Memorial Hospital Area Fagade Improvements

On December 13, 2011, the Commission approved a Professional Services Agreement with
the Troyer Group. This agreement was undertaken as part of the improvements in and
around the Memorial Hospital campus, and to specifically create a unified look of the
buildings on the east side of Michigan St., including the Skywalk over this main
thoroughfare leading into downtown South Bend.
The December 2011 agreement with Troyer was to have them finalize facade designs,
prepare construction bid specs, project bidding evaluation, and construction oversight. We
are currently in the construction oversight phase of these fagade improvements.
As the project progressed, it was discovered there were undocumented underground
utilities where the foundation of the west tower supports were to be placed. This discovery
required a redesign of the tower, which has led to additional work outside the scope of the
original agreement.
Cost for these additional services is $3,500.
Staff requests approval of this Professional Services Agreement Addendum in an amount of
$3,500.

DESCRIPTION
Redesign of the tower element on the project due to in place utilities /as -built conditions. The design work included developing
three options, pr renting them to the Owner /Contractor Team, revising the existing documents issuing them in the form of a
Proposal request for pricing and construction and review of new shop drawings and submittals related to the changes.
There are no additional eests for Construction Administration Services related to this work.
Reimbursables for this work will be covered via the original agreement.

Dear Mr. Relos
W c are pleased a) offer to yuu our Architectural Services to complete the construction documentation,
bidding, and construction administration phases for the new facades of the Skyway Office Building and
associated fencing. the Skyway Bridge, the E. Blair Warner office building, the hank. and MarMain
pharmacy. We have not included the N1PSC'O Building as Memorial Hospital of South Bend has yet to
finalize requirements for the property at this time. We have listed the components of each building and
broken the 2012 versus the 2015 phases out separately in this proposal. Thank you for your continued
confidence in our design team.
Sk*1xa_QMce Building and Sktwat Bridee -2012 chase
We understand that budgets of S1,000,000 for the Skyway Office Building and 51,100.000 for the
Skyway Bridge have been approved for expenditures in 2012. We also understand these amounts will
need to raver construction and design fees.
The design for the Skyway Office Building will need to be altered. 7'hc current design's probable cost is
52.625.000 without design fees. The design team has included in our proposed services working with
Gibson -Lewis for consiruciinn cost estimation to determine the scope of the fagade treatments. We have
allowed a maximum of two design review meetings /presentations; additional review
meetingvpresentations will be done at time and expense.
The Troyer Group has copies of the original Construction Documentation for both the Skyway Office
Building and the Skyway Bridge. This will aid us in verifying existing conditions 1'or both of these
facades. We have included field measuring of both in our senices to confirm existing as -built condition.
We have also included additional fagade treatments on the Skyway Office Building and the fencing for
both the north and south properly (as shown in the renderings) in the bid package. The fencing has
probably construction cost of $100.000 (North) and S( 85,000 (South). This will allow us to have

alternatives should the pricing come in lower than the expected construction cost. The Skywa) Bridge,
the Skyway Offiicc Building and the alternates noted above will comprise the first bid package.
We have not included an) coordination that might be required with the Indiana State Dcpanmcnt of
Transportation ( INI)OT) Por an% of the Skywa) Bridge treatments. We believe that everything proposed
should not encroach on the current clearance alread) in place. We do not expect an) additional INDOT
meetings. if there arc INDOT or other governments approvals, the design team will perform the work
required on a time and expense basis.
We anticipate being able to start the field verification and design probable cost determination to narrow
down the design once this contract is approved. We anticipate completing the first bidding package in the
first quarter of 21112 so that construction bidding can occur in approximately early March of 2012.
Construction would then he able to start when the x inter weather clears iputentialh April).
E. Blair Warner Met Buildine.IheHattk.& MarMlnin —2015 Phase
The Estimated Construction Pricing for the E. Blair Warner Building and the Bank Building is 5575,000.
MarMain is 5390,0(1(). The proposed decorative column at The MRI screen wall will also need to be
evaluated: a revised probable cost has not yet been determined. In addition, the Skyway Fencing will be
included in this bid package if it is not completed in phase one. This is considered to he bid package two.
Please note that these constriction costs have not been adjusted for inflation.
The design team will request copies of the original documentation to assist in verify the existing field
conditions of each of the buildings from Memorial Hospital of South Bend. Once we have these. we will
reproduce these drawings and return the originals. Completing this set of documentation will be second
priority but will be conducted in earl) 2012.
The anticipated bid time will be roughly March of 2015 with construction staring in April of 2015. We
can move forward with this schedule if funding allows. For ibis reason, we will break the bidding and
construction administration fee out from the bid documentation preparation fee.

Bid Package One
For the services listed above under 2012 Phase, we offer a fee of One Hundred Nineteen Thousand. Two
Hundred and Fift) Dollars ISI 19,250-00) not including reimbursable expenses. Reimbursable expenses
are composed of copies. facsimile. velum prints, and postage i deliver service costs. Expenses Hill be
billed at their cost times 1.1 (one point one)
Bid Package Two
For the preparation of the construction documents listed abo%c for the second bid package, we ofFera fee
of Forty -Five Thousand. Six Hundred Dollars ($45.600.00) not including reimbursable expenses.
Reimbursable expenses are composed of copies, facsimile, velum prints, and postage : delivery service
costs. Expenses will be billed at their cost times 1,1 (one point one).
For the bidding and construction administration phases of bid package two, which will occur in 2015, we
offer a fee of Ten Thousand. One Ifundred and Fifty Dollars {510,150) not including reimbursable

expenses. Reimbursable expenses arc composed of copies, facsimile. velum prints, and postage dcliarry
service costs. Expenses will be billed at their cost limes 1.1 lone point one).
The Troyer Group hills monthly. Consequently, at the beginning of each month you will receive a
statement covering the time and expenses expended during the previous month. Pa }'ment is due within
thirty 1301 days after statement dale.
