(the "Agency" or "Contractor"). This award is granted under CFDA 14.218, Community Development Block Grant Program, U.S. Department of Housing and Urban Development, Office of Community Planning and Development, Grant Number B-18-MC-18- 0011. WHEREAS, the City and Participant are parties to an agreement entitled "Community Development Program Contract" dated Jmuile 25�,,_2019y and made effective .May, 1 2O1 9 ("CDBG Contract"); and WHEREAS, this agreement is entered into as the Amendment to the CDBG Contract to modify certain terms and conditions expressed in the CDBG Contract; and NOW, THEREFORE, in consideration of the mutual promises and obligations set forth herein, the parties make this Amendment to the CDBG Contract as follows: (1) APPENDIX I entitled "SCOPE OF SERVICES / PROJECT DESCRIPTION" which is attached to and incorporated in the CDBG Contract, shall be and is hereby amended and the following sections are revised and replaced as follows: COST CATEGORIES Generally, costs must be necessary, reasonable and directly related to the grant. In accordance with the Statement of Purpose and Intent, certain eligible cost categories established include: • Partial cost of salaries for Project Director and Program Manager, based on hourly rate/position and hours related to management of Lead Outreach Program, including marketing events and applicant time; • Partial cost of salaries for two (2) part-time certified Community Health Workers that are conducting program/street level outreach, based on hourly rate/position and hours dedicated to Lead Outreach events, direct applicant work, marketing, and training; • Fringe benefits for all positions noted above, equal to 7% of work totals; • Office supplies not to exceed $375 based on approved list provided; • Postage and printing specific to outreach materials; • Monthly services charges for three (3) phones used by Community Health Workers; and, • Community Health Worker travel, hotel, and meals related to training for lead -based paint hazards.
