Description of Modifications: [NOTE TO USER: Include the following paragraphs that are appropriate and delete those not applicable to this amendment. Refer to paragraph numbers used in the Agreement or a previous amendment for clarity with respect to the modifications to be made. Use paragraph numbers in this document for ease of reference herein and in future correspondence or amendments. a. Engineer shall perform or furnish the following Additional Services: American Structurepoint will provide the construction administration services required to assist the City of South Bend required to complete the plaster repairs at the Morris Performing Arts Center. b. The Scope of Services currently authorized to be performed by Engineer in accordance with the Agreement and previous amendments, if any, is modified as follows: • Provide one site visit to inspect plaster installation mock-up at plaster contractor's facility with follow-up report of findings. • Provide no more than two inspections of the dome plaster to determine the extent of repairs required. For the purpose of this proposal it is assumed one inspection will occur during scaffold installation to keep on schedule and another inspection once scaffold construction is complete. • Communication and coordination as required with contractors during repairs. • Provide one final inspection of repairs upon completion. e.
