The issuance of this certificate does NOT in any manner, release
the recipient from the responsibility of complying with the requirements
of the zoning ordinances, building codes, safety codes, ADA or other
requirements of the City of South Bend, the County of St. Joseph, the
State of Indiana, or the United States Federal Government.

Hello Adam and Elicia,
Kurtt and I are moving into the contractor stage of rehabbing the house at 846 Forest Ave. We are updating all the
electric, plumbing, and eventually, the heating/cooling systems.
The attached COA Application is for the removal of the old electric meters and apartment fuse boxes and their
replacement with a new multi‐meter panel box that is a part of bringing the house up to code. We are moving the
individual electric boxes to the inside of each apartment as recommended, therefore the outside boxes will no longer be
in service. The new multi‐family meter panel is part of updating the system in general. Any new conduit will ultimately
be painted to match the exterior of the house.
I have included photos of the current panel boxes and the proposed location of the new multi‐meter box. I have also
included the link to the multi‐family meter panel
Of course we are hoping you can approve this in your office, but understand if it needs to go to committee. Please let
me know if you have any questions. Can I call in with the payment? I feel like I should just open a tab...
