We would like to request an equitable adjustment for additional services in the amount of $4,500. This modification will include: • Structural review and services • Mechanical/Electrical/Plumbing redesign services • Architectural redesign services • Bidder Inquiry and submittal review o Review substitutes/"or approved equals" with the Owner • Attend Pre-bid/Pre-Construction meetings (if required) • Final punchlist Excluded services for this modification: • Site visits (Owner will provide day today site visits per original contract) • Weekly Contractor meetings on site Please review this change order and let me know if you have any questions.
