Project Description: The Customer has requested the installation of electric distribution faciliLles
(hereinafLer referred to as "Basic Service") as Eollows: Relocate existing sing]e,
phase 120/240V 200A underground service per City A SB Waste WaLcr Dept,
Indiana MAchigan Power Company - IN (hereinafter rererred to as "Company") agrees to Cut existing
underground service, hard dig 10 feet, splice 35' of 110AI UT, abandon 10' of exisLing underground service,
wansfet existing meter N3000 to now cabinet.
Customer agrees to reimburse AEP for the relocation of the existing service per the Terms & Conditions of
Service (see attached)

Based on the custmner's preceding rep resenration, the ConTany estimates the 2-1/2 year Yevenue to be 0320.33.
Customer agrees to pay to the Company the potentially refundable amount of $0.00, which is the difference between
the Company's cost of installing the necessary facilities and the estdmated revenue credit. Tn addition, the.
customer agrees to pay as non-infundable amount of $530.38, kur any work requested that exceeds what is required to
piuvide adequate service.

Payment shall be made prior to the start of Company's construction ob facilities.
A. Recalculated Re nne - It the customer has not installed or contracted for the installation of the electr&,,
equipment as Mated in paragraph 13, Lhen the Company shalt recalculate the estimaLed revenue.
If the recalculated revenue is less Lhan the Company's cost of lac ilities, then the customer shall, within thirty
(30) ddYS Of ceceiv&g an invoice from the Company, pay to Cmapany as aid to construction, the difference between
the estimated revenue and the Company's cost of the tacilities . The fact that the customer may no longer be
associated with or have a properly interest in the project served under this agreement does not release the customer
from this obligation,
3. Cus-omer Refund - It the customer made an aid to construction payment, then the customer shall, after the number
of services used in the original revenue calculation has been instaLled, receive a refund for any additional
permanent customers utilizing the same line extension wiLhLn six (6) years from compieLion of the extension.
Refund, WIL be made for each additional purownent customer equal to 2-1/2 times the estimated annual revenue, less
Lhe estimated cost of service diop or laterals and metering equipment required to serve the new permanent
custower(s). The Mal of all refunds or allowances shall not exceed the total payment made by the cusLomei,

Company specifications.

abnormal conditions which are in excess of the original customer payment.
d. The customer hereby agrees Lao install an approved electric service entrance of sufficient
capacity for present requiremeuLs of the building to be erected on the property. Service shall nointally terminate
on the building exterior at a location suitabJe to the Company. Service entrance capacity shall he as indicated on
the ContribuL!on-In-Aid-Ot-ConsLruction Agreement For Electric Distribution Service,
u. The customer shall coordinate the installation at underground el.ectric facilities with other ut-ilities and
sorviros to permit efficient complotinn of Company's woKk, unimpeded acceoo to the installation site, and
installation of underground facilities at proper depth to avoid accidental diq-lns or other insta0aLlon prublems.
f. Customer undersLands that in order for Company to perform the above described work, Company (and/or its
contractor) may he required to excavate, dig, bore or bring large, heavy equipment onto the property. This presents
a danger to hidden underground styuctuies/taciAties. It is the customer's responsibilAy to locate and/or exposr,,,
all pAvately owned Krear-heal es/tac!Mies located within the propnsad work area. The Company (and/or its
cons racLur) will not ILSSUM Lespon sibility for damarle to water lines, drain tiles, septic systems, eLc., that have
not heen per identified and/or exposed by the customer.
Customei understands Lhat ali facilities installed by the Company will be and remain property Q the Company. The
Company expressly retains the right to use said lines and equipment LOL any purpose whi-ch Company deems advisable,
including the distribution cat alecLAw service Lu other customers.
Other KiliLieu may have lines and/or equipment Lhat utilize Company's pole or other facilities.Company is not
responsible for the AnstallaLion, relocation or removal of lines and/or equLpment owned by other uAliLies at this
location. Each utility is responsible foi its own equipment and the time it takes to insLall, remove or relacabo
it. My fees charged by other utilities are the responsibility of Lhe cusLamer, not Lhe Company. The customeL is
responsible for contacLing the other ut-ilities and making arrangements with them for any work that must be done to
facilitate this contract.
Nothing herein contained shall be construed as a waiver or relLnquishment by Company or any right it has or may
hereafter have to discontinue service for or an account at detault in the payment of any bill owing or to becomo
owLng thereunder or for any reason or cause staLed in the Company's Tariff.
It is agyeed and under sLood thaL, if Customer alLers the terms of this Agreement, it may be voided at the
discretion of the Company.
It is iuzLher understood and agreed that should circumstances, on the part of either the Company or the customer,
delay rendering service beyond 90 days after this AgueeuRnt is signed, the torms of Uds agreement may bo
renegoLialed at the request Q either. pauty.

Terms of this agreement begin upon Company's completion of the line extension
Binding Authority - The individual executing this Agieement hereby war ranLs to the Company that he or she has full
authority to do so and has authority to bind the customer.
CusLomer agrees to accept abovo dosribed Leans and condihions and larhear agrees WaL the Company shatl not be
liable fur any claim at loss, injury, or damage, caused by the installation of service to the customer or for wark,
on behalf of customer under this AqTeemont except that which results from the negligence of the Company.

Where service is supplied from an underground distribution system which has been installed at
the Company's expense, the customer shall make arrangements with the Company for the Company
to install a continuous run of cable conductors, including necessary ducts, from the manhole or
connection box to the meter location. Where it is necessary that the location of the meter be inside the
customer's building, the customer shall reimburse Company for the cost of the portion of cable and
duct from the exterior building wall to the meter location; however, all right and title to the cable shall
remain with the Company.
10. Relocation of Company's Facilities at Customer's Request.
Whenever, at customer's request, the Company's facilities are relocated solely to suit the
convenience of customer, the customer shall reimburse the Company for the entire cost incurred in
making such change, including any and all required engineering studies.
11. Company's Liability.
The Company will use reasonable diligence in furnishing a regular and uninterrupted supply of
energy, but does not guarantee uninterrupted service. The Company shall not be liable for damages
in case such supply should be interrupted or fail by reason of an act of God, the public enemy,
accidents, labor disputes, or orders or acts of civil authority. Further, the Company shall not be liable
for damages in case such supply should be interrupted due to causes or conditions beyond the
Company's reasonable control, including extraordinary repairs, breakdowns or injury to machinery,
transmission lines, distribution lines, or other facilities of the Company when the Company has carried
on a program of maintenance consistent with the general practices prevailing in the industry. Further,
the Company shall not be liable for damages for interrupting service to any customer whenever, in the
judgment of the Company, such interruption is necessary in order to prevent or limit any instability or
disturbance on the electric system of the Company or any electric system interconnected with the
Company, such interruptive action to be taken in accordance with a predetermined plan and only in
situations that threaten massive curtailments of service on the Company's system. Notwithstanding
any other provisions of the terms of these Tariffs and Terms and Conditions of Service, the Company
may shut off service temporarily for reasons of health, safety, maintenance of Company facilities,
infrastructure improvements, and new construction of Company facilities. To the extent possible, the
Company will make a reasonable attempt to inform all affected customers in advance of such events.
Unless otherwise provided in a contract between Company and customer, the point at which
service is delivered by Company to customer, to be known as "delivery point," shall be the point at
which the customer's facilities are connected to the Company's facilities. The Company shall not be
liable for any loss, injury, or damage resulting from the customer's use of customer -owned equipment
or occasioned by the energy furnished by the Company beyond the delivery point.
