Contractor agrees that he/she/it shall enroll in and verify the work eligibility status of all of Contractor's newly hired employees through the E-Verify Program as defined by I.C. 22-5-1.7-3. If a chemical needs to be transferred for its original container, the new container will be properly labeled. - The only exception to this is if the chemical is being transferred to a temporary container for immediate use of the employee that performs the Transfer. If a container is received or has become unmarked than this container will not be used or dispensed until it is properly marked to ensure the correct use of its containing. Safety data sheets - SDS • EGI will have the appropriate SDS for each and every hazardous chemical used by any EGI employee. • These SDS Sheets will help to identify the health and physical hazards of the chemicals that our employees are working with. The foremen will notify • All other employees of the contents within this packet and will make all information available at all times. • The foreman will also notify the employees of any updates to the SDS packet, this is done to ensure that all employees have been updated on the potential health hazards. • Employees will be advised on how to access SDS sheets upon their initial training. They will be updated during regular toolbox talks. Safety data sheet sections • Section 1 - manufacturer information This section lists the names, addresses and emergency telephone numbers of the manufacturer, distributor, or company that prepared it, as well as the date it was prepared. The identity of the chemical and the chemical family will be listed. • Section 2- hazardous ingredients This section lists the hazardous ingredients in a mixture and the common names of individual chemicals. It also provides property exposure and threshold limit values of the ingredients. • Section 3- chemical data This section provides information on the specific properties of the chemical plus a physical description of the material. • Section 4 - Physical hazard data This section alerts individuals to any specific physical hazards associated with the product. It further describes special firefighting techniques and any unusual fire hazards associated with the product. • Section 5- spill or leak procedures This section describes steps to be taken in case of the material is released or spilled. • Section 6 - Exposure control information This section outlines how exposure to chemicals can be controlled by the use of personal protective equipment. It includes the appropriate engineering controls and effective work practices. • Section 7 - Special precautions This section explained special precautions that should be taken during storage and handling of the chemical. Employee training It is Environmental Glass's policy that the project foreman will provide employees the information and training on hazardous chemicals in their work area at the time of their initial assignment. The foreman will update this information as needed • Employees are to be informed about the following. 1) The requirements of 29 CFR 1910.1200 2) Any operations in their work area where chemicals are present. 3) The location and availability of the "up to date" hazard communication program including the required lists of hazardous chemicals and safety data sheets. • Training 1) Methods and observations that may be used to detect the presence of release of a hazardous chemical in the work area 2) The physical and health hazards of the chemicals in the work area. 3) The measures that employees can take to protect themselves from these hazards. This is to include specific procedures to protect employees, such as appropriate work practices, emergency procedures and personal protective equipment to be used. 4) The details of the hazard communication program that we have developed • Before starting work each new employee will be given information on the following. 1) Chemicals and hazards in their workplace. 2) How to lesson or prevent exposure to these chemicals. 3) What we do to lesson exposure. 4) Procedures to follow if exposure occurs. General EGI construction safety rules Policy Each employer is responsible to use the means of prevention that he or she has been trained in. employees shall be trained in, and follow all provisions and safety laws to prevent accidents or injuries. Fall R 2acti n • Traluln (1) EGI shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. 2) EGI shall assure that each employee has been trained, as necessary, by a competent person qualified in the following areas: • The nature of fall hazards in the work area; • The correct procedures for erecting, maintaining, disassembling, and inspecting the fall protection systems to be used; • The use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used; The role of each employee in the safety monitoring system when this system is used; • The correct procedures for the handling and storage of equipment and materials • The role of employees in fall protection plans; Unprotected sides and edges. Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Leading Edges. (i) Each employee who is constructing a leading edge 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, or personal fall arrest systems. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan. (ii) Each employee on a walking/working surface 6 feet or more above a lower level where leading edges are under construction, but who is not engaged in the leading edge work, shall be protected from falling by a guardrail system, safety net system, or personal fall arrest system. If a guardrail system is chosen to provide the fall protection, and a controlled access zone has already been established for leading edge work, the control line may be used in lieu of a guardrail along the edge that parallels the leading edge. Hoist areas. Each employee in a hoist area shall be protected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems or personal fall arrest systems. If guardrail systems, [or chain, gate, or guardrail] or portions thereof, are removed to facilitate the hoisting operation (e.g., during landing of materials), and an employee must lean through the access opening or out over the edge of the access opening (to receive or guide equipment and materials, for example), that employee shall be protected from fall hazards by a personal fall arrest system. Holes." (i) Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. (d) Each employce on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers. (iii) Each employee on a walking/working surface shall be protected from objects falling through holes (including skylights).by covers. "Ramps, runways, and other walkways." Each employee on ramps, runways, and other walkways shall be protected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems. "Dangerous equipment." (i) Each employee less than 6 feet (1.8 m) above dangerous equipment shall be protected from falling into or onto the dangerous equipment by guardrail systems or by equipment guards. (ii) Each employee 6 feet (1.8 m) or more above dangerous equipment shall be protected from fall hazards by guardrail systems, personal fall arrest systems, or safety net systems. • Wall openings. Each employee working on, at, 6 feet or above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening is 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 m) above the walking/working surface, shall be protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system. "Walking/working surfaces not otherwise addressed." Except as provided in 1926.500(a)(2) or in 1926.501 (b)(1) through (b)(14), each employee on a walking/working surface 6 feet (1.8 m) or more above lower levels shall be protected from falling by a guardrail system, safety net system, or personal fall arrest system. Personal Protective E ui mmen.t • Environmental glass, Inc. employees shall wear clothing and footwear which is safe for the jobsite • Safety boots are required at all times with no exception • Hard hats, glasses and gloves are available on all job sites and should be worn in order to protect employees from accidents • Metal hard hats are prohibited • Bracelets, dangling jewelry, loose clothing, and neckties present a potential hazard and are prohibited. • Long hair is prohibited on the job sight unless it is up and properly secured and unable cause a hazard. • Safety harnesses must be used in accordance with working on elevated areas when necessary. Refer to scaffold and ladder section • If hazard cannot be controlled by means of engineering then we require all employees to use their training to acquire the proper means of PPE Electrical Hazards 9 On most sites the potential hazard of electricity applies • The improper use of power is one of the most common hazards. • Wet environments and improper grounding greatly increase the potential for electrocution. • Prior to beginning a project the site shall be inspected and any wiring fault shall be noted and recorded. • These faults can be detected be means of testers. Any issue shall be either fixed or put out of service prior to commencing work. • When possible circuits that will not be used should be locked out • Ground Fault Circuit Connectors shall be uses no matter what. • An equipment grounding program will be used on all sites. This will require inspection of all tools, cords and electrical devices with written documentation. • All power tools shall be inspected regularly for damage, proper grounding, and integrity of the installation. • Nonmetallic tools should be used for scrapping to prevent possible electrical shock • Workers will be furnished with insulated rubber boots and gloves when working around energized wiring or equipment. • Try to avoid leaving wires and cords on the ground where they could cause a potential trip hazard or become wet and be a potential hazard. • Elevate temporary wiring when possible and keep it away from potential damage. • Ensure electrical outlets are sealed tightly and use wooden or fiberglass ladders • Determine operating voltages of equipment and lines before working on or near energized parts to determine safe and compatibility with the equipment to be used. • Extension cords must be of three- conductor type and must be connected to the ground fault circuit interrupter. • Portable electrical hand tools should be equipped with a three conductor cord in other word the will be grounded tools. • Circuits to be de energized should be clearly identified and isolated from all energy sources. • A designated employee will provide notification that all switches and disconnections that could supply energy have been de energized, locked out and tagged out. • Ground fault circuit interrupters. All 120-volt, single phase 15 and 20 ampere receptacle outlets used for construction operations, which are not a part of the permanent wiring of the building or structure and which are in use by an employee, shall have approved ground fault circuit interrupters for personnel protection. A receptacle on a 2-wire, single phase portable or vehicle mounted generator rated at not more than 5 kilowatt, where the circuit conductors of the generator are insulated from the generator frame and all other grounded surfaces, need not be protected with ground fault interrupters. ,adders and Scaffolds Training EGI shall provide a training program for each employee who uses a ladder. The program shall enable each employee to recognize hazards related to the ladder and shall train each employee in the procedures to be followed to minimize these hazards. We shall ensure that each employee has been trained by a competent person in all of the following areas, as applicable: (a) The nature of fall hazards in the work area. (b) The correct procedures for erecting, maintaining, and disassembling the fall protection systems to be used. (c) The proper construction, use, and placement of, and care in handling, ladders. (d) The maximum intended load -carrying capacities of ladders that are used. (e) The rules contained in this part. Retraining shall be provided for each employee as necessary so that the employee maintains the understanding and knowledge acquired through compliance with these rules. • Defective ladders must be tagged immediately, repaired or discarded • Be sure that safety feet are in good condition. Missing safety feet will cut polyethylene floor covering and make the ladder unsafe. • Keep rungs free of build up, dirt grease, oil, paint • Extension ladders shall be used with a 1 to 4 ratio • No metal ladders are permitted • Extension and straight ladders shall be equipped with ladder feet. • Ladders shall be inspected for defects prior to each use • Ladders should be stored on a flat surface in order to prevent it from becoming bowed or warped. When going up and down the ladder be sure to always have three points of contact. Face the ladder and grip the side rails Do not over reach when on a ladder, keep between the side rails. Always tie off when applicable - When using ladders in front of a door way or passage way the opening should be barricaded - Ladders must have a solid footing. Do not place on boxes or unstable footings - Do not use ladders as platforms of scaffolding planks - Use all ladders with their engineered design do not alter a ladder or use stepladders as straight ladders. Do not stand on the top two steps of a ladder �am' The training shall enable an employee to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize the hazards. The training shall include the following areas as applicable: d. - The proper use of the scaffold, and the proper handling of materials on the scaffold. - The maximum intended load and the load -carrying capacities of the scaffolds used. - Any other pertinent requirements. • WE shall have each employee who is involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question. The training shall include the following topics, as applicable: - The nature of scaffold hazards. The correct procedures for erecting, disassembling, moving, operating, repairing, inspecting, and maintaining the type of scaffold being used. The design criteria, maximum intended load -carrying capacity, and intended use of the scaffold. Any other pertinent requirements. If WE have reason to believe that an employee lacks the skill or understanding needed to safely perform work that involves the erection, use, or dismantling of scaffolds, then the employer shall retrain the employee so that the requisite proficiency is regained. Retraining is required in all of the following situations: - Where changes at the worksite present a hazard about which an employee has not been previously trained. - Where changes in the types of scaffolds, fall protection, falling object protection, or other equipment present a hazard about which an employee has not been previously trained. - Where inadequacies in an affected employee's work involving scaffolds indicate that the employee has not retained the requisite proficiency for the work involved. • When free standing mobile scaffolding is used the height should not exceed four times the minimum base dimension • Wheels should turn freely and locks in good working condition Guardrails should be installed on scaffolding • Flanking should not exceed a 6 inch overhang over supports and should be properly secured. * Toe boards should be implemented a All components should be inclusive to ensure the correct use of the scaffold. Drug and Alcohol use policy We take pride in ensuring that our employees are safe and accident free. Construction can be a dangerous place when the use of drugs or alcohol takes place. This policy prohibits any employee behavior involving drugs or alcohol when at work. Discipline policies and procedures Safety and health on the job site and in the office is very important. Here at E.G.I. we operate with a goal or no damage to people, product and property. All of our employees, including all levels of management, are held accountable for obeying site safety and health rules. All jobs and tasks must be performed in a safe manner. To ensure heath, safety and quality, we practice equal discipline for all employees. This is important to ensure hazard prevention and control. The following five step disciplinary policy will be applied, depending on the issue, to everyone by the appropriate level of supervisor: a oral warning; a written reprimand; three days away from work; Formal meeting discussing possible dismissal. a Dismissal An example of why an employee would have to be disciplined would be any 'unsafe' behavior, such as not following regulation in a hazardous situation. It is also important that an adequate crew is allotted to meat time and production quotas in order to ensure a safe project. Proper training will also be implemented to reduce the need for discipline actions. All employees have certain responsibilities regarding health and safety which if not exercised adequately will result in disciplinary action. The findings must be documented on our investigation form. 2. Major injuries — fatality or multiple hospitalizations: Top management must see that the Department of Labor and Industries is notified as soon as possible, but at least within 8 hours of the incident. Call or contact in person the nearest office of the Department or call the OSHA toll free central number (1-800-321-6742). Top management will then assist the Department in the investigation. 3. The findings must be documented on our incident investigation report form and recorded on the OSHA 300 log, if applicable. (Sample incident investigation report form included in this document.) If emergency occurs on the job site the Construction Manager should be notified immediately. If necessary the on hand emergency facility will be available to all employees. We will train and designate a sufficient number of employees to assist in the safe and orderly process of handling a medical emergency. These employees will be trained and re-trained as needed. Environmental Glass, Inc Emergency Response Plan PURPOSE The goals of this emergency response plan are to protect the lives and health of our employees, and protect and minimize damage to company property in the event of an emergency. PRIORITIES • To contact local emergency service organizations • To assemble the company's Emergency Response Team (ERT) To plan for violence in the workplace and to prevent accidents caused by violence. e To train employees on medical emergencies • To react to accidents at the job site CA-41M VMI� We at E.G.I. will train all employees on the procedures contained in this plan. New employees will be trained upon hiring, and re-trained any time the employees responsibilities under the plan change or whenever the plan changes. Violence in workplace. Serious violence incidents should be reported properly. If an employee of Environmental Glass can't resolve his or her violent issues after oral warning they are to be sent home for the day and may not return to the place of work till they can control with their violence. If counseling is - b. An adequate rescue procedure. C. Someone to be readily available who has been trained in cardio- pulmonary resuscitation (CPR). 3. Equipment for confined space entry: a. A combination combustible gas/oxygen meter which samples for combustible vapors and oxygen deficiency simultaneously is suggested. However, individual meters to sample for combustible atmospheres and oxygen deficiency may also be used. b. Supplied -air breathing apparatus, such as a self-contained respirator with full face piece operated in pressure demand mode (SCBA), or Type-C supplied -air respirator with full face piece operated in pressure demand mode with an emergency backup SCBA or escape bottle operated in a pressure demand mode, must be in atmospheres immediately dangerous to life. C. A supplied -air Type-C respirator, in either continuous flow or pressure demand mode, may be used in areas which are not immediately hazardous to life and from which the wearer can readily escape. d. Harness and Lifelines: Harness should be capable of retrieving an inert body in an upright position. A parachute -type harness, with a single lifting ring attached to the upper back, or with duel lifting rings attached to the shoulder strap is recommended for work in open areas. Where egress through narrow openings is necessary, wristlets with attached lifting rings may be required instead of a body harness. Sufficient lifelines of at least one-half inch manila must be provided to insure constant connection between the worker In the confined space and the attendant outside. e. Ventilation: A portable blower with a minimum capacity of 600 cfm a 1/5 inches static pressure should be used to supply air and ventilate the enclosed space prior to and during the occupancy. If the space is large enough, additional air volume may be required. 4. Maintenance of Equipment - a specifically trained person must: a. Maintain and issue all required self-contained and/or supplied -air breathing apparatus. b. Maintain and insure calibration of all combustible gas/oxygen meters which must be readily available as required. ENVIRONMENTAL GLASS, INC. CONFINED. SPACE PRE ',' It is very important that asbestos/lead removal contractors establish safe procedures for entering and/or working in confined spaces which may be hazardous to employees. Confined space means any space having a limited entrance or exit which is subject to the accumulation of toxic or flammable containments or the development of an oxygen deficient atmosphere. Confined spaces include, but are not limited to: storage tanks, bins, boilers, ventilation or exhaust ducts, sewers, underground utility vaults, tunnels and crawl spaces. Recommended confined space entry procedures: a. Before any employee is allowed to enter a tank or other confined space, supervisory authorization must be obtained. b. Appropriate tests of the atmosphere must be made by authorized personnel from outside the confined space to determine if established air contaminant limits are exceeded or if the oxygen concentration is less than 19.5 percent by volume. Tests must be made with the appropriate monitoring equipment. The person authorized to monitor the atmosphere must be trained in the proper use, calibration, and care of the monitoring instruments and must remain at the site until all monitoring is completed. C. If the tests indicate that the atmosphere is unsafe, the confined space must be ventilated until the hazardous atmosphere is removed, prior to employee entry. d. (ii) Do not drop or throw blocks from an elevation or deliver blocks through chutes. • Do not stack lumber more than 20 feet high; if handling lumber manually, do not stack more than 16 feet high. (i) Remove all nails from used lumber before stacking. (ii) Stack lumber on level and solidly supported sills, and such that the stack is stable and self-supporting. Frequently inspect stock piles of sand, gravel, and crushed stone to prevent their becoming unsafe by continued adding to or withdrawing from the stock. (i) Do not remove frozen material in a manner that would produce an overhang. General Rigging Equipment Safety: Inspect rigging equipment for material handling prior to use on each shift and as necessary during its use to ensure that it is safe. Remove defective rigging equipment from service. • Never load rigging equipment in excess of its recommended safe working load. • Remove rigging equipment when not in use from the immediate work area so as not to present a hazard to employees. • Mark special rigging accessories (i.e., spreader bars, grabs, hooks, clamps, etc.) or other lifting accessories with the rated capacity. Proof test all components to 125% of the rated load prior to the first use. Maintain permanent records on the job site for all special rigging accessories. Disposal of waste materials: • Whenever materials are dropped more than 20 feet to any point lying outside the exterior walls of the building, use an enclosed chute of wood or equivalent material. When debris is dropped without the use of chutes, make sure that the area onto which the material is dropped is completely enclosed with barricades at least 42 inches high and 20 feet back from the projected edge of the opening above. Post at each level warning signs of the hazard of falling materials. Do not remove debris in this lower area until debris handling ceases above. • Remove all scrap lumber, waste material, and rubbish from the immediate work area as the work progresses. Make sure to comply with local fire regulations if disposing of waste material or debris by burning. OBJECTIVE The purpose of this Fire Prevention Plan is to eliminate the causes of fire, prevent loss of life and property by fire, and to comply with the Occupational Safety and Health Administration's (OSHA) standard on fire prevention, 29 CFR 1910.39. It provides employees with information and guidelines that will assist them in recognizing, reporting, and controlling fire hazards. II. BACKGROUND EGI is committed to minimizing the threat of fire to employees, visitors, and property. This Fire Prevention Plan serves to reduce the risk of fires at the job site in the following ways: A. identifies materials that are potential fire hazards and their proper handling and storage procedures; B. distinguishes potential ignition sources and the proper control procedures of those materials; C. describes fire protection equipment and/or systems used to control fire hazards; D. identifies persons responsible for maintaining the equipment and systems -installed to prevent or control ignition of fires; E. identifies persons responsible for the control and accumulation of flammable or combustible material; F. describes good housekeeping procedures necessary to insure the control of accumulated flammable and combustible waste material and residues to avoid a fire emergency; and G. provides training to employees with regard to fire hazards to which they may be exposed. M. ASSIGNMENT OF RESPONSIBILITY Fire safety is everyone's responsibility. All employees should know how to prevent and respond to fires, and are responsible for adhering to company policy regarding fire emergencies. A. Management Management determines the EGI fire prevention and protection policies. Management will provide adequate controls to provide a safe workplace, and will provide adequate resources and training to its employees to encourage fire prevention and the safest possible response in the event of a fire emergency. B. Plan Administrator Ryan Snabes shall manage the Fire Prevention Plan for EGI and shall maintain all records pertaining to the plan. Supervisors Supervisors are responsible for ensuring that employees receive appropriate fire safety training, and for notifying when changes in operation increase the risk of fire. Supervisors are also responsible for enforcing fire prevention and protection policies. C. Employees All employees shall: Complete all required training before working without supervision. Conduct operations safely to limit the risk of fire. Report potential fire hazards to their supervisors. Follow fire emergency procedures. IV. PLAN IMPLEMENTATION A. Good Housekeeping To limit the risk of fires, employees shall take the following precautions: 1. Minimize the storage of combustible materials. 2. Make sure that doors, hallways, stairs, and other exit routes are kept free of obstructions. 3. Dispose of combustible waste in covered, airtight, metal containers. 4. Use and store flammable materials in well -ventilated areas away from ignition sources. 5. Use only nonflammable cleaning products. 6. Keep incompatible (i.e., chemically reactive) substances away from each other. 7. Perform "hot work" (i.e., welding or working with an open flame or other ignition sources) in controlled and well -ventilated areas. 8. Keep equipment in good working order (i.e., inspect electrical wiring and appliances regularly and keep motors and machine tools free of dust and grease. 9. Ensure that heating units are safeguarded. 10. Report all gas leaks immediately Foreman shall ensure that all gas leaks are repaired immediately upon notification. 11. Repair and clears up flammable liquid leaks immediately. 12. Keep work areas free of dust, lint, sawdust, scraps, and similar material. 13. Do not rely on extension cords if wiring improvements are needed, and take care not to overload circuits with multiple pieces of equipment. 14. Ensure that required hot work permits are obtained. 15. Turn off electrical equipment when not in use. B. Maintenance Foreman will ensure that equipment is maintained according to manufacturers' specifications. EGI will also comply with requirements of the National Fire Protection Association (NFPA) codes for specific equipment. Only properly trained individuals shall perform maintenance work. The following equipment is subject to the maintenance, inspection, and testing procedures: 1. equipment installed to detect fuel leaks, control heating, and control pressurized systems; 2. portable fire extinguishers, automatic sprinkler systems, and fixed extinguishing systems; 3. detection systems for smoke, heat, or flame; 4. fire alarm systems; and 5. emergency backup systems and the equipment they support. V. TYPES OF HAZARDS The following sections address the major workplace fire hazards at EGI facilities and the procedures for controlling the hazards. A. Electrical Fire Hazards Electrical system failures and the misuse of electrical equipment are leading causes of workplace fires. Fires can result from loose ground connections, wiring with frayed insulation, or overloaded fuses, circuits, motors, or outlets. To prevent electrical fires, employees shall: 1. Make sure that worn wires are replac6d. 2. Use only appropriately rated fuses. 3. Never use extension cords as substitutes for wiring improvements. 4. Use only approved extension cords [i.e., those with the Underwriters Laboratory (UL) or Factory Mutual (FM) label]. 5. Check wiring in hazardous locations where the risk of fire is especially high. 6. Check electrical equipment to ensure that it is either properly grounded or double insulated. 7. Ensure adequate spacing while performing maintenance. B. Portable Heaters All portable heaters shall be approved. Portable electric heaters shall have tip -over protection that automatically shuts off the unit when it is tipped over. There shall be adequate clearance between the heater and combustible furnishings or other materials at all times. C. Office Fire Hazards Fire risks are not limited to EGI's industrial facilities. Fires in offices have become more likely because of the increased use of electrical equipment, such as computers and fax machines. To prevent office fires, employees shall: 1. Avoid overloading circuits with office equipment. 2. Turn off nonessential electrical equipment at the end of each workday. 3. Keep storage areas clear of rubbish. 4. Ensure that extension cords are not placed under carpets. 5. Ensure that trash and paper set aside for recycling is not allowed to accumulate. D. Cutting, Welding, and Open Flame Work Responsible Person(s) will ensure the following: 1. All necessary hot work permits have been obtained prior to work beginning. 2. Cutting and welding are done by authorized personnel in designated cutting and welding areas whenever possible. 3. Adequate ventilation is provided. 4. Torches, regulators, pressure -reducing valves, and manifolds are UL listed or FM approved. 5. Oxygen -fuel gas systems are equipped with listed and/or approved backflow valves and pressure -relief devices. 6. Cutters, welders, and helpers are wearing eye protection and protective clothing as appropriate. 7. Cutting or welding is prohibited in sprinklered areas while sprinkler protection is out of service. 8. Cutting or welding is prohibited in areas where explosive atmospheres of gases, vapors, or dusts could develop from residues or accumulations in confined spaces. 9. Cutting or welding is prohibited on metal walls, ceilings, or roofs built of combustible sandwich -type panel construction or having combustible covering. 10. Confined spaces such as tanks are tested to ensure that the atmosphere is not over ten percent of the lower flammable limit before cutting or welding in or on the tank. 11. Small tanks, piping, or containers that cannot be entered are cleaned, purged, and tested before cutting or welding on them begins. 12. Fire watch has been established E. Flammable and Combustible Materials Responsible Person shall regularly evaluate the presence of combustible materials at EGI Certain types of substances can ignite at relatively low temperatures or pose a risk of catastrophic explosion if ignited. Such substances obviously require special care and handling. 1. Class A combustibles. These include common combustible materials (wood, paper, cloth, rubber, and plastics) that can act as fuel and are found in non -specialized areas such as offices. To handle Class A combustibles safely: a. Dispose of waste daily. b. Keep trash in metal -lined receptacles with tight -fitting covers (metal wastebaskets that are emptied every day do not need to be covered). c. Keep work areas clean and free of fuel paths that could allow a fire to spread. d. Keep combustibles away from accidental ignition sources, such as hot plates, soldering irons, or other heat- or spark -producing devices. e. Store paper stock in metal cabinets. f. Store rags in metal bins with self -closing lids. g. Do not order excessive amounts of combustibles. h. Make frequent inspections to anticipate fires before they start. Water, multi -purpose dry chemical (ABC), and halon 1211 are approved fire extinguishing agents for Class A combustibles. 2. Class B combustibles. These include flammable and combustible liquids (oils, greases, tars, oil -based paints, and lacquers), flammable gases, and flammable aerosols. To handle Class B combustibles safely: a. Use only approved pumps, taking suction from the top, to dispense liquids from tanks, drums, barrels, or similar containers (or use approved self -closing valves or faucets). b. Do not dispense Class B flammable liquids into containers unless the nozzle and container are electrically interconnected by contact or by a bonding wire. Either the tank or container must be grounded. c. Store, handle, and use Class B combustibles only in approved locations where vapors are prevented from reaching ignition sources such as heating or electric equipment, open flames, or mechanical or electric sparks. d. Do not use a flammable liquid as a cleaning agent inside a building (the only exception is in a closed machine approved for cleaning with flammable liquids). e. Do not use, handle, or store Class B combustibles near exits, stairs, or any other areas normally used as exits. f. Do not weld, cut, grind, or use unsafe electrical appliances or equipment near Class B combustibles. g. Do not generate heat, allow an open flame, or smoke near Class B combustibles. h. Know the location of and how to use the nearest portable fire extinguisher rated for Class B fire. Water should not be used to extinguish Class B fires caused by flammable liquids. Water can cause the burning liquid to spread, making the fire worse. To extinguish a fire caused by flammable liquids, exclude the air around the burning liquid. The following fire -extinguishing agents are approved for Class B combustibles: carbon dioxide, multi -purpose dry chemical (ABC), halon 1301, and halon 1211. (NOTE: Halon has been determined to be an ozone -depleting substance and is no longer being manufactured. Existing systems using halon can be kept in place.) F. Smoking Smoking is prohibited in all EGI buildings. Certain outdoor areas may also be designated as no smoking areas. The areas in which smoking is prohibited outdoors are identified by NO SMOKING signs. VI.
