PUBLIC AGENDA SESSION JANUARY 9, 2008
The Board of Public Safety met in the Public Agenda Session at 8:05 a.m. on Wednesday,
January 9, 2008 with Board President Donald L. Alford and Board Members Juanita Dempsey,
Patrick D. Cottrell, Bruce J. BonDurant, and Jack Reed present. Also present were Board
Attorney Thomas Bodnar, Police Chief Darryl Boykin, and Assistant Fire Chief Jim Lopez.
The meeting was held in the Chiefs Conference Room, Central Fire Station, 1222 South
Michigan Street, South Bend, Indiana.
Board members discussed the following items:
- Addition to the Agenda - Discipline - Fire Department
Mr. Bodnar noted the addition of an appeal by Mr. Cyril Quinn for the Board to review
the disciplinary action against him by Fire Chief Buchanon. He notified the Board by e-
mail that he disagreed with the charges and the Chiefs recommended discipline against
him.
- Monthly Statistical Analysis - Police
Chief Boykins noted the arrest of one individual that has confessed to eighteen (18) bank
robberies, and his girlfriend who was responsible for a series of robberies. He stated the
arrests of these two individuals and the arrest of the individuals responsible for a series of
home invasions has solved a large number of robberies, and reduced those statistics
significantly.
- Swearing in of Officers - Police
Chief Boykins noted that the way the current hiring standards are written cause the
Department to lose qualified candidates, and they need to be revised. He informed the
Board that the Police Department may hire their first black woman in thirteen years. She
is currently going through the hiring process and he feels certain that she will make it
through the process.
- Resolution No. 01 -2008 - Commendation for Members of C -Shift, Engine 8
Assistant Fire Chief Jim Lopez praised the life- saving efforts of the above shift at the
scene of a choking. He noted the firefighters were out of service but in close proximity to
the scene when the call came in. It was their swift response that saved the gentleman's life
before the ambulance could get there, and they were being presented with a
commendation at today's meeting for their heroic efforts.
ADJOURNMENT
There being no further business to come before the Board, Mr. Alford declared the meeting
adjourned at 8:39 a.m.

POLICE SELECTION PANEL JANUARY 9, 2008
The Police Selection Panel met at 8:40 a.m. on Wednesday, January 9, 2008 in the Chiefs
Conference Room, Central Fire Station, 1222 South Michigan Street, South Bend, Indiana, for
the purpose of formulating a recommendation to be made to the Board of Public Safety. Present
were Police Chief Darryl Boykins, Board Attorney Thomas Bodnar, Assistant Fire Chief Jim
Lopez, Board President Donald Alford, Board Members Juanita Dempsey, Bruce J. BonDurant,
Patrick Cottrell, Jack Reed, and Board Clerk Linda Martin. Upon a motion by Mr. Bodnar,
seconded by Ms. Dempsey and carried, the recommendation from Police Chief Boykins to
appoint Mr. Joshua Mann, Mr. Chadwick Goben, Mr. Jamil Elwaer, and Mr. Colin Bums to the
South Bend Police Department as probationary employees for a period of one (1) year was
approved. The Police Selection Panel adjourned at 8:42 a.m.

EXECUTIVE SESSION JANUARY 9, 2008
The Board of Public Safety met in Executive Session on Wednesday, January 9, 2008, at 8:43
a.m. The meeting was held in the Chiefs Conference Room, Central Fire Station, 1222 South
Michigan Street, South Bend, Indiana, for the purposes specified in I.C. 5- 14- 1.5 -6.1 (b) (2) (B)
and (b) (6) (A). Board President Donald Alford, and Board Members Juanita Dempsey, Bruce J.
BonDurant, Patrick Cottrell and Jack Reed were present. Also present were Board Attorney
Thomas Bodnar, Police Chief Darryl Boykins, and Assistant Fire Chief Jim Lopez. Board
members did not discuss any subject matter other than the subject matter as specified in the
Public Notice. The meeting adjourned at 9:25 a.m.

REGULAR MEETING JANUARY 9, 2008
The Board of Public Safety met in the regular meeting at 9:29 a.m. on Wednesday, January 9,
2008, with Board President Donald Alford, and Board Members Juanita Dempsey, Bruce J.
BonDurant, Patrick D. Cottrell, and Jack Reed present. Also present were Police Chief Darryl
Boykins, Assistant Fire Chief Jim Lopez and Board Attorney Thomas Bodnar.
The meeting was held in Classroom One, Central Fire Station, 1222 South Michigan Street,
South Bend, Indiana.
APPROVE MINUTES OF PREVIOUS MEETING
Upon a motion by Mr. BonDurant, seconded by Ms. Dempsey and carried, the minutes of the
Public Agenda Session, Executive Session, and Regular meetings of the Board held on
December 11, 2007, were approved.
POLICE DEPARTMENT
POLICE SELECTION PANEL
Upon a motion by Mr. BonDurant, seconded by Mr. Cottrell and carried, the Police Selection
Panel approved the recommendation of Police Chief Boykins that Mr. Joshua Mann, Mr.
Chadwick Goben, Mr. Jamil Elwaer, and Mr. Colin Burns be accepted for employment as
probationary police officers with the South Bend Police Department for a probationary period of
one (1) year.

Mayor Steve Luecke and Mr. John Voorde, City Clerk, were present for the swearing in of the
above officers. Mr. Voorde administered the Oath of Office. Mayor Luecke and Chief Boykin
joined the Board in congratulating the newly sworn in officers and wishing them well in

their endeavors with the South Bend Police Department.
President Alford stated that before Mr. Voorde left the meeting, the Board wished to extend their
condolences to him for the recent loss of his mother.
ACCEPT AND APPROVE PROMOTION - CORPORAL KRIS HINTON
In a letter to the Board, Chief Boykins recommended that Corporal Kris Hinton be promoted
permanently to the rank of Sergeant effective January 10, 2008. Chief Boykins stated in his letter
that Corporal Hinton was placed in a special assignment as a Sergeant in the Uniform Division
on January 10, 2007, and his progress was reviewed by Captain Jeff Walters who recommended
Corporal Hinton be permanently promoted. Upon a motion made by Mr. BonDurant, seconded
by Mr. Cottrell and carried, the request for the promotion was approved as outlined above.
ACCEPT AND APPROVE RETIREMENT — CAPTAIN WANDA SHOCK
President Alford advised that former Police Chief Thomas Fautz has submitted to the Board the
Application and Declaration for Retirement for Captain Wanda Shock effective January 14,
2008. In his letter, Chief Fautz noted that Captain Shock is retiring in good standing after
serving for the last twenty -six (26) years and one (1) day, and joined the Board in thanking her
for her commitment, dedication and years of distinguished service to the South Bend Police
Department and the citizens of South Bend. Board Members also wished her a long and happy
retirement. Upon a motion made by Mr. BonDurant, seconded by Mr. Cottrell and carried, the
retirement was accepted and approved as outlined above.
ACCEPT AND APPROVE RETIREMENT — CORPORAL PATRICK LAHEY
President Alford advised that former Police Chief Thomas Fautz has submitted to the Board the
Application and Declaration for Retirement for Corporal Patrick Lahey effective January 7,
2008. In his letter, Chief Fautz noted that Corporal Lahey is retiring in good standing after
serving for the last thirty -two (32) years and twenty -three (23) days, and joined the Board in
thanking him for his commitment, dedication and years of distinguished service to the South
Bend Police Department and the citizens of South Bend. Board Members also wished him a long
and happy retirement. Upon a motion made by Mr. BonDurant, seconded by Mr. Cottrell and
carried, the retirement was accepted and approved as outlined above.
FILING OF MONTHLY STATISTICAL ANALYSIS REPORT — NOVEMBER 2007
President Alford indicated that unless there were questions or objections by members of the
Board, the statistical analysis report for the month of November 2007, as submitted by the Police
Department, would be accepted for filing. There being no objections, the following report was
accepted for filing:

President Alford advised that Chief Howard Buchanon has submitted to the Board the
Application and Declaration for Retirement for Assistant Chief Robert Sanders effective
December 21, 2007. Chief Buchanon noted that Assistant Chief Sanders has served the
Department with loyalty and distinction during the last thirty (30) years, and two (2) months. In
his letter, Chief Buchanon joined the Board in thanking him for his commitment, dedication and
years of distinguished service to the South Bend Fire Department and the citizens of South Bend.

ACCEPT AND APPROVE RETIREMENT — ROBERT SANDERS
President Alford advised that Chief Howard Buchanon has submitted to the Board the
Application and Declaration for Retirement for Assistant Chief Robert Sanders effective
December 21, 2007. Chief Buchanon noted that Assistant Chief Sanders has served the
Department with loyalty and distinction during the last thirty (30) years, and two (2) months. In
his letter, Chief Buchanon joined the Board in thanking him for his commitment, dedication and
years of distinguished service to the South Bend Fire Department and the citizens of South Bend.

during the last thirty-four (34) years, and eleven (11) months. In his letter, Chief Buchanon
joined the Board in thanking him for his commitment, dedication and years of distinguished
service to the South Bend Fire Department and the citizens of South Bend. Members also

President Alford advised that Chief Howard Buchanon has submitted to the Board the
Application and Declaration for Retirement for Engineer Tom Cira effective December 2, 2007.
Chief Buchanon noted that Engineer Cira has served the Department with loyalty and distinction
during the last thirty -four (34) years, and one (1) month. In his letter, Chief Buchanon joined the
Board in thanking him for his commitment, dedication and years of distinguished service to the
South Bend Fire Department and the citizens of South Bend. Members also wished him a long

December 7, 2007. Chief Buchanon noted that Engineer Walczewski has served the Department
with loyalty and distinction during the last thirty (30) years. In his letter, Chief Buchanon joined
the Board in thanking him for his commitment, dedication and years of distinguished service to
the South Bend Fire Department and the citizens of South Bend. Members also wished him a

with loyalty and distinction during the last twenty -seven (27) years, and one (1) month. In his

years of distinguished service to the South Bend Fire Department and the citizens of South Bend.

distinction during the last thirty -one (3 1) years, and eleven (11) months. hi his letter, Chief

distinguished service to the South Bend Fire Department and the citizens of South Bend.

Application and Declaration for Retirement for Engineer John E. Zanka, Jr. effective December
3, 2007. Chief Buchanon noted that Engineer Zanka has served the Department with loyalty and
distinction during the last thirty-four (34) years and one (1) month. In his letter, Chief Buchanon
joined the Board in thanking him for his commitment, dedication and years of distinguished
service to the South Bend Fire Department and the citizens of South Bend. Members also

Application and Declaration for Retirement for Pump Engineer Tim Szalai effective December
3, 2007. Chief Buchanon noted that Engineer Szalai has served the Department with loyalty and
distinction during the last thirty-two (32) years, and two (2) months. In his letter, Chief
Buchanon joined the Board in thanking him for his commitment, dedication and years of
distinguished service to the South Bend Fire Department and the citizens of South Bend.

President Alford advised that Chief Howard Buchanon has submitted to the Board the
Application and Declaration for Retirement for Captain Tommie Flowers effective December 1,
2007. Chief Buchanon noted that Captain Flowers has served the Department with loyalty and
distinction during the last thirty -four (34) years, and one (1) month. In his letter, Chief
Buchanon joined the Board in thanking him for his commitment, dedication and years of
distinguished service to the South Bend Fire Department and the citizens of South Bend.
Members also wished him a long and happy retirement. Upon a motion made by Mr. Reed,
seconded by Ms. Dempsey and carried, the retirement was accepted and approved.
ACCEPT AND APPROVE PROMOTIONS TO THE RANK OF CAPTAIN — MARK DAHMS
ANDY MYER, ANDREW PETRIE, SCOTT TABERSKI, SCOTT WEBBER ,
In a letter to the Board, Fire Chief Howard Buchanon stated that the South Bend Fire Department
has five (5) vacancies for Captain in the Fire Suppression Division. To fill these positions, Chief
Buchanon recommends that Pump Engineers Mark Dahms, Andrew Petrie, Scott Taberski, Scott
Webber, and Lieutenant Paramedic Andy Myer be promoted to the rank of Captain effective
January 9, 2008.
In his letter, Chief Buchanon stated that Pump Engineer Dahms was appointed to the South Bend
Fire Department on November 23, 1994. He is an Emergency Medical Technician and has
proven his leadership skills and ability in fire suppression. Pump Engineer Petrie was appointed
to the Fire Department on March 25, 1998. He is an emergency Medical Technician and has

proven his leadership skills and ability in fire suppression. Pump Engineer Taberski was
appointed to the Fire Department on October 24, 1997 and is an Emergency Medical Technician,
Swiftwater Specialist and is on the Tactical Rescue Team. He has proven his leadership skills
and ability in fire suppression. Pump Engineer Webber was appointed to the Fire Department on
May 10, 1989. Scot is an Emergency Medical Technician and is on the Tactical Rescue Team,
and he has proven his leadership skills and ability in fire suppression. Lieutenant Paramedic
Myer was appointed to the Fire Department on October 4, 1995. He is a Paramedic for EMS and
has proven his leadership skills and ability in fire suppression.
Therefore, upon a motion made by Mr. Reed, seconded by Ms. Dempsey and carried, the
promotions were accepted and approved. Assistant Fire Chiefs Jim Lopez and Mark Nowicki,
and Mayor Luecke pinned the new badges on the officers and congratulated them. The Board
wished them well in their endeavors.
ACKNOWLEDGE PROMOTION — FEDERICO RODRIGUEZ
In a letter to the Board, Chief Buchanon stated the Fire Department has one vacancy for the
position of Assistant Chief of Fire Prevention/Inspection. Chief Buchanon recommends the
promotion of Battalion Chief Federico Rodriguez to Assistant Chief of Fire
Prevention/Inspection. In his letter Chief Buchanon stated that Chief Rodriguez was appointed
to the Fire Department on September 16, 1972. He was promoted to the position of Lieutenant on
July 15, 1975 and became a Battalion Chief on February 25, 1998 and has maintained the
position since that time. President Alford noted that Mayor Steve Luecke has approved the
promotion and the Board would acknowledge the promotion.
ACKNOWLEDGE PROMOTION — MARK CATANZARITE
In a letter to the Board, Chief Buchanon stated the Fire Department has one vacancy for the
position of 24 -hour Battalion Chief. Chief Buchanon recommends the promotion of Captain
Mark Catanzarite to the position. In his letter Chief Buchanon stated that Captain Catanzarite
was appointed to the Fire Department on May 10, 1989. He was promoted to the position of
Captain on November 27, 1996 and has maintained the position since that time. President Alford
noted that Mayor Steve Luecke has approved the promotion, and the Board would acknowledge
the promotion.
ACCEPT RESIGNATION - JEREMY DOWNS
President Alford advised that the Board is in receipt of a letter of resignation from Engineer
Jeremy Downs in which he tenders his resignation from the South Bend Fire Department,
effective January 2, 2008. Chief Buchanon stated, in his letter, that Engineer Downs has been
employed with the South Bend Fire Department for the past five (5) years and six (6) months.
The resignation was accepted by the Board.

Upon a motion made by Mr. Reed, seconded by Ms. Dempsey and carried, the following
Resolution was adopted by the Board of Public Safety:

Commendation for Members of C -Shift
Engine 8
WHEREAS, the Board of Public Safety wishes to issue commendations to individuals
that perform meritorious service on behalf of the citizens of this community, and;
WHEREAS, such service shall be recognized by the Board of Public Safety and
appropriate commendation shall be presented to the individuals performing said meritorious
service;
NOW, THEREFORE, BE IT RESOLVED, that commendation be given to:

On December 8, 2007, at approximately 6:45 PM, Medic 1 and Engine 10 were
dispatched to a call in reference to a man choking. When the call was dispatched, members of the
Engine 8 crew, which were out of service for a Christmas event on Erskine Boulevard,
recognized that they were in close proximity to the call. Engineer Pajakowski radioed the
Dispatch Center that he would check out the patient and call back with information. When
Engineer Pajakowski entered the home, he immediately identified that the gentleman was in
distress and in grave danger because he was unable to breathe. After unsuccessful attempts to
employ the Heimlich maneuver, he asked the other Engine 8 crew members to retrieve the
Advanced Life Support equipment and for their assistance. At this point, the patient was
unconscious and not breathing. Using Magill forceps, Engineer Pajakowski, with assistance from
Captain Schrader and Engineer Banacki, proceeded to remove a large piece of pizza that was
lodged in the patient's throat. The pizza had completely blocked the patient's airway, making it
impossible to breathe. Without performing this procedure, the patient would not have survived
the encounter. Even after removing the airway blockage, the Engine 8 crew had to continue to
work on the patient providing oxygen, placing an IV, and checking the patient's blood sugar.
This was all done along with a total patient assessment prior to the arrival of the ambulance
which was ten minutes after the Dispatch Center received the call.
The vigilance, quick action, and heroic efforts of these individuals are directly
responsible for this man's survival.
Be it further resolved that Nicholas Schrader, John Banacki, and Peter Pajakowski be
commended and recognized for their quick actions, dedication, and professionalism. Their
actions reflect highly upon themselves, the South Bend Fire Department, and the City of South
Bend.

NOTIFICATION OF DECISION - DISCIPLINARY ACTION - CYRIL QUINN
President Alford stated the Board reviewed Chief Buchanon's recommendation for disciplinary
action against Engineer Cyril Quinn, at Mr. Quinn's appeal. Upon a motion by Mr. BonDurant,
seconded by Mr. Cottrell and carried, the Board denied Mr. Quinn's appeal for a hearing and
moved to accept Chief Buchanon's recommended disciplinary action as follows:
1. Fined $397.68, which is three eight hour days pay (24 hours).
PRIVILEGE OF THE FLOOR
Mr. Jim Sipocz stated his son, who is in a wheelchair, has visited various fire stations around the
City and he wished to thank Mark Catanzarite and Scott Taberski`for their kindness and help in
his endeavors. He also noted the assistance of his son's nurse, Ms. Annie Short, who is a
paramedic, as an initiator for his son James being able to see fire stations around the city.
President Alford stated that these kinds of actions make it worthwhile to be a part of the Board of
Safety, and he thanked Mr. Sipocz for recognizing the firefighters' kindness and assistance.
ADJOURNMENT
There being no further business to come before the Board, President Alford declared the meeting
adjourned at 9:59 a.m.
