The Board of Public Works, at its meeting held on November 13, 2018, approved Change
Order No. 1 to eliminate the Intermediate Phase I completion date of November 30, 2018 with
no change in final completion date or contract amount.
Additionally, Change Order No. 2 was approved for an increase of $7,899, bringing the
current contract amount to $914,899.

GacoFlex Coating
50-Year Limited Material Warranty
Gaco Western, LLC warrants GacoFlex Coating to be free
of defects in material for a period of 50 years and to meet
published physical properties at the time of manufacture.
IMPORTANT LIMITATIONS:
There are no expressed warranties, other than the warranties
described above, and no implied warranties, such as warranties
of fitness or merchantability.
The buyer's sole remedy in the event of a breach of warranty is
for Gaco Western, LLC to replace any non -conforming products
with conforming Gaco Western, LLC products. Gaco Western, LLC
will not be liable for direct, incidental or consequential damages
of any kind under any circumstances.
OTHER LIMITATIONS AND CONDITIONS:
The warranty is void if the GacoFlex Coating is painted, patched or
covered over in any way with any product other than GacoFlex Coatings.
1. The warranty does not cover any labor costs for inspection, testing or
repair of the GacoFlex Coating or any other labor costs.
2. Any claim under this warranty shall be sent in writing to Gaco Western,
LLC Warranty Claims Manager at the address below and shall include a
description of the product defect and any information indicating
warranty liability and also shall include proof of purchase with original
receipt.
3. This warranty gives you specific legal rights, and you may also have
other rights which vary from state to state. Some states do not allow
the exclusion or limitation of incidental or consequential damages, so
the above limitation or exclusion may not apply to you.

Floyd said the coating looks good and has lasted 15 years. The material is a lot
more for coating but the labor is a lot less than for the EPDM. We could coat it
again and deduct $720.00 off of our contract.

Information Requested:
There is a downspout on the northwest comer of the boiler room that is to be repaired
replaced. (Note #I I A 1.4) The existing downspout was not attached and was piped into
an existing drywall that is no longer there. (See attached picture)

1. Sanitary sewer lines: Shown on pg. P1.1 entering building at east wall but not
connecting to the lift station on the east side of the building. Contractor is not sure
where existing SS lines.
2. Kitchen SS line currently wraps past fireplace through men's restroom and down
under porch. Contractor believes they need to be removed in -order to tie into
proposed SS line according to 4" main on pg. P1.1. They suggest the removal of
overhead piping and dropping new line against wall in the corner next to the fireplace
and connecting the line under the slab. (Ref. - RFI 005)

DESCRIPTION
Apollo°Air Gap Drains (AGD4Ax) are designed tofu nne I minor discharges from a Reduced Pressure Backflow Preventer's relief valve due to line pressure fluctuations
and/or minor check valve fouling into the drainage system. Drainage piping is easily attached to the threaded portion of the AGD. The female I size is listed in the
table under "Outlet Connection". Drainage piping may be attached with a coupling and the appropriate cement or with a flexible pipe coupling. Piping compatible
with the outlet is listed in the table under "Outlet Pipe Size".
Air Gap Drains are NOT designed to collect the full discharge capacity of the relief valve. Please refer to page 3 for relief valve full discharge rates.

IMPORTANT LIMITATIONS:
There are no expressed warranties, other than the warranties
described above, and no implied warranties, such as warranties
of fitness or merchantability.
The buyer's sole remedy in the event of a breech of warranty is
for Gaco Western, LLC to replace any non -conforming products
with conforming Gaco Western, LLC products, Gaco Western, LLC
will not be liable for direct, incidental or consequential damages
of any kind under any circumstances.
OTHER LIMITATIONS AND CONDITIONS:
The warranty is void if the GacoFlex Coating is painted, patched or
covered over in any way with any product other than GacoFlex Coatings.
1. The warranty does not cover any labor costs for inspection, testing or
repair of the GacoFlex Coating or any other labor costs.

replaced. (Note #11 A1.4) The existing downspout was not attached and was piped into
an existing drywall that is no longer there. (See attached picture)

connecting to the lift station on the east side of the building. Contractor is not sure
where existing SS lines.
2. Kitchen SS line currently wraps past fireplace through men's restroom and down
under porch. Contractor believes they need to be removed in -order to tie into
proposed SS line according to 4" main on pg. P1.1. They suggest the removal of
overhead piping and dropping new line against wall in the corner next to the fireplace
and connecting the line under the slab. (Ref. - RFI 005)

The Apollo" MODEL RPLF 4A Reduced Pressure Principle Backflow Preventers consist of two independently acting, TriForce'" center stem guided check
valves with a differential pressure relief valve located between the check valves. The unit is designed to give maximum protection against backflow of
health or non -health hazard fluids by either back -pressure or back-siphonage. The durable domestic stainless steel units (2-1/2"-8") and the FDA epoxy
coated ductile iron units (10" and 12") are easily maintained in the line without any special tools. The TriForce'" check valves operate with a spring assist in

DESCRIPTION
Apollo'Air Gap Drains (AGD4Ax) are designed to funnel minor discharges from a Reduced Pressure Backflow Preventer's relief valve due to line pressure fluctuations
and/or minor check valve fouling into the drainage system. Drainage piping is easily attached to the threaded portion of the AGD, The female NPT size is listed in the
table under "Outlet Connection". Drainage piping may be attached with a coupling and the appropriate cement or with a flexible pipe coupling, Piping compatible
with the outlet is listed in the table under "Outlet Pipe Size".
Air Gap Drains are NOT designed to collect the full discharge capacity of the relief valve. Please refer to page 3 for relief valve full discharge rates.
