Description ofModications: [NOTE TO USER: Include the following paragraphs that are appropriate and delete those not applicable to this amendment. Refer to paragraph numbers used in the Agreement or a previous amendment for clarity with respect to the modifications to be made. Use paragraph numbers in this document for ease of reference herein and in future correspondence or amendments.] a. Engineer shall perform or furnish the following Additional Services: The initial project scope for this On -Call Survey/Right-of-Way Services agreement is to have JPR under contract to perform various survey tasks as requested by the City of South Bend, Board of Public Works. Several projects have been assigned and have either been completed or partially completed. The following are a list of projects that have either been completed or are currently being worked on: During the design process the following items have been added to the initial project scope: 1) Portage -Boland R/W Acquisition (COMPLETE): JPR originally prepared Right - of -Way Engineering Documents for the roundabout at Portage Avenue/Boland Drive on July 6, 2018. Subsequently a request to prepare a Deed of Dedication Plat for said area was prepared and submitted on August 15, 2019 2) McKinley Avenue Topographic Survey (COMPLETE): JPR was requested to prepare a topographic survey for a portion of McKinley Avenue east of Ironwood Drive where the north edge of pavement was eroding. 3) 4502 South Michigan Street Easement (COMPLETE): JPR was requested to prepare an easement for the City's existing sanitary sewer located on private property. All rights reserved. 4) Privet Lane Topographic Survey (COMPLETE): JPR was requested to prepare a topographic survey for the failing structure located on Privet lane. 5) Laurel Woods Structure Survey (COMPLETE): JPR was requested to collect existing structure data for various structures in the Laurel Woods area for determining issues with existing storm drainage. 6) Nimtz Parkway Boundary Survey/Easement Preparation (CURRENT/ACTIVE): JPR was requested to prepare a boundary survey and easement description for a City owned parcel for the Nimtz Parkway Retention Basin. This is an active project. 7) Drainage Request areas (CURRENT/ACTIVE): topographic surveys requested for the following areas to correct drainage issues: Marion Street (Williams St. to Lafayette Blvd.), Fox Street (High St. to Dale Ave,), Beale Street (Inglewood Pl. to Sherman Ave.), Huey Street (Pershing St. to Bergan St.), Elmer Street (Pershing St. to Bergan St.), and Kaley Street (Right-of-way to drainage ditch) b. The Scope of Services currently authorized to be performed by Engineer in accordance with the Agreement and previous amendments, if any, is modified as follows: c. The responsibilities of Owner are modified as follows: d. For the Additional Services or the modifications to services set forth above, Owner shall pay Engineer the following additional or modified compensation: Construction Documents $21,500.00 Total Additional Services Request $21,500.00 e. The schedule for rendering services is modified as follows: f. Other portions of the Agreement (including previous amendments, if any) are modified as follows: 3. Agreement Summary (Reference only) a. Original Agreement amount: $10,000.00 b. Net change for prior amendments: $6,000.00 c. This amendment amount: $21,500.00 d. Adjusted Agreement amount: $37,500.00 Please note that the basis of payment and hourly rates set forth in the original Professional Services Agreement dated June 12, 2018 is applicable to this amendment.
