The Board of Public Works, at its meeting held on April 9, 2019, approved the above
referenced Change Order for an increase of $3,601.27 plus an additional ten (10) days,
bringing the current contract amount to $242,239.27 and the revised project completion date
to April 12, 2019.
Enclosed is a copy of the Change Order for your records.

We are now $3,273.88 over the $5,000 allowance amount. We are requesting the
following: a
Change order for $3,273.88
10% Oh/P 327.39
Total change $3,601.27
We are also asking for a time extension of eight (8) days beyond the specified 90
days because of the time to complete the drainage pipe video, the concrete floor issue
needing metal centering, the time when the City offices were closed because of sub -zero
weather and we could not get inspections, the concrete plants were closed and the boat
and RV shows that would not allow us to move materials thru the great hall.
We expect the carpet to be onsite by April 5th with install taking until April 121n
