Temporary Occupancy Permit for a new curtainwall system, scaffolding, and boom lifts for an
additional ninety (90) days through November 30, 2018 on 635, S. Lafayette Blvd.

Linda,
Due to weather delays, Architectural and Structural delays, and additional work requested
by the Owner, we would like to request an extension of the sidewalk closure on Lafayette
St. for 90 additional days.
We would like it to extend until 11/30/2018.
1 have attached the existing permit, traffic control/barricading map along with additional
pictures for your review and comment.
