AGREEMENT FOR PROFESSIONAL SERVCIES BY AND BETWEEN THE CITY OF
SOUTH BEND, INDIANA AND NEIL MIHALICH
'I 11IS FIRST ADDENDUM (this "Addendum") is made and entered into this 104'day

corporation20_ff, by and between the CITY OF SOUTH BEND, INDIANA, a
municipal organized and operating under the laws of the State of Indiana, acting by
and through its Board of Public Works (the "City"), and NEIL MIHALICH, an individual
("Contractor").
RECITALS
WHEREAS, the City and the Contractor have entered into an Agreement for
Professional Services By and Between the City of South Bend, Indiana and Neil Mihalich, dated
January 22, 2019 (the "Agreement"), providing for assistance in managing a program to address
lead in homes in the City and educate the community regarding the hazards of lead paint and
childhood poisoning; and
WHEREAS, the parties desire to amend and extend the Agreement by this Addendum;
and
WHEREAS, the City believes that the actions contemplated by this Addendum are in the
best interests of the health, safety and welfare of the City and its residents and that those actions
comply with the public purposes and applicable provisions of state and local law.
NOW THEREFORE, the parties mutually agree as follows:
1. In Section 3, Term and Renewal Option, the phrase "December 31, 2019" shall be
deleted and replaced with "December 31, 2020."
2. The parties agree that all other terms and conditions as set forth in the Agreement
shall remain in full force and effect.
