill should not be frost susceptible. The structural fill should be compacted to at least 9 8 percent of its maximum standard Proctor dry density (ASTM D698) under the foundations and roadways. The near surface soils are considered suitable for use as structural fill. 5.4 Fill Placement Control To achieve the recommended compaction of structural fill, we recommend that the fill be placed and compacted in layers not exceeding 8 inches in loose lift thickness. To observe compliance with the recommended density standards, we recommend that in -place density tests be performed at a frequency of at least one test for every 100 square feet of pipe length per each lift of compacted fill placed in the proposed construction areas. 5.5 Construction Observations We recommend that all pavement and structure subgrades be observed by a qualified Geotechnical Engineer or his representative prior to placement. These observations are to evaluate that the exposed soils are consistent with those encountered in the borings and to check that the materials are of uniform consistency and adequate density. Weaver Consultants Group 9 5.6 Groundwater Concerns Depending on the depth of the sewer and structures, dewatering may be required to remove and replace very loose soil conditions or improve subgrades prior to placement of pipe . However, whenever a rain event occurs during construction, water flow in the creek can be expected and construction activities should be managed to account for these events and protect completed work. If minor dewatering is required in silty areas, it can be accomplished by pumping from sumps installed at the bottoms of the excavations. 5.7 Excavation Slope Stability Our exploration did not include a detailed analysis of slope stability for any temporary excavation condition. Based on the soil conditions encountered at the boring locations, temporary shallow construction excavations are expected to expose fine sandy soils. For such conditions, it is our opinion that shallow temporary excavations can be cut with side slopes of 1.5H:1V. However, current OSHA standards must be met and may be more restrictive. Hence, if safe side slopes cannot be maintained due to loose granular soil conditions, then the excavation sides should be flattened, shielded or shored in accordance with current OSHA standards. The Contractor is solely responsible for constructing and maintaining stable excavations. Additionally, soil should not be stockpiled immediately adjacent to the top of the excavation. 5.8 Limitations WCG has prepared this report in accordance with generally accepted geotechnical engineerin g practices to aid in the evaluation of the site subsurface soils. No other warranty, expressed or implied, is made. The scope of this report is limited to the specific project and location described herein, and our description of this project represents our understanding of the project. The geotechnical engineering analysis and foundation recommendations presented herein were developed based on the information obtained during the subsurface investigation. It should be noted that the borehole data reflects the subsurface conditions only at the specific locations designated on the borehole logs, and that soil and groundwater conditions could vary widely throughout the site. If variations do appear during construction activities, it may become necessary to re-evaluate the recommendations of this report. We wish to point out that the soils actually recovered from our borings for observation and  testing represent a very small percentage of the site soils.  Our records depict subsurface  conditions only at specific locations and at the particular time when drilling.  Soil conditions at  other locations may differ from conditions occurring at these boring locations.  The passage of  time may result in a change in the subsurface soil and groundwater conditions at the boring  locations.  The interface between differing subsurface materials on the logs and profiles  represent approximate boundaries.  The transition between materials may be gradual.  Also,  thin strata that occur between sample depths may be present, but remain undetected by  routine sampling procedures.  Drilling Procedures  Soil borings were performed at the approximate locations shown on the attached boring plan.   The soil borings were advanced by mechanically twisting a continuous steel‐flight, hollow‐stem  auger into the soil.  The inside diameter (I.D.) of the hollow‐stem auger is typically 3‐¼ in.  (sometimes a 6‐in. I.D. auger is used, particularly when installing 4‐in. diameter monitoring  wells).  The auger is turned into the ground, which displaces the soil upwards as it advances.  Once the  desired sample depth is achieved, the advancement of the hollow‐stem auger is stopped.  The  hollow‐stem is then cleaned of any soil and the sampling tools are inserted, and the sampling is  performed.  When drilling below the water table in pervious soils, a head of water is maintained  in the hollow‐stem, to prevent a "quick" condition at the auger tip.  Penetration Testing and Split‐Barrel Sampling  Standard Penetration Testing and split‐barrel sampling are normally conducted in the borings to  provide relative density information and soil samples for visual classification and laboratory  testing.  The standard split‐barrel (commonly called split‐spoon) sampler is a 2‐in. O.D.,  1.375‐in. I.D., typically 18 to 24 in. long and is connected to an AW or N size drilling rod.  The  sampler is then driven into the soil with a force of a 140 lb. hammer free‐falling a distance of 30  in.  The number of hammer blows required to drive the sampler into the soil is recorded for  each 6‐in. interval.  The sampler is typically driven a total of 18 in., and the last two 6‐in.  interval blow counts are added together and commonly referred to as the "N" value, blow  count or penetration resistance.  Representative samples are placed in airtight glass jars and  returned to our laboratory for further observation and testing.  Descriptions of the spilt‐barrel  samples and the penetration resistances are shown on the boring logs.  Shelby Tube Sampling Procedure  In the Shelby tube sampling procedure, a thin‐walled steel seamless tube with a sharp cutting  edge is pushed hydraulically into the soil and a relatively undisturbed sample is obtained.  This  procedure is generally employed in cohesive soils.  The tubes are carefully handled in the field  to avoid excessive disturbance and are returned to the laboratory for extrusion and further  analysis and testing.  Calibrated Pocket Penetrometer Testing  The strength of cohesive soils does not correlate as well as granular materials with the Standard  Penetration Testing described above.  Typically, we test split‐barrel samples of cohesive soils  with a calibrated pocket penetrometer in the field.  This test involves pushing a spring‐loaded  piston, 0.25‐in. in diameter, into the sample and measuring the spring deflection, which has  been correlated to shear strength.  This test is used as a rough approximation method only.   More refined results require undisturbed Shelby tube sampling and laboratory unconfined  compressive strength testing.  Water Level Readings  When the drilling crew notices groundwater or significant variations in soil moisture, they are  recorded on the boring logs.  Generally, the level of water at the time of drilling is measured  and recorded.  The readings may indicate the approximate level of the hydrostatic water table  at the time of our drilling activities.  Where low permeability soils are encountered, the water seeps into the borings at a slow rate,  and it is generally not possible to establish accurate groundwater level readings in an open  borehole during the drilling operations.  If water‐drilling methods are used, a local groundwater  "mound" could be created, taking several days to dissipate.  Also, the groundwater level  typically fluctuates on a long‐term or seasonal basis, due to variations in precipitation, surface  run‐off, evaporation, etc.  When these long‐term readings are required, piezometers or  monitoring wells are necessary to maintain an open hole.  Boring Log Preparation  The subsurface conditions encountered during drilling are reported on a field log recorded by  the chief driller.  The driller's field record contains information concerning the boring method,  samples attempted and recovered, indications of the presence of various materials such as  coarse gravel, cobbles, etc., and observations between samples.  Therefore, these records  contain both factual and interpretive information.  The field logs are on file in our office.  The soil samples, plus the field logs, are reviewed by a geotechnical engineer, geologist, or  geotechnician.  The engineer/geologist/geotechnician then classifies the soil in general  accordance with the Unified Soil Classification System and prepares the final boring logs, which  are the basis for our evaluations and recommendations.  The group symbol for each soil type is  indicated in parentheses following the soil descriptions on the boring logs.  The final boring logs  represent our interpretation of the contents of the field logs based on the results of the  engineering review and laboratory testing of the field samples.  The final boring logs are Rpts\Field Exploration Appendix\Lab\Lab Testing Proced+Grain-size_MC_Qp.doc_09/24/20 11:41 AM LABORATORY TESTING PROCEDURES Representative soil samples were selected and tested in our laboratory in order to check field classifications and to evaluate pertinent engineering properties. The laboratory testing program included visual classification of all samples and hand penetrometer tests on all cohesive samples. In the hand penetrometer test, the unconfined compressive strength of a cohesive soil is estimated by measuring the resistance of the soil sample to penetration by a small spring calibrated cylinder. Any additional tests are described below or on the following sheet(s). Appropriate data obtained from laboratory tests are also included on the respective boring logs. A geotechnical engineer classified each soil sample on the basis of texture and plasticity in accordance with the Unified Soil Classification System (ASTM D 2487 and/or ASTM D 2488). The group symbol for each soil type is indicated in parentheses following the soil descriptions on the boring logs. A brief explanation of the Unified System is included with this report. Data obtained from the field logs and appropriate laboratory tests have been shown on the boring logs. The procedures used in preparing the final boring logs are described on the sheet entitled “Field Exploration Procedures.” It should be noted that the geotechnical engineer grouped the various soil types into the major zones noted on the boring logs. The stratification lines designating the interfaces between earthen materials shown on the boring logs and profiles are approximate; in-situ, the transitions may be gradual. All samples will be retained in our Granger, Indiana laboratory for a period of thirty (30) days after which they will be discarded unless other instructions as to their dispos al are received. Grain Size Tests Grain-size tests are performed to determine the soil classification and the grain-size distribution. The soil samples are prepared for testing according to ASTM D 421 (dry preparation) or ASTM D 2217 (wet preparation). The grain-size distribution of soils coarser than a No. 200 U.S. Standard sieve (0.074 mm opening) is determined by passing the samples through a standard set of nested sieves. Materials passing the No. 200 U.S. Standard sieve are either suspended in water and the grain-size distribution calculated in accordance with ASTM D 422, or are washed over the No. 200 sieve in accordance with ASTM D 1140. If there is a substantial lapse of time between the submission of our report and the start of  work at the site, or if conditions have changed due to natural causes or construction operations  at or adjacent to the site, we urge that our report be reviewed to determine the applicability of  the conclusions and recommendations considering the changed conditions and time lapse.  We urge that we be retained to review those portions of the plans and specifications that  pertain to earthwork and foundations to determine whether they are consistent with our STANDARD CLAUSE FOR UNANTICIPATED SUBSURFACE CONDITIONS  The owner has had a subsurface exploration performed by a Geotechnical consultant, the  results of which are contained in the consultant’s report.  The consultant’s report presents his  conclusions on the subsurface conditions based on his interpretation of the data obtained in  the exploration.  The contractor acknowledges that he has reviewed the consultant’s report and  any addenda thereto, and that his bid for earthwork operations is based on the subsurface  conditions as described in that report.  It is recognized that a subsurface exploration may not  disclose all conditions as they actually exist and further, conditions may change, particularly  groundwater conditions, between the time of a subsurface exploration and the time of  earthwork operations.  In recognition of these facts, this clause is entered in the contract to  provide a means of equitable additional compensation for the contractor if adverse  unanticipated conditions are encountered and to provide a means of rebate to the owner if the
