This encroachment was approved subject to the improvements not becoming a safety hazard, proper occupancy and excavation bonds be obtained along with permits, and a copy of any reports generated be sent to the City. Enclosed please find a copy of the permit. The encroachment will be permitted to remain and continue until such time as the Board of Public Works of the City of South Bend shall determine that such encroachment is in any way impairing or interfering with the highway or with the free and safe flow of traffic thereon. Said encroachment shall not in any way prejudice or preclude the City's rights with respect to the future use of that portion of the right-of-way. Groundwater & Environmental Services, Inc. (GES) has been contracted by Honeywell International Inc. (Honeywell) to provide operation, maintenance and monitoring services for the groundwater extraction systems that operate to control groundwater at the Honeywell Plant Complex (Site) in South Bend, Indiana. After 20 years of operation existing remediation well EW-3 has failed and needs to be replaced. The new replacement remediation well (EW-3r) will be hooked up to existing infrastructure to allow for extracted groundwater to be discharged under Site Remediation Permit No. 004 issued by Department of Public Works, City of South Bend to Honeywell on April 14, 2015. GES has prepared the following required forms for a new remediation well adjacent to the Site in road easement between sidewalk and Bertrand Street: BPW Encroachment Application; • Long -Term Occupancy Request Application; and • Excavation Permit Request Form.
