Memorial Hospital Area Facade Improvements

On May 23, 2011, the Commission approved a professional services agreement with The
Troyer Group, for design studies of facade improvements to the Skyway Bridge and along
the eastern side of Michigan Street in the Memorial Hospital campus area. On December
13, 2011, the Commission approved Troyer to move forward with the next phase of work,
which was to finalize those designs, prepare construction bid specs, project bidding
evaluation assistance, and construction oversight and close out.
Bids have now been opened for the facade improvement project, allowing the scope of work
to be expanded. An identified project that compliments the work being done is to make all
the windows and frames in the Skyway Building the same.
The attached proposal from Troyer is to provide addendum documentation, bidding, and
construction administration. Troyer estimates the fees for this additional work to be $5,000
to $7,000.
Staff requests approval of this Professional Services Agreement, in a not to exceed amount
of $7,000.

Re: Facade Studies -Phase I1- Window Replacement Addendum - Added Services
Dear Mr. Relos:
We are pleased to offer to you our Architectural Services to complete the addendum
documentation, bidding, and construction administration phases for the Addendum
to new facades of the Skyway Office Building. Once again we would like to thank
you for your continued confidence in our design team.
We understand that this Addendum will cover the added work of replacing of all
windows in the west and south elevations and the added painting and repair of the
EIFS on the north and east elevations of the Skyway Office Building.
We understand that budget for this work has been estimated to be between $130,000
and $150,000 not including design fees, permitting, etc.
Services
The Troyer Group will provide services to support the window replacement and
EIFS work as described above. TTG will utilize Addendum documentation
consisting of 8 1/2" x 11" sheets including both drawn and written specifications as
necessary for pricing, review, and construction.
For this additional work we hove not included any coordination that might be
required with the Indiana State Department of Transportation (INDOT). We do not
expect any additional INDOT meetings for this work. If there are INDOT or other
government's approvals, the design team will perform the work required on a time
and expense basis; as directed.

The Troyer Group will also provide construction administration services for the
work covered in the addendum. Additional time has been valued for to provide
these services in conjunction with those provided on the main contract. Dedicated
separate visits and meetings have not been valued for and will need to be negotiated
on a case by case basis.
What it will cost:
We will provide the services listed above on a time and expense basis. We suggest
you budget between Five Thousand Dollars ($5,000.00) and Seven Thousand
Dollars ($7,000.00), this includes reimbursable expenses.
The Troyer Group bills monthly. Consequently, at the beginning of each month you
will receive a statement covering the time and expenses expended during the
previous month. Payment is due within thirty (30) days after statement date,

questions. If the terms are acceptable, please print and sign two copies, keeping one
for your records and returning one to us.
