workmanship, and pay for any damage to other work resulting therefrom, which shall appear within a period of three (3) year(s)
from the date of substantial completion of the work provided for in the Contract, then this obligation to be void; otherwise to
remain in full force and effect.
PROVIDED, HOWEVER, that Owner shall give Contractor and Surety notice of observed defects with reasonable promptness.

The Division of Engineering and Division of Utilities request the Board of Public Works' approval of
the Project Completion Affidavit for Project No. 117-026 Wastewater Treatment Plant Headworks
Influent Gates Improvements. All work is complete and all new influent gates are operational.
The Original Contract Sum: $966,500.00
Final Project Cost as a result of previously approved Change Orders: $635,731.06
Substantial completion was established on schedule. Seeding and restoration efforts were completed
in the Spring when weather was acceptable for seed application. All punch list items are complete.
This project is completed with a total cost savings of $330,768.94 in comparison to the original bid
price of $966,500.00. This was a result of a materials change from cast iron gates to stainless steel
gates. Construction was completed for a total cost of $635,731.06 (-34.22%).
The funding source for the project is Wastewater, Capital Account (642-0630-415-43-66). If there are
