Attached to this memorandum is Resolution 3058 and Amendment to the Addendum To Master
Agency Agreement for the Main - Lafayette Corridor Project, Supplement #6.
The purpose of this supplement is to add funding to cover demolition services related to three
structures and related out buildings purchased for the Main - Lafayette Connector Project. The three
structures proposed for demolition are as follows:
4101 South Main Street (DeBuysser Parcel — residential structures)
• 4219 South Main Street (Mink Parcel — residential structures)
• 4043 South Michigan Street (Premier college of Cosmetology— commercial structure)
The Board of Public Works has sent Notice of Quote letters to area contractors and quotes are to be
opened by the Board on August 9th. Depending on prices received, the Board will award demolition
on the 14th of August. The project is scheduled to be completed by November and all sites will be
prepped for the winter season.
Staff requests approval of Resolution 3058 and the Addendum To Master Agency Agreement for the
Main - Lafayette Corridor Project, Supplement #6.

RESOLUTION OF THE SOUTH BEND REDEVELOPMENT COMMISSION
APPROVING AND AUTHORIZING THE EXECUTION OF AN AMENDMENT
TO THE ADDENDUM TO THE MASTER AGENCY AGREEMENT
(Main - Lafayette Connector Design Project - Supplement #6)
WHEREAS, effective January 1, 2012, the South Bend Department of
Redevelopment, acting by and through its Redevelopment Commission (the
"Commission ") and the South Bend Board of Public Works (the "BPW ") entered into a
Master Agency Agreement which authorized the BPW to act as agent for and on behalf
of the Commission for certain projects during 2012; and
WHEREAS, effective January 1, 2012, the Commission and the BPW entered
into a Master Agency Agreement which authorized the BPW to act as agent for and on
behalf of the Commission for the limited purpose of contracting for and managing the
completion of existing Projects; and
WHEREAS, pursuant to the Master Agency Agreement, the Commission added
the Main - Lafayette Connector Design Project to the Master Agency Agreement by way
of an Addendum on August 23, 2010; and
WHEREAS, the Commission amended the Addendum with Supplement #1 by
way of Resolution 2838 on January 21, 2011; and
WHEREAS, the Commission amended the Addendum with Supplement #2 by
way of Resolution 2873 on May 10, 2011; and
WHEREAS, the Commission amended the Addendum with Supplement #3 by
way of Resolution 2953 on November 8, 2011; and
WHEREAS, the Commission amended the Addendum with Supplement #4 by
way of Resolution 2954 on November 8, 2011; and
WHEREAS, the Commission amended the Addendum with Supplement #5 by
way of Resolution 3023 on April 24, 2012; and
WHEREAS, the Commission desires to amend the Addendum by way of this
Amendment.
NOW, THEREFORE, BE IT RESOLVED by the South Bend Redevelopment
Commission as follows:
Section 1. The Commission hereby approves the Amendment to the
Addendum to the Master Agency Agreement (Main- Lafayette Connector Design Project -
Supplement #6) and hereby authorizes its execution in substantially the form attached
hereto with such changes as the Commission may deem necessary or appropriate upon
the advice of counsel, said execution thereof to be conclusive evidence of the
Commission's approval of such changes. The Clerk is hereby directed to file a copy of
this Amendment to the Addendum with the BPW.
Section 2. This Resolution shall be in full force and effect after its adoption
by the Commission.
Section 3. Commission staff members are authorized to execute on behalf of
the Commission any documents necessary to carry out the intent of this resolution.
ADOPTED at a meeting of the South Bend Redevelopment Commission held on
July 26, 2012, at 9.30 a.m., in Room 1308, County -City Building, South Bend, Indiana

AMENDMENT TO THE ADDENDUM TO THE
MASTER AGENCY AGREEMENT
(Main- Lafayette Connector Design Project - Supplement #6)

This Amendment to the Addendum to the Master Agency Agreement (this
"Amendment "), made and entered into as of the 14th day of August, 2012, by and
between the South Bend Department of Redevelopment, acting by and through its
Redevelopment Commission (the "Commission ") and the City of South Bend, Indiana, a
municipal corporation duly organized and existing pursuant to the laws of the State of
Indiana, acting by and through its Board of Public Works (the "BPW ") for purposes of
the Commission designating the BPW to act as the Commission's agent to undertake the
Main - Lafayette Connector Design Project - Supplement #6 (the "Project ").
WHEREAS, effective January 1, 2012, the Commission and the BPW entered
into a Master Agency Agreement which authorized the BPW to act as agent for and on
behalf of the Commission for certain projects during 2012; and
WHEREAS, effective January 1, 2012, the Commission and the BPW entered
into a Master Agency Agreement which authorized the BPW to act as agent for and on
behalf of the Commission for the limited purpose of contracting for and managing the
completion of existing Projects; and
WHEREAS, pursuant to the Master Agency Agreement, the Commission added
the Main- Lafayette Connector Design Project to the Master Agency Agreement by way
of an Addendum on August 23, 2010; and
WHEREAS, the Addendum was amended by both the BPW and the Commission
by Supplement #1 during February 2011; and
WHEREAS, the Addendum was amended by both the BPW and the Commission
by Supplement #2 during May 2011; and
WHEREAS, the Addendum was amended by both the BPW and the Commission
by Supplement #3 during November 2011; and
WHEREAS, the Addendum was amended by both the BPW and the Commission
by Supplement #4 during November 2011; and
WHEREAS, the Addendum was amended by both the BPW and the Commission
by Supplement #5 during April 2012; andWHEREAS, the Commission desires to amend
the Addendum by way of this Amendment.
NOW, THEREFORE, in consideration of the mutual covenants and promises
contained herein, and for other good and valuable consideration, the receipt of which is
hereby acknowledged, the BPW and the Commission agree as follows:
The Commission hereby empowers and appoints the BPW, pursuant to the
Master Agency Agreement, to act as the Commission's agent for the limited purpose of
contracting for and managing the completion of the Project, the scope of said Project
being originally described in "Exhibit A ", amended by Supplement #1 as described in
"Exhibit B ", amended by Supplement #2 as described in "Exhibit C ", amended by
Supplement #3 as described in "Exhibit Q ", amended by Supplement #4 as described in
"Exhibit E ", amended by Supplement #5 as described in "Exhibit F ", and now amended
as described in "Exhibit G ", attached hereto and made a part hereof.
2. All of the terms and conditions of the Master Agency Agreement shall
control this appointment and this Addendum shall be attached to the Master Agency
Agreement.
Commission staff members are authorized to execute on behalf of the
Commission any documents necessary to carry out the intent of this resolution.
IN WITNESS WHEREOF, the undersigned execute this Addendum to Master
Agency Agreement to be effective as of the date first written above.

See attached quote request to advertise for the demolition of three structures located
within the project area. Quotes are to be received by the Board of Public Works on
August 9, 2012.
Structures for Demolition are as follows:
4101 South Main Street (DeBuysser Parcel — residential structures)
4219 South Main Street (Mink Parcel — residential structures)
4043 South Michigan Street (Premier College of Cosmetology — commercial structure)
INVITATION TO QUOTERS
Notice is hereby given that the City of South Bend, Indiana, Board of Public Works will receive
sealed quotes at the office of the Board of Public Works, County City Building Room 1316, 227
W. Jefferson St., South Bend, Indiana, 46601 until the hour of 30:30 a.m., on Thursday,
August 9. 2012 for the following:
Demolition of:
Former Debuysser Property, Parcel 420, 4101 S. Main St.
Former Premier College of Cosmetology, Parcel 377, 4043 S. Michigan St.
Former Mink Property, Parcel 430, 4219 S. Main St.
For Project No. 110 -058A
Work to be performed shall include furnishing all labor, services, materials, insurance and
equipment for the demolition and environmental abatement of buildings on the aforementioned
properties, all more particularly described in the following documents contained in this invitation.
The Contract Documents are on file and available for public inspection during regular working
hours at the Department of Public Works (1316 County -City Building, South Send, Indiana), and
at the offices of Lawson - Fisher Associates, P.C., 525 West Washington Avenue, South Bend,
Indiana 46601, phone (574) 234 -3167
Quotes must be on the City of South Bend Contractor's Quote for Public Work form.
Each quoter or contractor (hereinafter the contractor) must fully comply with either the
Contractor's Non - Collusion Affidavit and Common Construction Wage Scale of these bid
conditions as to each construction trade it intends to use on this construction contract and all
other construction work (both federal and non - federal) in the St. Joseph County area during the
performance of this contract or subcontract. The contractor commits itself to the goals for
minority manpower utilization in either the Contractor's Non- Collusion Affidavit and Common
Construction Wage Scale, as applicable, and all other requirements, terms and conditions of
these quote conditions by submitting a properly sealed quote.
Work under this project is subject to a Common Wage Determination as established in
accordance with IC 5- 16 -7-4.
The Contractor shall appoint a company executive to assume the responsibility for the
implementation of the requirements, terms and conditions of the quote conditions.
A Mandatory Pre -Quote Conference will be held on Wednesday, July 25, 2012 at 10:30
a.m. at the former Premier Coilege of Cosmetology, 4043 S. Michigan Street. Each quoter is
required to visit the project site and become familiar with the job conditions prior to making a
quote. Any questions regarding bidding conditions must be addressed to the Owner in writing
no later than Friday, August 3, 2012.
The Board reserves the right to reject any or all quotes or to accept a full or partial award of the
quote or quotes which, in its judgment, will be to the best interests of the City of South Bend.
