Our company is dedicated to the preservation of trees in regard to their health and beauty as they add value to the landscape in general and to a home in particular. The following recommendations are based on my 30+ years of experience and continued training as a Certified Arborist. I look forward to partnering with you in the care and health of your trees. ESTIMATED COSTS OF WORK TO BE PERFORMED 1. The nearly dead ash tree in the parkway should be removed, cutting the stump low = $360.00 Note: We will need to have a Certificate of Appropriateness to remove this tree. 2. The tulip poplar at the front left corner of the house should be pruned off of the roof for a 4' clearance, and thinned above the roof to lightly reduce weight; repair the broken section in the crown = $720.00 3. The old apple tree south of the porch is poorly structured, with decay in the main trunk as well as the roots. We recommend removing the tree before it falls, cutting the stump low = $420.00 Note: We will need a Certificate of Appropriateness to remove this tree as well. When the apple is removed, is will allow for more light to reach the small maple sapling. This should help it grow up into a beautiful tree for your front yard. 4. The crabapple off the back right corner of the home should be pruned lightly off of the neighbor's home, removing deadwood (1" and larger) throughout. Lightly thin the end of the branching throughout the tree for a cleaner appearance and less weight on the old scaffold branches, all of which have some decay present = $360.00 4a. We recommend annual (every fall) in-ground fertilization = $95.00 per treatment 4b. We should also treat annually (every spring) on a three spray fungicide program = $105.00 per spray x3 = $315.00 annually Note: There may be some dripping from the fungicide spray onto the neighbor's property to the south. We will need to have their signed agreement to this treatment because of this. If they have any questions or concerns, they are more than welcome to give us a call! ____________________________________________________________________________________________ Neighbor's Signature Printed Name Date 5. The dogwoods in the backyard should be cleaned up, removing deadwood (1/2" and larger). We will leave it up to our tree crew to decide what they are able to safely prune next to the electric service to the north = $220.00 for all 2 Note: You may want to consider getting some Bayer Advanced Tree and Shrub Pest Control and treating the dogwood annually in the spring. This will help protect against dogwood borer infestation. 6. Remove the old, dead crabapple, cutting the stump low = no additional charge while crew is out 7. The large sycamore should be pruned for a 6' clearance off of the house and chimney, removing deadwood (2" and larger in diameter), and thinning the interior = $1260.00 To accept this Proposal, please return a signed copy of this Proposal as soon as possible, keeping one copy for your reference. For trimming and removal services, we schedule the work based on the order we receive the signed Proposals in our office. The num ber of Proposals we receive varies from day to day as does the size of each job but we do our best to schedule the jobs in a timely manner, weather permitting. An office staff member will call you the Thursday or Friday prior to the week your job is scheduled to le t you know what day the crew is expected to arrive. Please note: For your convenience, the treatments that are recommended to be done on a “semi-annual”, “annual”, “biennial”, etc. basis will be automatically scheduled for the appropriate time and intervals unless you specify otherwise. We do it in this way for the continued optimum health of your trees and so that none are missed. We do send reminder cards two to three weeks prior to whe n treatment period is to begin. You may discontinue the service at any time with a n e-mail, note or call to our office prior to the treatment being done. Also note that the cost for treatments that are recommended to continue over a long period of time could increase as our cost s increase. Additionally, please note that for any type of spray (fungicide, pesticide, dormant oil, etc.), there could be overspray on surrounding structures including, but not limited to: home, garage, cars, patio furniture, windows and other outdoor amenitie s located within close proximity of the tree(s). Although the technician will do their best, many times it cannot be avoided. Arborcare is not responsible for moving outdoor items or cleaning up overspray. Please remember that watering your trees is very important to their health, especially if they are being fertilized. TERMS AND CONDITIONS UNDERGROUND SYSTEMS: Customer bears responsibility for informing Arborcare, Inc. employees of the location of any underground systems (i.e. sprink ler, septic, etc.) that may be affected by the trees being serviced or the path(s) needed for the equipment. This may be done by having the affected area(s) clearly marked or by providing a map of the system(s) prior to the job. Arborcare, Inc. is not liable for damage done to any systems of which we a re not informed. DRIVEWAYS/WALKWAYS: Arborcare personnel will make every reasonable effort to be careful on driveways/walkways to avoid, to the best of their ability, a ny cracks or breakage from equipment. Neither Arborcare, Inc. nor its employees will be held financially responsible for any cracks or breakage that may occur. Please note that if we are not permitted to use a driveway/walkway (if needed) and this is the only or most efficient way to access the tree(s) needing worked on, we may not be able to service the tree or there could be an additional charge for a climber to climb the tree (only if the tree is safe to climb). STATED COST: Each service is calculated considering all services or like-treatments are to be performed at one time. Cost(s) of individual services and like-treatments could be higher if customer requests only partial job. Any prices marked as “approximate” indicate the possibility that the price could vary (most often slightly) should the crew leade r determine if any additional labor/equipment charges are warranted for optimum execution of the job. Additionally, changing economic conditions which are beyond our control may dictate the need for us to include a surcharge for fuel if those costs rise dramatically between the time the Proposal is done and when the work is completed. CHANGES TO ORIGINAL PROPOSAL: Any additional services requested by customer at time of job will be executed only upon agreement with an acting Arborcare, Inc. representative and will become an extra charge over and above the agreed upon Proposal. Likewise, there are inst ances when a job requires more labor or use of equipment not anticipated at the time of the original estimate which may require an additional charge. This includes (but is not limited to) foreign substances, such as cement or bolts, in tree trunk/stump, significantly more deadwood in tree, compromised integrity of tree. In such case, we will make every attempt to contact you. Should we not be ab le to reach you, your signature below is authorization for Arborcare, Inc. to complete the job. QUALITY ASSURANCE: All work is guaranteed to be as specified and will be completed in a professional manner according to the current Tree Care Guidelines. For your protection, Arborcare, Inc. is fully covered by liability insurance and Worker’s Compensation. Please note that in any tree there exists an inherent possibility of failure due to structural defects. As Certified Arborists, we are highly tra ined but even with our best efforts, the possibility remains of unseen/undetected structural defects/flaws that could pose a risk in the future. The customer will not hold Arborcare, Inc. or any of its employees liable for any and all claims for bodily injury and/or property damage as a result of the failure of any tree after we leave the premises. All agreements are contingent upon delays above and beyond our control. PAYMENT: Due upon receipt of invoice; 2% per month finance charge after 30 days. MasterCard/Visa accepted. COLLECTIONS: Customer agrees to pay all collection fees and court costs should an invoice remain unpaid after 60 days. _________Jim Rudolph_______________ ________________________________________________________________ Jim Rudolph, ISA-Certified Arborist #IN-0217A Customer Acceptance Signature Date OR _________Sarah Nelson______________ Sarah Nelson, ISA-Certified Arborist #IN-3457A OR _________Justin McCloskey__________ Justin McCloskey, ISA-Certified Arborist #IN-3602A
