RE: Handicap Parking Permit Renewal - No. 4
Dear Ms. Savely:
On December 8, 2020, the City of South Bend’s Board of Public Works approved your
application for the renewal of your reserved handicap parking permit No. 4 for your Chevy,
Monte Carlo.
The renewal authorizes you to park your vehicle in the reserved space located on the north side
of Jefferson Blvd., east of Lafayette Blvd, from January 1, 2021 to December 31, 2021. Please
be advised that you must display the reserved handicap parking permit on your dashboard at all
times when you are parked in the corresponding reserved handicap parking space. Also, please
note that this space is reserved as a handicap parking space only between the hours of 6 a.m. to
6 p.m., Monday through Friday.
The fee for the Reserved Handicap Parking Permit will be Forty-Five Dollars ($45.00) per
month, due by the first of every month, and made payable to the City of South Bend. Checks
may be hand-delivered or mailed to the City’s Engineering Department at the address above. A
yearly renewal of this application is required.
