James Oliver Plaque design, construction, and installation

As part of the final elements to complete the James Oliver Memorial Plaza project, there is a need
to install a plaque to identify and recognize the sculpture and sculptor of the James Oliver sculpture.
As part of the City's contractual obligations with purchasing the sculpture, a plaque was to be
included with the project but that item has yet to be completed.
This professional service proposal will allow Danch, Harner & Associates, the project designer, to
coordinate the design, fabrication and installation of the dedication plaque at the site. South Bend
Monuments has helped with initial pricing of the plaque and the total projected cost of the entire
process is $1,200.00. Staff would request a not -to- exceed budget of $1,500.00 to cover any
unforeseen expenses that might arise during the design, fabrication and installation process.

Re: At the request of the City of South Bend, services associated with the James Oliver Memorial project;
provision of a bronze dedication plaque.
Dear Mr. Schalliol,
At the request of the City, Danch Harner, & Associates will coordinate the acquisition, creation, and
installation of a bronze dedication plaque to be placed at the recently completed James Oliver Memorial.
The plaque will be purchased, fabricated, and installed by South Bend Monument, Inc. The cost to
provide the above plaque and services will be $1,200.
Based on the above, please let this document serve as a letter of agreement between Danch, Harner &
Associates and The City of South Bend, Community & Economic Development.
