The Board of Public Works at its meeting held on December 17, 2020, awarded the above
referenced quotation to you in the amount of $71,735.00.
Please forward the following documents in one submittal by January 7, 2021 to my attention
for Board of Public Works approval to lhensley@southbendin.gov:
1) One (1) signed original of the Contract (enclosed); and
2) Certificate of Insurance naming the City of South Bend as an additional insured.

The opportunity to bid on the South Bend Dam Signage and Staff Gage Installation was opened
on December 8, 2020. Board of Public Works did not receive quotes for the above project and
was recommended for special purchasing. Two companies were contacted for a quote with the
same contract from initial quote open and one quote was received.

Sufficient funds have been appropriated from Storm Water Sewer Fund, 667-06-605-517-442002.

THIS AGREEMENT, made and entered into this 17th Day of December, 2020, by and between,
Northern Indiana Construction Co., Inc., PO Box 1333, Mishawaka, IN 46546, HEREINAFTER
called the Contractor, and the Board of Public Works herein called the "Board".
WITNESSETH: That the Contractor covenants and agrees to make the following improvement,
as fully set out in the Request for Quotations, Standard Specifications, Special Provisions, Plans,
and Bid Proposals, all of which are set forth as a part of this Contract, for:

DESCRIPTION: SOUTH BEND DAM SIGNAGE AND STAFF GAGE
INSTALLTION
COMPLETION DATE: SEE SPECIFICATIONS
AMOUNT: $71,735
FUNDING: PR-00005673
The unit prices for this improvement were those prices as received and accepted by the Board on
the 8th Day of December, 2020.
The Contractor further agrees to notify the Engineer when this improvement is completed. This
notification shall be in the form of a Completion Affidavit, signed by the Contractor. Upon final
acceptance of the improvement by the Engineer, the Contractors final estimate will be presented
to the Board for final Payment with one (1) copy of the Completion Affidavit and one (1) copy of
a Waiver of Lien.
