AMENDMENT TO OWNER -ENGINEER AGREEMENT Amendment No. 1 1. Background Data: a. Effective Date of Owner -Engineer Agreement: March 26, 2019 b. Owner: City of South Bend acting by and through the Board of Public Works C. Engineer: DLZ Indiana, LLC, 2211 East Jefferson Blvd. South Bend, Indiana 46615 d. Project: DOUGLAS ROAD LIFT STATION RELOCATION 2. Description ofModications: a. Engineer shall perform or furnish the following Additional Services: 1) Add the following to the end of the Project Description of Engineer's Services: Professional engineering services for the City's relocation of approximately 275 lineal feet of 12" water main due to conflicts with the Douglas Road Reconstructions Project Des. No. 1400639 project, which includes added travel lanes and bridge reconstruction from Ivy Road to S.R. 23. The 12" water main will be relocated in the segment in the immediate vicinity of Juday Creek. The existing water main is in conflict with the proposed bridge crossing of Juday Creek. The relocation of the 12" water main will include an approximate 75-foot segment that will be above grade and supported on the bridge (near the road elevation). The above grade water mains will include heat trace, insulation and a hanger system to suspend the watermain on the side of the bridge deck. b. The Scope of Services currently authorized to be performed by Engineer in accordance with the Agreement and previous amendments, if any, is modified as follows: 1) ADD Item A.2.e in Exhibit A Paragraph A1.02 Final Design Plans as follows: e. Water Main Design 1) The relocation of the approximate 275 ft segment of 12" water main will be incorporporated into the road reconstruction plans, of which 75 ft will be above grade and supported by the bridge structure.
