26 of the St. Joseph County Code.
The issuance of this certificate does NOT in any manner, release
the recipient from the responsibility of complying with the requirements
of the zoning ordinances, building codes, safety codes, ADA or other
requirements of the City of South Bend, the County of St. Joseph, the
State of Indiana, or the United States Federal Government.
This certificate is good for one year from the date of issuance and
is effective from the date entered herein. Plans are on file and open for
public inspection at the office of the Historic Preservation Commission of
South Bend and St. Joseph County, 227 West Jefferson Blvd., Suite 1400

The current roofline millwork is completely deteriorated; multiple large
openings are present, allowing wildlife to enter the attic. Original millwork will
be replicated in cedar, treated, painted, and installed by North Coast Window
Works as described in their proposal.
When the millwork replacement is complete, gutters will be reinstalled.

Dear Kevin,
In accordance with your request and our site review with you on Thursday May 28th,
North Coast Window Works, Inc. (NCWW) is please to provide you with this proposal.
As per your request North Coast Window Works, Inc is prepared to do the following:
Shop Work:
1. Build 2 casement windows, including inward swing 2 lite sash, to replace existing
dormer windows in attic.
2. Replicate one, 6/1 light, sash set for the north gable attic window. Single hung
configuration.
3. Replicate two, fixed one light sashes for the south gable attic window
4. Replicate one, 11/4 circle top sash set for the south gable attic window.
5. Build one basement window, one 3 light sash fixed into position, for the east
basement window.
Sills will be made from Accoya. Sashes and jambs will be made from Northern White
Pine, primed on all sides with 1/8” glass bedded and glazed in place.
Site Work:
1. Install all new sashes and windows.
2. Paint strip south exterior attic casing.
3. Replace sill on south double hung window.
4. Prime and finish paint installed windows and woodwork.
5. Seal perimeter of window casing to the siding with caulk.
6. Re-glaze basement windows and fix sashes in place.
Other work not included in budget at this time.
Measure, order, and install storm windows with screens for attic windows.
Mill and install crown molding and cove molding to the soffett.
Scope of Proposal:
Please keep in mind the proposal listed above is offered as a general budget towards the
work you requested. The proposal of repairs is based on limited visual inspection of the
existing windows. Concealed conditions may exist, which may alter the budget above.
Removal of the sashes from the jambs and removal of paint and glazing from the sashes
may expose other areas in need of repair. Repairs, if required will be handled through the
change order form.
The site work will require scaffolding &/or man lift. The charges for these will be include
on the final invoice.
If accepted, the sequence of work would be to build all the new windows and sashes in
the shop first. Mill the required amount of wood for the soffett repairs and prime. Once
completed the on-site work will be scheduled.

Insurance: General Liability, $1,000,000
Automobile Liability for Company Owned Vehicles, $500,000

Handling of Hazardous Material: Lead Based Paint
• NCWW assumes paint on & around windows contain some lead based paint and
therefor take precautions specified by the Environmental Protection Agency,
EPA. Renovate Right booklet is provided to you so you can ensure our practice is
of a high standard.
• All employees are certified or trained using standards published by the
Environmental Protection Agency (EPA) and Occupational Safety And Health
Administration (OSHA) for Repair, Restoration, and Painting. Copies of
certificate are available on owners request.
• NCWW, Inc is not responsible for detecting, analyzing, confirming the presence
of lead based paint & asbestos in plaster or glazing.
• NCWW, Inc. is not responsible for testing for the presence before or after of lead
dust levels in the immediate and immediate surrounding work areas. NCWW,
Inc. will show documentation of a “clean sweep test” as per EPA regulations.
• NCWW, Inc will provide to you, at your request, a copy of the Material Safety
and Data Sheet (MSDS) for any product or material used on this project..
Disposal: All material will be removed from the site and disposed of properly.
Warranty: Two year labor & materials. NCWW, Inc will not be responsible for the
repairs and work done by others and any such repairs shall void NCWW’s warranty on
those locations. Under no circumstance will NCWW, Inc. be liable for loss of use of
such property, or for incidental, general or consequential damages of any kind
whatsoever.
Exclusions: Permits or waivers for side walk encroachment if necessary. Certificates of
appropriateness from the local historic district.

The budget for the project follows:
Shop work, Building windows and sashes…..$4,600.00
Milling new crown and cove molding……na.
Site work is completed on a time and material basis. Labor charges at $55/person/hour
and Materials at cost plus 15%.
We estimate 6 days, 2 trades people plus staging, plus materials fir installation. The
estimated budget is $6,280.00. You are responsible for only the actual time and materials
to complete the project to your satisfaction.
A one-time fee of $2,170.00 (20% of estimate) is added to account for site visits,
contracting, supervision & overhead.
Billings for the shop work are 50% down payment plus the one time administrative and
overhead charge. Net due 15 days after completion.
Billings for the on site work begin upon commencement of work. Invoices will be mailed
bi-weekly. Net due with in 15 days.
After the due date, a finance charge of 11/2% per month will be added to past due
balances. (18% annual rate)
