We are a family owned business, located in South Bend for 97 years now. We are dedicated to providing on time, professional, plumbing, heating and air-conditioning service. We are a union business with qualified, professional technicians that are fully trained and licensed. Niezgodski Plumbing Inc.'s technicians have expertise in plumbing service repair and replacement. We specialize in repair and replacement of the following items but are not limited to these: 1. Water heaters 2. Water Softeners 3. Water closets, sinks, faucets 4. Reverse Osmosis systems 5. Testing and installation of Backflow prevention devices 6. Boilers 7. Underground repair and installation, sewer and water 8. Backwater valves 9. Water and drainage system repair 10. Sewer televising and cleaning 11. Thawing frozen water lines Niezgodski Plumbing Inc. currently has 16 employees divided between office and shop. Our annual volume of projects is approximately 1200. Our clients include the City of South Bend Water Works and the Bureau of Sewers. We have been performing emergency water works repairs for South Bend Water Works for 47 continuous years. In addition we test and repair as called upon all emergency shower and eyewash devices located on Campus for the University of Notre Dame. We have been providing this service to Notre Dame for four years now and will continue to do so once again for the year 2020 beginning this coming January. There are in excess of 1000 of these devices located on campus. We are capable of securing all liability insurance and performance bonding as needed for this RFP. Enclosed please find our current insurance certificate that Niezgodski Plumbing Inc. carries with the City of South Bend and the University of Notre Dame to be used as examples. Niezgodski Plumbing Inc. performs service during regular and emergency after normal working hours. We have always been dedicated to be on site as required for emergency repairs. Niezgodski Plumbing has performed these repairs for South Bend City Water Works for 28 years as Niezgodski Plumbing Inc. When the Business was known as Niezgodski & Sons Plumbing and Heating we performed the same work for a period of 19 years for a combined total of 47 continuous years of service to South Bend City Water Works. w 2"d opinions with camera and underground sewer repairs as called upon. Regular hours and emergency repairs as required. Niezgodski Plumbing has been a part of this program for numerous years but consecutively for at least four years at this time. Niezgodski Plumbing Inc. on an annual basis is responsible for testing emergency shower and eyewash devices for the entire Campus at the University of Notre Dame. We will begin our 5t" continuous year of testing these devices in January of 2020. In 2019 there were 1029 devices tested on campus. In addition Niezgodski Plumbing Inc. performs repairs and installation of these devices as directed. 0 Services Cost Information Emergency Double Time Hourly Rate (licensed) $175.00 Emergency Double Time Hourly Rate (non -licensed) $128.00 Applicability for Emergency Double Time Hourly Rate From 12:00 midnight until 8AM the rates shall be 2 times the daily working non -emergency hourly rate. Work performed on Saturdays between the period beginning at 513M until 8AM on Monday the rate shall be two times the daily working non -emergency hourly rate. Work performed on those days officially celebrated as the following Holidays: New Year's Day, Memorial Day, Fourth of July, Labor Day, Thanksgiving Day and Christmas Day shall be paid for at two times the daily working non -emergency hourly rate. When any of the above Holidays falls on a Saturday, it will be observed on Friday. When it falls on a Sunday, it will be observed on a Monday. Version 1/17/2019 Specification and Service Requirements Niezgodski Plumbing Inc., will have the ability to troubleshoot all plumbing issues which will be required under the specifications of this RFP that fall under General Facility Plumbing Repairs. We will coordinate these repairs at all times with COSB staff in an efficient, professional and timely manner. Niezgodski Plumbing Inc., has the capacity and ample room to grow in order to absorb the increased volume of plumbing requests originating from COSB • Material markup shall not exceed 10% • At all times every effort shall be made to use replacement parts generally and readily available on the plumbing market. At times certain items may have to be special ordered because of the age of existing plumbing fixtures or appliances. Niezgodski Plumbing has numerous business vendor accounts, all in good standing. These vendors provide a wide range of choices necessary in order to locate specialty items that may be called for from time to time. Professional workmanship is the end goal for the completion of every job. At all times Niezgodski Plumbing Inc. will coordinate all information prior to any commencement of work. Work will not proceed before proper approval is provided by COSB staff • Hourly service rate is the only labor rate which applies to this RFP • Niezgodski Plumbing shall at all times coordinate access to facilities with appropriate COSB representatives listed in Appendix 2 of this RFP • All work shall be performed in a professional manner according to the standards of excellence of the plumbing industry • All work shall conform to all applicable local, state and federal codes. o Permits necessary to perform work shall be the sole responsibility of the contractor and will be itemized on all invoices • Niezgodski Plumbing Inc., shall inform all employees performing work that all COSB facilities are smoke -free. • Niezgodski Plumbing's vehicles are all clearly marked with the name, address, phone number and license number of our business. • Niezgodski Plumbing employees all have appropriate uniforms that identify our company name and employee's name. Any other required name badge or other form of identification with credentials required also shall be worn. • Niezgodski Plumbing Inc. will report on COSB plumbing work orders on a quarterly basis as required. Reports shall include: Total volume and value of work orders, Volume and value of work orders by COSB department/division, Volume and value of work orders by type (non - emergency vs. emergency, Average and median time to close a work order. • Reports shall be delivered no later than the 15th day of the first month following the close of the standard calendar quarter. Reports shall be due the following business day if the 15th day lands on a Saturday, Sunday, or holiday • Niezgodski Plumbing Inc shall provide service within two hours of the time of the call • These provisions shall apply twenty-four hours a day seven days a week When the prospective Contractor is unable to certify to any of the statements below, it shall attach an explanation to this Affidavit. For purposes of this Certification, "Iran" means the government of Iran and any agency or instrumentality of Iran, or as otherwise defined at Ind. Code § 5-22-16.5-5, as amended from time -to -time. b. As provided by Ind. Code § 5-22-16.5-8, as amended from time -to -time, a Contractor is engaged in investment activities in Iran if either: i. Contractor does not knowingly employ or contract with an unauthorized alien, nor retain any employee or contract with a person that the Contractor subsequently learns is an unauthorized alien. Contractor agrees that he/she/it shall enroll in and verify the work eligibility status of all of Contractor's newly hired employees through the E-Verify Program as defined by I.C. 22-5-1.7-3. Contractor's documentation of enrollment and participation in the E-Verify Program is included and attached as part of this bid/quote; and 5. Contractor shall require his/her/its subcontractors performing work under this public contract to certify that the subcontractors do not knowingly employ or contract with an unauthorized alien, nor retain any employee or contract with a person that the subcontractor subsequently learns is an unauthorized alien, and that the subcontractor has enrolled in and is participating in the E-Verify Program. The Contractor agrees to maintain this certification throughout the term of the contract with the City of South Bend, and understands that the City may terminate the contract for default if the Contractor fails to cure a breach of this provision no later than thirty (30) days after being notified by the City. 6. Persons, partnerships, corporations, associations, or joint venturers awarded a contract by the City of South Bend through its agencies, boards, or commissions shall not discriminate against any employee or applicant for employment in the performance of a City contract with respect to hire, tenure, terms, conditions, or privileges of employment, or any matter directly or indirectly related to employment because of race, sex, religion, color, national origin, ancestry, age, gender expression, gender identity, sexual orientation or disability that does not affect that person's ability to perform the work. In awarding contracts for the purchase of work, labor, services, supplies, equipment, materials, or any combination of the foregoing including, but not limited to, public works contracts awarded under public bidding laws or other contracts in which public bids are not required by law, the City, its agencies, boards, or commissions may consider the Contractor's good faith efforts to obtain participation by those Contractors certified by the State of Indiana as a Minority Business ("MBE") or as a Women's Business Enterprise ("WBE") as a factor in determining the lowest, responsible, responsive bidder. In no event shall persons or entities seeking the award of a City contract be required to award a subcontract to an MBE/WBE; however, it may not unlawfully discriminate against said WBE/MBE. A finding of a discriminatory practice by the City's MBE/WBE Utilization Board shall prohibit that person or entity from being awarded a City contract for a period of one (1) year from the date of such determination, and such determination may also be grounds for terminating the contact for which the discriminatory practice or noncompliance pertains. 7.
