referenced quotation to you in the amount of $71,670 for the Base Quote.
Please forward the following documents in one submittal by January 7, 2021 to my attention
for Board of Public Works approval to lhensley@southbendin.gov:
1) One (1) signed original of the Contract (enclosed); and
2) Certificate of Insurance naming the City of South Bend as an additional insured.

On November 24, 2020, the Board of Public Works opened and read quotes for the above
project.
The quotes were reviewed and found to be complete. The quotes are summarized as follows:

Base Quote $71,670.00 $88,130.00
Alternate No. 1 $28,130.00 $34,399.00
Total, Base plus Alternate $99,800.00 $122,529.00
Division of Engineering has reviewed all the quotes and recommends award of a contract for
East Race Sidewalk Repairs (Base Quote Only) to the lowest responsive / responsible bidder,
Walsh & Kelly, Inc. in the amount of $71,670.00.
Sufficient funds have been appropriated from Curb and Sidewalk, by way of PR-00005484.

PUBLIC WORKS CONTRACT
THIS AGREEMENT, made and entered into this 17th Day of December, 2020, by and between,
Walsh & Kelly, 24358 State Road 23, South Bend, IN 46614, HEREINAFTER called the
Contractor, and the Board of Public Works herein called the "Board".
WITNESSETH: That the Contractor covenants and agrees to make the following improvement,
as fully set out in the Request for Quotations, Standard Specifications, Special Provisions, Plans,
and Bid Proposals, all of which are set forth as a part of this Contract, for:
PROJECT NO. 117-130
DESCRIPTION: EAST RACE SIDEWALK REPAIRS
COMPLETION DATE: SEE SPECIFICATIONS
AMOUNT: $71,670; BASE QUOTE
FUNDING: PR-00005484
The unit prices for this improvement were those prices as received and accepted by the Board on
the 24th Day of November, 2020.
The Contractor further agrees to notify the Engineer when this improvement is completed. This
notification shall be in the form of a Completion Affidavit, signed by the Contractor. Upon final
acceptance of the improvement by the Engineer, the Contractors final estimate will be presented
to the Board for final Payment with one (1) copy of the Completion Affidavit and one (1) copy of
a Waiver of Lien.
