Proposed 4" sidewalk shall be graded to drain to existing inlet with pump. Max. longitudinal slope = 4.5% where possible. 2. Sidewalk shall be removed to existing joints. 3. Preformed 1 2" joint filler shall be placed between proposed sidewalk and existing structures such as retaining wall, existing sidewalk, etc. Concrete joints in sidewalk shall be per City of South Bend Standards. 4. The East Race shall be dewatered fully in the concrete repair area prior to any repair activity for evaluation by the Engineer. The contractor shall coordinate with the South Bend Parks and Recreation Department to shut the head gates on the East Race to dewater. 5. See sheet 5 for concrete repair details. 6. Limited clearance and access to work site under bridge. 7. There shall be no disturbance, modifications, or impact to Colfax Bridge or its appurtenances. REPAIR SCHEDULE NOTES: 1. Refer to Specification Section XVIII, Measurement and Payment, for measurement and payment of structural repair types detailed on Sheet 5. 2. Engineer may direct Contractor to perform repair work that is greater or less than the estimated quantities identified on the bid form. Contractor shall be paid for actual quantity of repairs. Remove existing concrete using hand chipping methods as required. Do not cut or damage existing reinforcing during concrete removal process. Remove all deteriorated concrete to sound concrete. Prepare concrete substrate to obtain a surface profile of 1/8" in depth with a new fractured aggregate surface. Where reinforcing steel is present and with active corrosion encountered, Engineer will review condition of corroded rebar prior to repair. Replacement is required where loss on rebar cross section is over 10%. After repair where reinforcing remains, clean reinforcing steel to remove all contaminants and rust. If reinforcing replacement is required, cut existing corroded reinforcing bar as required and weld new rebar of same size, as shown. If rebar is not present, place new #3 rebar in field drilled hole in concrete with an approved chemical anchor system according to Detail 5. Surface preparation shall comply with repair mortar manufacturer's instructions. Install concrete per Project Specification Section XXVIII. For bid purposes, total depth of repair is equal to member depth. PROJECT DESCRIPTION A. The project includes the furnishing of all equipment, material, insurance, and labor for the repair of the cracks, sidewalk, and flap valve along the West side of the East Race under the Colfax Avenue bridge within the City of South Bend, Indiana. Work to be performed shall include the repair of existing cracks, removal of existing concrete sidewalk, excavation and placement of #8 stone base drainage layer under the sidewalk, and replacement of concrete sidewalk as specified in these documents. II.PREVAILING SPECIFCIATIONS AND DESIGN & CONSTRUCTION STANDARDS A. The City of South Bend's PREVAILING SPECIFICATIONS, most recent version, and DESIGN & CONSTRUCTION STANDARDS, most recent version, are to be used on this project. B. Each Quote provider is specifically instructed to become completely familiar with the most recent version of the PREVAILING SPECIFICATIONS and the DESIGN & CONSTRUCTION STANDARDS prior to submitting a quote. C. Wherever the PREVAILING SPECIFICATIONS refer to “State Specifications,” it shall mean the 2020 INDOT Standard Specifications for the letting effective after September 1, 2019. D. These SPECIAL PROVISIONS will list only "Additions" or "Deletions" to the PREVAILING SPECIFICATIONS and are to be used only in conjunction with the PREVAILING SPECIFICATIONS. E. In the event of conflict between the SPECIAL PROVISIONS and the PREVAILING SPECIFICATIONS, the SPECIAL PROVISIONS will govern. III.TERM “OR EQUAL” A. Prevailing Specifications: None B. Additions: 1.Where the term "or equal" is used in these specifications, the Quote provider deviating from specified item shall file with his/her Quote a letter fully explaining and justifying his/her proposed article or equal. The City of South Bend shall be the sole judge in determining if the “or equal” offered meets the specification. IV.TAX EXEMPT A. Prevailing Specifications: None B. Additions: 1. Materials and properties purchased under contract with the Owner that becomes a permanent part of the structure or facilities constructed are not subject to the Indiana Gross Retail Tax (Sales Tax). The exemption number will be furnished to the Contractor upon award. Version 10/31/2017 Page - 4 Public Works Quote V. INDEMNIFICATION A. Prevailing Specifications: None B. Additions: 1. Contractor agrees to indemnify, defend and hold harmless the City of South Bend, its agents, officers and employees, from all costs, losses, claims and suits, including court costs, attorney fees, and other expenses, arising from or out of the negligent performance of this Contract by Contractor, or because of arising out of any defect in the goods, materials or equipment supplied by the Quote provider. VI. INSURANCE A. Prevailing Specifications: 2020, INDOT Standard Specifications Section 103 B. Additions: 1. All Contractors and subcontractors doing business with the City of South Bend shall present a Certificate of Insurance showing coverage in the following minimum amount: a. General Liability: Premises-Completed Operations or Products, Bodily Injury and Property Damage Combined Single Limit - $5,000,000. b. There shall be no exclusion for explosion, collapse or underground hazard. c. Workmen’s Compensation: Statutory State of Indiana Employer's Liability - $100,000. d. Auto Liability: Bodily Injury and Property Damage Combined Single limit - $1,000,000. e. The City of South Bend shall be named as additional insured on the Certificate of Insurance. VII. CONTROL OF WORK A. Prevailing Specifications: 2020, INDOT Standard Specifications Section 105 B. Additions: 1. The complete responsibility for this project lies with the Director of Public Works of the City of South Bend, Indiana acting through his authorized representatives. 2. Construction Engineering - The Contractor shall provide all the necessary, qualified personnel, equipment and supplies to perform all work required under this item. There will be no direct payment for this item. 3. The contractor is responsible to maintain the site which includes but is not limited to; dust control, site security, erosion control, and protecting adjacent properties. 4. All work activities and closures of the East Race shall be coordinated by the Contractor and approved by the Parks and Recreation Department such that it does not interfere with scheduled events at the East Race. No dewatering or restriction of the East Race during the weekends shall be permitted without prior approval. Work hours for the Project shall be from 7:00 a.m. through 6:00 p.m., Monday through Friday. No work shall be permitted on weekends, Holidays, or after hours unless approved by the City of South Bend Department of Public Works. 5. Any dewatering required for any work activities shall be incidental to the work item and not be paid for separately. VIII. LEGAL RELATIONS A. Prevailing Specifications: 2020, INDOT Standard Specifications Section 107w B. Additions: 1. The Owner, where mentioned in these documents, is the City of South Bend. The Engineer, where mentioned in these documents, is Lawson-Fisher Associates P.C. 2. The Contractor shall apply for and obtain any and all required permits for the work from local, state, and federal agencies and shall comply with permit requirements, including the St. Joseph County / City of South Bend Building Department. 3. If the Contractor awarded this contract is not a resident of Indiana, within thirty days, the Contractor shall provide the Engineer with proof that the Contractor is duly licensed, qualified and registered with the Secretary of State of Indiana to engage in business within the State of Indiana. 4. The Contractor shall comply with all provisions of Indiana Code 5-16-13, as amended. The Owner reserves the right to immediately terminate the public work project awarded should the Contractor Version 10/31/2017 Page - 5 Public Works Quote be found to be in violation of any provision of Indiana Code 5-16-13 and such violation shall result in the forfeiture of Contractor’s performance bond to the Owner. IX. PROSECUTION AND PROGRESS A. Prevailing Specifications: 2020, INDOT Standard Specifications Sec. 108 B. Additions: 1. The project will have a completion date of twenty-one (21) calendar days for all Base Quote work and thirty (30) calendar days for all Base Quote and Alternation No. 1 work. The contract time will start when the Notice to Proceed is delivered and signed. All work shall be completed by October 31, 2021. 2. This project addresses the repair of settled sidewalk along the West side of the East Race under the Colfax Avenue bridge. 3. The lowest, most responsive Contractor will be awarded this work for completion within the earliest timeframe possible. 4. The most responsive Contractor should be prepared to have all bonds, insurance and other documents required herein at project award. 5. All work activities and closures of the East Race shall be coordinated by the Contractor and approved by the Parks and Recreation Department such that it does not interfere with scheduled events at the East Race. No dewatering or restriction of the East Race during the weekends shall be permitted without prior approval. 6. Access to the site shall be through Gintz Avenue and may require coordination due to other projects in the area. 7. Equipment access under the Colfax bridge will be limited due to restricted headroom. There is approximately 8-ft. of clearance under the bridge to the finished walk elevation. 8. The City, Engineer, and Contractor will hold a pre-construction meeting following award of the contract. 9. The Contractor shall be prepared to receive Notice to Proceed coincidental with the Preconstruction Meeting. 10. Contractor shall provide a schedule to the Owner prior to beginning any work on the site. X. CHANGE OF CONTRACT TIME A. Prevailing Specifications: 2020, INDOT Standard Specifications Section 108 B. Additions 1. The Contract Time may only be changed by Change Order. Any Claim for an extension in the Contract Time shall be based on written notice delivered to the Department of Public Works within seven (7) calendar days of the occurrence of the event giving rise to the claim. Notice of the extent of the claim with supporting data shall be delivered within fourteen (14) calendar days after such occurrence unless an official of the Public Works Department allows an additional period of time to ascertain more accurate data. The Contract Time will be extended in an amount equal to time lost to delays beyond the control of the Contractor if a claim is made in accordance with this provision. Such delays shall include acts of neglect by the Public Works employees, or to fires, flood, labor disputes, epidemics, abnormal weather conditions, governmental procedures, or acts of God. 2. Unless otherwise provided, the Contract time is based upon normal weather conditions. An extension is granted for weather conditions significantly more severe than normal if the Contractor demonstrates to the satisfaction of the City that the delay in the progress of the work was due to such weather. The basis to define normal weather with be the data compiled by the United States Department of Commerce, National Oceanic and Atmospheric Administration (NOAA). 3. No extension of time will be granted if the Contractor, by his/her/its own action or inaction, including fault or negligence of Contractor’s subcontractors, caused the delay, or for which any remedies are provided under any other provision of this agreement. 4. The grant of an extension of time under this Section in no way constitutes a waiver by the City of any rights or remedies existing under this contract at law or in equity. Version 10/31/2017 Page - 6 Public Works Quote XI. DEFAULT AND TERMINATION A. Prevailing Specifications: 2020, INDOT Standard Specifications Section 108 B. Additions: 1. Events of Default shall include Contractor’s failure to perform any of its obligations under this contract including failure to commence work at the time specified, failure to perform the work in accordance with these specifications, unauthorized discontinuation of the work, failure to carry out the work in a manner acceptable to the City, failure to observe Federal, State, or local laws or regulations, and failure to comply with any other term of this contract. 2. If an Event of Default occurs, the City shall provide Contractor written notice and may permit Contractor ten (10) calendar days after the date of the notice to cure the default. If the default is not cured within the ten (10) day cure period, the City may at any time thereafter terminate this contract in which case the termination shall be final and effective. 3. Upon an Event of Default, the City may invoke the following remedies in addition to those remedies provided under separate provisions of this contract, the right of set-off against any payments due or to become due to the Contractor against the retainage, the right to take over and complete the Work. If the City notifies Contractor that City is invoking its right to complete the Work, all rights that the Contractor has in order under Contractor’s subcontracts are assigned to the City, subject to the City’s right to take assignment of all or only selected subcontracts at the City’s discretion. The sole obligation accepted by the City under such subcontracts is to pay for Work satisfactorily performed after the date of the assignment. In the event a conditional assignment has not been executed, the Contractor shall execute or cause to be executed any assignment, agreement, or other document that may be necessary in the sole opinion of legal counsel to the City’s Board of Public Works to evidence compliance with this provision. The Contractor shall promptly deliver such documents upon the City’s request. In the case of such assignment, unless otherwise agreed in writing, The Contractor remains liability to subcontractors for any payment already involved, and for any claim, suit or cause of action based upon or resulting from any error, omission, negligence or other breach of contract by the Contractor, its officers, employees, or agents arising prior to the date of assignment to the City. XII. LIQUIDATED DAMAGES A. Prevailing Specifications: 2020, INDOT Standard Specifications Section 108 B. Additions: 1. The contractor shall proceed with the work at such rate of progress to insure full completion within the Contract Time. It is expressly understood and agreed, by and between the Contractor and the Owner, that the Contract Time for completion of the work described herein is a reasonable time, taking into consideration the average climatic and economic conditions and other factors prevailing in the locality of the work, and excludes the time for unavoidable delays which were beyond the control and without the fault of the Contractor. 2. If the Contractor shall fail to complete the work within the Contract Time, or extension of time granted by the Owner, then the Contractor will pay to the Owner the amount for liquidated damages a sum of five hundred dollars ($500.00) for each calendar day that the Contractor shall remain in default after the time of completion stipulated in the Contract Documents. 3. The Contractor shall not be charged with liquidated damages or any excess cost when the delay in completion of the work is due to the following and the Contractor has promptly given written notice of such delay to the Owner and Engineer/Architect. a. To any preference, priority, or allocation order duly issued by the Owner. b. To unforeseeable causes beyond the control and without the fault or negligence of the Contractor, including but not restricted to acts of God, acts of public enemy, acts of the Owner, acts of another Contractor in the performance of a Contract with the Owner, fires, floods, epidemics, quarantine restrictions, strikes, freight embargoes, and abnormal and unforeseeable weather. Version 10/31/2017 Page - 7 Public Works Quote XIII. RETAINAGE AND FINAL PAYMENT A. Prevailing Specifications: 2020, INDOT Standard Specifications Section 109 B. Additions: 1. Payments will be made every thirty (30) calendar days 2. Consistent with provisions of IC 36-1-12-14, the Board of Public Works shall retain a percentage of payments throughout the duration of the project. 3. Before final payment and retainage are released the Contractor must satisfy the following: a. All parts and labor meet requirements stated in the specifications. b. Provide copies of test reports or cut sheets on all materials supplied. c. Provide As-Built drawings in accordance with the City of South Bend Prevailing Specifications for Public Works. d. One (1) copy of the City of South Bend Completion Affidavit and one (1) copy of a Final Waiver of Lien. XIV. WARRANTY A. Prevailing Specifications: None B. Additions: 1. The Contractor shall provide the following: a. A Maintenance Bond equal to 10% of the construction cost for a period of 3 years. XV. OTHER UTILITIES A. Prevailing Specifications: None B. Additions: 1. The Contractor shall verify the locations of all utilities by contacting Holey Moley at 1-800-382-5544 at least two (2) working days, not counting Saturdays, Sundays or federal and state holidays before proceeding with construction. It shall also be the Contractors responsibility to contact any other utility that is not contacted by Holey Moley and verify the utility locations. 2. The Contractor shall be responsible for working with the other utilities, i.e., gas electric, telephone, etc. in order to assure that all utilities that need to be replaced or relocated can be done with a minimum disturbance to service. The Contractor shall also be responsible for coordinating schedules with the various utilities such that they can proceed with their relocation work as efficiently as possible. 3. If the odor of natural gas is detected in a work area at any time during the course of work, the Contractor shall immediately notify NIPSCO at 1-800-634-3524. The Contractor shall also immediately notify the residents of adjacent properties. The Contractor shall advise the residents to evacuate their homes immediately if the odor is present within the dwelling. 4. Restoration of sprinkler systems damaged by the Contractor's operations shall be repaired by the Contractor at no additional cost to the City or the Owner of the system. 5. The following utilities have been identified within or near to the project area. Additional utilities may be present and the Contractor shall be responsible for all coordination required to complete the work as shown in the plans. a. The facilities of American Electric Power (AEP) exist within the project area but are not expected to be affected by the proposed construction. The project includes the furnishing of all equipment, material, insurance, and labor for the repair of various channel and retaining wall cracks, removal and replacement of sidewalk, and a flap valve repair along the West side of the East Race under the Colfax Avenue bridge within the City of South Bend, Indiana. 2. The Contractor shall preserve and protect all surrounding property, structures, tenants, visitors and their property from damage caused by the Contractor’s operations. XVII. PLANS A. Prevailing Specifications: City of South Bend Design and Construction Standards B. Additions: 1. The plans consist of 5 Sheets. 2. The work shall conform to the general description and scope described in the plans. 3. The drawings are schematic in nature. XVIII. QUOTE REQUIREMENTS A. Prevailing Specifications: 2020, INDOT Standard Specifications Section 102.01 B. Additions: 1. The Contractor shall submit the following fully executed documents with his Quotation: a. Contractor’s Quote for Public Works. XIX. MOBILIZATION AND DEMOBILIZATION A. Prevailing Specifications: 2020, INDOT Standard Specifications Section 110 B. Additions: 1. The Lump Sum (LS) unit price for this item shall not be more than five (5%) percent of the total of all other quote items of the proposal. 2. For payment purpose, the mobilization portion of this item will be limited to 75% of the LS price. The remaining portion will be considered as demobilization when work is completed and upon final site cleanup. XX. CONSTRUCTION ENGINEERING A. Prevailing Specifications: 2020, INDOT Standard Specification Sections 105.08(b), 105.08(c) B. Additions: C. 1. The Contractor will provide all work required under this item. 2. The Engineer will be responsible for the horizontal and vertical control points as shown on the plans. 3. The Contractor will be responsible for all the horizontal alignment from the Engineer's control points and also the vertical grades shown on the Plans from the Engineer's benchmarks. 4. The Contractor shall be responsible for witnessing the horizontal and vertical points, establishing construction limits, and maintenance of same for the duration of the project. Construction Engineering will be paid for at the lump sum price bid for this item, and shall not exceed two (4%) percent of the total of all other bid items of the original proposal. 6. The Contractor shall maintain at the site for the Owner, one (1) record copy of all drawings, specifications, Addenda, Change Orders and other modifications, in good order and marked currently to record all changes made during construction. 7. The Contractor shall not be able to make additional costs claim for compressed schedule unless the delay is attributed to events or the work of others beyond their control. XXI. MAINTENANCE OF TRAFFIC A. Prevailing Specifications: 2020, INDOT Standard Specifications Sections 105 & 801 B. Additions: 1. Maintenance of traffic during construction shall conform to the “Indiana Manual on Uniform Traffic Control Devices” and the City of South Bend Design and Construction Standards. 2. The attached “Traffic Closure Request” form is to be used for any lane restrictions or closures and required to be filled out and sent to the Department of Public Works 3. The Contractor shall arrange and prosecute the work specified for this contract in such a manner that traffic on existing streets is unrestricted throughout the Project. The Engineer shall approve the method of traffic control. No construction equipment, vehicles, materials, supplies or temporary facilities shall be left unattended in the right-of-way of any street or left parked overnight without proper marking and lighting 4. The Contractor shall accommodate for any and all pedestrian traffic in their Traffic Control Plan. Any pedestrian detours and sidewalk closures shall be safe, protected, and free of hazards. 5. After the award of the contract and before beginning the work, the Contractor shall submit his proposed schedule of operations for the review of the Engineer. This schedule shall be available at the Preconstruction meeting as further described in the “Prosecution and Progress” section. The schedule of operations as reviewed by the Engineer shall be maintained at all times. 6. “Road Construction Ahead” signs in each direction, signing for flagging operations and all other necessary signing shall be provided by the Contractor. 7. This item will not be measured and shall be paid for at the Lump Sum (LS) unit price for MAINTENANCE OF TRAFFIC. XXII. CLEARING RIGHT OF WAY A. Prevailing Specifications: 2020, INDOT Standard Specifications Sections 201, 202 B. Additions: 1. The lump sum price bid for “Clearing Right-of-Way” shall include “Clearing and Grubbing” and “Removal of Structures and Obstructions” which shall include removal of all brush, trees, other vegetation, man-made materials, and other items identified on the plans for removal, required to perform the scope of work, except for those items which are specifically listed in the proposal for separate payment. 2. Any fencing, including the black raceway fence, or man-made obstructions (i.e. guardrail, utility markers, landscape rocks, etc.) which are in place at the start of construction shall be carefully removed. These items shall be restored using recovered serviceable materials. These shall include obstructions within existing right-of-way impacted by the scope of the work. Removal and restoration of fencing and other man-made obstacles within the right-of-way will not be paid for separately and shall be included in the cost of the pay item. Damaged items caused by the Contractor shall be restored or replaced at no additional cost to the Owner. 3. The Contractor shall provide a pre-construction video record and photo journal of the entire project area to verify the existing conditions prior to construction. 4. The Contractor shall have all underground utilities field located prior to the start of construction. The Contractor shall protect existing and relocated utilities from damage. 5. Burial of debris on site will not be allowed. Disposal of debris shall be at an off-site location in an approved manner according to all County and State Statutes. 6. This item will not be measured, this work shall be paid for at the Lump Sum (LS) unit price for CLEARING RIGHT-OF-WAY. Version 10/31/2017 Page - 10 Public Works Quote XXIII. TEMPORARY EROSION AND SEDIMENT CONTROL A. Prevailing Specifications: 2020, INDOT Standard Specifications Section 205 B. Additions: 1. Temporary erosion and sediment control shall be provided to prevent disturbed soil from leaving the project site and shall include perimeter protection, temporary surface stabilization, concentrated flow protection, concrete washouts, and other items as shown on the plans and described in Section 205 as required. 2. This work will not be measured for payment separately but shall be paid at the contract unit price for TEMPORARY EROSION AND SEDIMENT CONTROL (LS). XXIV. COMMON EXCAVATION A. Prevailing Specifications: 2020, INDOT Standard Specifications Section 203 B. Additions: 1. The Contractor shall keep all public roadways, row, and sidewalks that are used for hauling excavation or borrow material clean and free of any spillage of material. Said public areas shall be cleaned on a daily basis. The cost of removal of any spillage shall be included in the unit cost of excavation or borrow material and no further compensation will be made. Failure to comply with this requirement will result in cleaning of the spillage by the Owner with a back charge of two hundred fifty dollars ($250.00) per occurrence. 2. The Contractor shall field verify the location of all utilities prior to beginning work. 3. The quantities of excavation shown on the proposal sheet will not be measured separately by the Engineer. If the Contactor has acceptable data that indicates a excavation quantity error by more than 10%, then additional measurements will be performed by the Contractor and submitted to the Engineer for review and determination. Measurement will not include material excavated beyond authorized limits. If the quantity error is agreed by the Engineer, then the unit price on the proposal form will be extended. 4. This item in this described work shall be paid for at the contract unit price for COMMON EXCAVATION per cubic yard (CYS). XXV. COMPACTED AGGREGATE A. Prevailing Specifications: City of South Bend Design and Construction Standards 2020, INDOT Standard Specification Sections 105, 207, 301, and 303. B. Additions: 1. All coarse aggregate shall be Class D or higher of the specified size. The cost of placing, compacting, water and incidentals necessary for placement shall be included in the cost of the compacted aggregate. 2. The condition of the subgrade at the time paving material is placed is required to be in accordance with INDOT Standard Specifications 105.03 and 207.03. 3. The material type shall be either natural washed stone or limestone placed on geotextile. Materials such as crushed concrete and slag which are susceptible to leachate shall not be permitted. 4. The quantities of compacted aggregate shown on the proposal sheet will not be measured separately by the Engineer. If the Contactor has acceptable data that indicates a compacted aggregate quantity error by more than 10%, then additional measurements will be performed by the Contractor and submitted to the Engineer for review and determination. Measurement will not include material installed beyond authorized limits. If the quantity error is agreed by the Engineer, then the unit price on the proposal form will be extended. 5. The accepted quantity of COMPACTED AGGREGATE, NO. 8 will be paid for at the contract unit price per ton (TON), complete in place. Version 10/31/2017 Page - 11 Public Works Quote XXVI. This pay item shall consist of the labor and materials to construct the 4" concrete sidewalk as shown on the plans. 2. The finished concrete shall be finished, jointed, and cured in accordance with INDOT Standard Specification 604.03. Joints shall match existing adjacent concrete sidewalk. 3. Concrete shall be performance based and follow: a. All concrete shall have limestone aggregate. b. A minimum 28 day strength of 4,000 PSI. c. Shall have a 5% to 8% entrained air with an admixture that conforms to ASTM C-260. d. Slump shall be a maximum of 4". C. The accepted quantity of CONCRETE, 4" SIDEWALK will be paid at the contract unit price per square yard (SYS), complete in place. XXVIII. CONCRETE REPAIR AND REHABILITATION A. Prevailing Specifications: None. B. Additions: 1. The Contractor shall provide all labor, materials, equipment, and incidentals as shown, specified, and required to repair or rehabilitate, as required, all existing concrete shown or indicated in the Contract Documents as being repaired or rehabilitated. 2. Review installation procedures under this and other Work Items and coordinate the work that must be installed with or before repair and rehabilitation of concrete. MATERIALS: Standards referenced in this Section are: 1. ASTM C109/C109M-16a, Standard Test Method for Compressive Strength of Hydraulic Cement Mortars (Using 2-in. or [50-mm] Cube Specimens). 2. ASTM C882/C882M-13a, Standard Test Method for Bond Strength of Epoxy-Resin Systems Used with Concrete by Slant Shear. 3. ASTM D3574-17, Standard Test Methods for Flexible Cellular Materials – Slab, Bonded, and Molded Urethane Foams. 4. ASTM D1042-12(2017), Standard Test Method for Linear Dimensional Changes of Plastics Caused by Exposure to Heat and Moisture. Repair Mortar: A. Product Description: Repair mortar shall be prepackaged, cement-based product specifically formulated for repairing concrete surface defects. B. Products and Manufacturers: Provide one of the following: 1. SikaTop 122 Plus, SikaTop 123 Plus, or SikaTop 126 Plus, by Sika Corporation. 2. Crack Injection Materials: A. Structural Crack Repair System: 1. Epoxy for injection shall be low-viscosity, high-modulus moisture insensitive type. 2. Products and Manufacturers: Provide one of the following: a. Sikadur 35, Hi-Mod L.V. and Sikadur 31, Hi-Mod Gel, by Sika Corporation. b. Eucopoxy Injection Resin, by Euclid Chemical Company. c. Or approved equal. B. Non-structural Crack Repair System: 1. Hydrophobic Polyurethane Chemical Grout: a. Provide hydrophobic polyurethane that forms a flexible gasket. b. Products and Manufacturers: Provide one of the following: 1. SikaFix HH LV, by Sika Chemical Company. 2. Hydro Active Flex SLV, by De Neef Construction Chemicals, Inc. 3. Or approved equal. c. Shrinkage limit shall not exceed 4.0 percent in accordance with ASTM D1042. d. Minimum elongation of 250 percent in accordance with ASTM D3574. e. Minimum tensile strength of 150 psi in accordance with ASTM D3574. 2. Hydrophilic Acrylate-Ester Resin: a. Hydrophilic crack repair system shall be acrylate-ester resin that forms a flexible gasket and increase in volume by at least 50 percent when in contact with water. b. Products and Manufacturers: Provide one of the following: 1. Duroseal Multigel 850, manufactured by BBZ USA, Inc. 2. Or approved equal. SUBMITTALS: A. Action Submittals: Submit the following: 1. Product Data: Information on all products proposed for use, including manufacturer’s brochures, technical data, specifications, and other applicable data. 2. Manufacturer’s Instructions: Manufacturer’s recommended procedures for installing materials proposed for use. B. The Contractor shall submit any alternate repair mortar or crack injection materials to the engineer for review and approval. Version 10/31/2017 Page - 13 Public Works Quote CONSTRUCTION METHODS: Inspection: A. Examine areas and conditions under which the repair Work is to be installed and notify ENGINEER in writing of conditions detrimental to proper and timely completion of the Work. Do not proceed with the Work until unsatisfactory conditions have been corrected. B. Engineer will inspect all surfaces and determine initial location and extents of all concrete repairs and rehabilitations. Contractor shall coordinate with Engineer with regard to scheduling inspections. Inspections will be conducted after the area is dewatered by the Contractor. The Contractor shall coordinate with the South Bend Parks and Recreation Department to shut the head gates on the East Race to dewater. Preparation: A. Surface Preparation: 1. Initial Surface Preparation: Remove by handchipping, abrasive blasting, or hydro blasting all laitance, foreign material, and unsound concrete from entire area to be repaired. Further roughen surface as specified in this Section. Where non-shrink grout or repair mortar is used, perform additional surface preparation, if any, recommended by product manufacturer. 2. Wetting Procedure: Where repair concrete, shotcrete, or cement grout is used, and bonding agent is not required, or where repair mortar or non-shrink grout manufacturer recommends wet or saturated surface, perform the following: a. Continuously apply water for at least four hours to surface being repaired. Where large surface areas are to be repaired, use fog-spray nozzles, mounted on stands, in sufficient number so that entire surface to be repaired is contacted by fog spray cloud. b. Prevent concrete from drying until after repair is completed. Re-wet surfaces not yet repaired using water sprays at least daily; should more than four days elapse without re-wetting surfaces not yet repaired, repeat the original saturating procedure. c. Remove standing water in areas to be repaired before placing repair material. Provide means to remove excess water from structure. 3. Preparation for Epoxy Bonding Agent: Where repair material manufacturer recommends use of epoxy-bonding agent, conform to recommendations of both repair material manufacturer and bonding agent manufacturer. Installation, General: A. Care shall be taken to fully consolidate repair material, completely filling all portions of space to be filled. B. Bring surface being repaired into alignment with adjacent surfaces, providing uniform, even surface. Surface repair shall match adjacent existing surfaces in texture and shall receive coatings or surface treatments, if any, provided for the existing surface adjacent to repaired surface. C. Curing: 1. Curing of repair mortar and non-shrink grout shall be in accordance with manufacturer’s recommendations, except that minimum cure period shall be three days. 2. Curing of other materials shall be in accordance with requirements of Section XVII. Concrete 4” Sidewalk. Repair of Surface Defects: A. Surface defects are depressions in a concrete surface that do not extend all the way through the concrete. Surface defects can result from removal of an embedded item, removal of an intersecting concrete member, physical damage, or unrepaired rock pockets created during original placement. For spalls that result from corroded reinforcing steel or other embedment refer to REPAIR OF DETERIORATED CONCRETE in Work Item. Version 10/31/2017 Page - 14 Public Works Quote B. Preparation: Perform the following in addition to requirements of PREPARATION of this Section: 1. Remove by handchipping all loose, damaged concrete to sound material. 2. Where existing reinforcing is exposed, remove concrete to minimum of one-inch around exposed bars. If existing bars are cut through, cracked, or cross-sectional area is reduced by more than 10 percent from original, immediately notify Engineer. 3. Score-cut perimeter of area to be repaired to minimum depth of 1/2-inch and maximum depth that will not cut existing reinforcing steel. Chip out existing concrete to the score line so that minimum thickness of repair mortar will be 1/2-inch. C. Repair Material: 1. Completely fill the surface defect with specified repair material, in accordance with material manufacturer’s instructions and the Contract Documents. 2. Perform, with repair mortar, repairs of surface defects in concrete normally in contact with water or soil, and interior surfaces of structures that contain water. 3. Repair of other surface defects may be by applying repair mortar, repair concrete, shotcrete, or cement grout, as appropriate. Repair of Deteriorated Concrete: A. This Article pertains to deteriorated concrete which has been damaged due to corrosion of reinforcing steel, physical damage due to abrasion, or damage due to chemical attack. Use repair mortar, as specified in this Article, for repairing deteriorated concrete. B. Surface Preparation: In addition to requirements of the PREPARATION section, perform the following surface preparation: 1. Remove loose, broken, softened, and acid-contaminated concrete by abrasive blasting and handchipping to sound, uncontaminated concrete. 2. Upon completion of removal of deteriorated concrete, notify ENGINEER in writing. Allow one week for ENGINEER to evaluate the surface, perform testing for acid contamination if required, determine if additional concrete shall be removed, and to develop special repair details (if any) required. Should ENGINEER determine that additional concrete be removed to reach sound, uncontaminated concrete, allow another one-week period for further evaluation and testing following the additional removal. 3. Surface preparation shall conform to recommendations of repair mortar manufacturer. 4. Repair and rehabilitate isolated areas of exposed reinforcing bars in accordance the REPAIR OF SURFACE DEFECTS section. If extensive areas of reinforcing steel are uncovered after removal of deteriorated concrete, ENGINEER will determine the repair methods required. C. Repair Mortar Placing: 1. Conform to manufacturer’s recommended procedures for mixing and placing repair mortar. 2. After initial mixing of repair mortar, addition of water is not allowed. 3. Minimum Thickness: a. Install repair mortar to not less than minimum thickness recommended by manufacturer, and not less than 1/2-inch. b. Where removal of deteriorated concrete results in repair thickness of less than minimum required thickness to return to original concrete surface in isolated areas totaling less than 10 (ten) percent of total repair surface area, remove additional concrete to obtain at least the required minimum thickness. c. Where surface area with repair thickness less than minimum required thickness exceeds 10 (ten) percent of total repair area, notify ENGINEER. d. Provide repair mortar so that minimum cover over existing reinforcing steel is two inches. Do not place repair mortar creating locally raised areas. e. Where transitioning to or from wall surfaces not requiring repair, do not feather-out repair mortar at transition. Instead, form the transition by saw cutting a score line to not less than minimum required repair mortar depth and chip out concrete to the saw cut line. Do not cut or otherwise damage reinforcing steel. 4. Place repair mortar to an even, uniform plane to restore concrete member to its original surface. Out-of-plane tolerance shall be such that the gap between 12-inch long straight edge and repair mortar surface does not exceed 1/8-inch, and gap between a four-foot long straight edge and repair Version 10/31/2017 Page - 15 Public Works Quote mortar surface shall not exceed 1/4-inch. Tolerances specified in this paragraph apply to straight edges placed in any orientation at any location. D. Finishing: 1. Provide smooth, steel trowel finish to repair mortar. 2. When completed, there shall be no sharp edges. Provide exterior corners, such as at penetrations, one-inch radius. Interior corners shall be square. Crack Injection: A. Examine areas under which injection Work will be installed and locate cracks that require injection. Identify and inject cracks greater than 0.010-inch wide in structures that retain or contain water. B. Install injection material in accordance with crack injection manufacturer’s requirements. C. After injecting and curing, verify that injected material penetrated the crack adequately and that there is no visible leakage through the crack. After injecting, if crack continues to leak, re-inject crack at no additional cost to OWNER until structure is watertight. D. If proper penetration of crack cannot be achieved, submit to ENGINEER a proposed alternate approach for modifying the specified injection procedure to properly seal the crack. In concrete cracked as a result of CONTRACTOR’s operations, perform modifications to crack injection procedure and fully repair the crack without additional cost to OWNER or extension of the Contract Times. MEASUREMENT AND PAYMENT: Deep Concrete Surface Repair and Shallow Concrete Surface Repair shall be measured by the square foot at the exposed surface of the concrete repair. Once the repair depth reaches one-half of the members overall depth or deeper, it shall be measured as a Deep Concrete Surface Repair. Crack Repair and Joint Repair shall be paid for by the linear foot. The accepted quantities of Deep Concrete Surface Repair and Shallow Concrete Surface Repair shall be paid for at the contract unit price per square foot. Crack Repair and Joint Repair shall be paid for at the contract unit price per linear foot. The estimated volumes shown on the plans shall be used for to determine the unit price per pay item listed above. The mobilization and demobilization shall be paid for under Item XIX Mobilization and Demobilization and not this work item. Reinforcing steel will not be measured and the cost shall be included in the cost of other pay items in this section.
