The Board of Public Works, at its meeting held on December 20, 2018, approved the above
referenced permit to access segments of the East Bank Trail for a soil boring to survey
proposed electric transmission line route.

I/We hereby give AEP Indiana Michigan Transmission Company, a(n) Indiana corporation, a
unit of American Electric Power ("AEP"), its affiliates, agents, employees, contractors and the
appropriate federal, state and local agencies, archeologists, biologists, and/or environmental
scientists, permission to enter upon my/our premises to conduct such surveys, soundings,
drillings, appraisals, and examinations and tests necessary (including the right to drill holes for
testing soil and bedrock) for the proposed electric transmission line route.
AEP has the right to trim or cut vegetation necessary for survey purposes and agrees to pay the
prevailing market price for standing timber for any marketable trees cut down in the course of
such survey. AEP will be responsible for any damage done to such property and further agrees to
indemnify against all liability to third persons caused by negligent acts of AEP's employees
while on the property.

Special Conditions: A 48-hour notice will be provided to the City once the borings have been
staked for the City to review the boring locations.
No trees will be removed for soil borings.
