RE: Proposal to Conduct Off -Site Monitoring Well Installation and /or Groundwater Sampling
in the Vicinity of the Oliver Industrial Park, South Bend, Indiana (the Site);
S131066.400.0001.
Dear Ms. Kolata:
Hull & Associates, Inc. (Hull) is pleased to present the City of South Bend (Client) the following
proposal to provide professional engineering services for the above referenced project. The
purpose of this document is to establish the Scope of Work as we understand it and to provide a
fee schedule for the project.
The Site, known as the Oliver Industrial Park, and formerly known as the Oliver Plow Works
facility, is enrolled in the Indiana Department of Environmental Management (IDEM) Voluntary
Remediation Program (VRP). Additional off -site delineation of chemicals of concern (COCs) in
groundwater is necessary prior to completion of a Remediation Work Plan, which is a necessary
milestone of the VRP toward closure of the Site.
Task 1 Monitoring Well Installation and Groundwater Sampling Event
Hull will install monitoring wells and /or temporary groundwater sampling points at or in the
vicinity of the sampling locations shown on Figure 1, as discussed below. Please note the
location of historical sampling point CTW -3S /D on Figure 1, which is the basis for the sampling
locations (both temporary and permanent) proposed herein. Sampling point CTW -3S /D is the
location of the downgradient -most sampling point installed during a January 2003 sampling
event; at this sampling location, IDEM VRP Tier II Residential and Nonresidential Cleanup
Goals were exceeded, but have not been defined further downgradient.
In general, our proposed approach entails defining the vertical extent of chlorinated volatile
organic compounds (VOCs) downgradient of CTW -3S /D by installing temporary sampling points
north and east of CTW -3S /D; submitting those samples to a laboratory and requesting 3 -day
turnaround; installing permanent monitoring wells south and west of CTW -3S /D while laboratory
analyses on the samples from the temporary points are completed; and installing permanent
monitoring wells downgradient of CTW -3S /D at appropriate locations once the vertical extent is
defined. As further explained below, this scope of work includes a contingency for an additional
sampling event to install temporary sampling points to define the vertical extent of COCs in
groundwater downgradient of sampling point CTW -3S /D, should it be necessary.

This task will begin with the advancement of temporary screens using direct -push technologies
(e.g., a ST -15 Hydropunch) to a shallow depth (i.e., 26 to 30 feet below ground surface (bgs))
and a deeper depth of approximately 36 to 40 feet bgs at each of four (4) or five (5) locations,
subject to approval of these locations from the Client's Board of Public Works (BPW). These
four or five locations generally correspond to the locations northwest (i.e., near the intersection
of Laurel and Thomas Streets); east (i.e., near the intersection of Scott and Wayne Streets); and
north (i.e., along Colfax Avenue between LaPorte Avenue and William Street) of CTW -3S /D as
shown on Figure 1. The locations may be adjusted slightly in the field based on field
observations such as utility locations, etc.
Hull will work with the drilling subcontractor and the appropriate City and St. Joseph County
authorities to arrange for access for the work and to clear utilities and to prepare appropriate
permits for drilling in the right -of -way. Drill cuttings and sample purge and decontamination
water will be containerized and placed at a location designated by the City.
At each location where groundwater sampling is to be conducted, the borehole will initially be
advanced to the deeper depth (i.e., between approximately 36 and 40 feet bgs), at which point
the outer casing of the sampling system will be withdrawn to expose a four foot long stainless
steel wire wrapped screen. Once the screen is exposed, Hull will measure the static water level
and lower a decontaminated Waterra foot -valve sampling pump or peristaltic pump to purge the
equivalent of five well volumes of water to develop the aquifer surrounding the screen.
Following extraction of each well volume, Hull will measure pH, temperature and conductivity to
demonstrate that representative groundwater is being sampled. In addition, Hull's
representative will note if there is a visible sheen on the extracted water. After development, the
temporary well will be allowed rest for approximately 20 minutes prior to sample collection.
Following purging, Hull will collect groundwater samples that will be pumped through the
Waterra or bladder pump tubing at a flow rate of less than 100 ml /min. Samples will be
transferred directly into laboratory- preserved sample containers and placed on ice in a cooler.
Following collection of each sample, Hull will retrieve and decontaminate the Waterra foot -valve
pump by washing it with Alconox and water and rinsing it with deionized water. In addition, the
sample tubing will be replaced between each sampling interval.
The groundwater sample in the upper portion of the aquifer (i.e., between approximately 26 and
30 feet bgs) will be collected by withdrawing the screen to the appropriate depth and purging
and collecting a groundwater sample as described above. The process for purging and
collecting the more shallow groundwater sample will then be completed as described above.
Between each of the sampling locations, the drilling rods and well screen will be
decontaminated by washing with Alconox and water solution and rinsing with deionized water.
Up to 10 groundwater samples (two samples at variable depths per borehole) plus six
duplicates, collected from the temporary boreholes using direct -push technologies, will be
analyzed for VOCs. Two field blanks and two trip blanks will also be analyzed for VOCs.
Results for the laboratory analyses will be requested to be reported by the laboratory within
three business days.
Pending laboratory results from the temporary sampling points, three monitoring wells will be
installed at locations between CTW -3S /D and the Oliver Industrial Park (i.e., along Western

Avenue and along Chapin Street at the locations shown on Figure 1). Soil borings associated
with the proposed monitoring wells will be drilled using 4.25 -in., inside - diameter hollow -stem
augers and continuously sampled using 24 -in. split -spoon samplers to the appropriate depth
(i.e., either 30 feet bgs or 45 feet bgs). As with the direct -push sampling locations, the
monitoring well locations may be modified based on field observations.
Monitoring wells will be constructed of Schedule 40 PVC slotted screens and risers. Once the
targeted depth has been reached, the well column will be slowly lowered to the base of the
borehole. A clean silica quartz sand filter pack will be placed around the screen and will extend
no more than two feet above the top of the screen. Sodium bentonite chips or pellets will then
be placed on top of the sand pack. Above the chips or pellets, a thick bentonite slurry will be
pumped into the annular space using a tremie pipe to a depth of approximately three feet below
ground surface. The remaining three feet will be filled with concrete to anchor a flush -mount
manhole.
Monitoring wells will be developed following installation to remove fines that may have entered
the well screen or filter pack during installation. Well development activities will continue until
pH, temperature, and conductivity have stabilized for three consecutive well volumes, or until
five well volumes have beer} removed, whichever is greater. Development waters will be
collected and stored in a DOT approved 55- gallon drum.
Following installation of the monitoring wells between CTW -3S /D and the Oliver Industrial Park,
laboratory results from the temporary boreholes north and east of CTW -3S /D will have been
received, and will determine whether additional downgradient temporary sampling point
installation is necessary, or whether permanent monitoring wells may be installed in the vicinity
of the previous temporary sampling points. This estimate provides a contingency for additional
temporary sampling point installation should it be deemed necessary. Ultimately, permanent
monitoring wells will be installed downgradient of CTW -3S /D as part of this scope of work; the
costs for installation of all permanent well locations are included. We have included an
additional mobilization and demobilization cost for the driller to install permanent wells in two
stages should the second temporary sampling event be required. If this charge is not incurred,
the Client will not be billed.
Groundwater samples will be collected from all newly installed monitoring wells and submitted to
the laboratory for VOC analysis using U.S. EPA Method 8260 and lead and arsenic analyses
(because of historical detections of these metals in off -Site groundwater) in accordance with
U.S. EPA Method 846 6010. Samples for lead and arsenic analyses will be submitted to the
laboratory in both filtered and unfiltered form to allow for analysis of total and dissolved metals
concentrations.
Prior to sampling, the monitoring wells will be purged until pH, temperature, and conductivity
measurements have stabilized, or until three well volumes have been removed, whichever is
greater. Following purging, groundwater will be collected using low -flow methods and
transferred to laboratory- preserved sample containers at rate of less than 100 ml /min and
placed on ice in a cooler.
Duplicate samples will be collected at randomly selected monitoring well locations.
Field /equipment blanks will also be collected and submitted along with a trip blank for analysis
as part of QA/QC of field procedures. Purge and decontamination waters will be collected and

stored in U.S. DOT - approved 55- gallon drums. As described above, drill cuttings, excess
samples, purge water and decontamination water will be drummed and staged at a location
acceptable to the Client.
Costs to remove, transport, and dispose of approximately 45 55- gallon drums of containerized
groundwater and drill cuttings (which are presumed to be non - hazardous) are included in this
task.
Task 2 Prepare a Remediation Work Plan
The RWP that was submitted to City in April 2004 will be revised to include additional narratives
that describe all past environmental sampling programs completed since 1989 at the Site. In
addition, this revised RWP will include a description and analytical results of the impacted soil
areas discovered during demolition activities at Area C. Furthermore, references to
groundwater (previously removed when groundwater was to be remediated through a separate
VRP number in conjunction with other areas within the City) will be reincorporated into the
RWP. To the extent possible, the revised RWP will include copies of analytical reports,
summary tables, and location plans for historical soils investigations. Note that there may be
inaccuracies in historical sampling locations, as presented on the maps, due to a lack of survey
data and /or detailed historical site plans. Hull envisions that multiple figures will need to be
prepared to clearly show the previous sampling locations and analytical summaries.
Hull will revisit and update as necessary the draft health and safety plans, quality assurance
project plans and the community relations plans that were submitted in April 2003 into a single
set of project documents that will be inclusive of all anticipated activities associated with the
general remediation approaches for soil and groundwater.
Four hard copies and four electronic (PDF on compact disc) copies of the final deliverables will
be prepared and submitted to the City.
SCHEDULE
Hull has tentatively scheduled the groundwater sampling event to begin on October 27 and 28,
2011, subject to your approval of this proposal. Permanent monitoring well installation is
scheduled to begin October 31, 2011. Assuming the second contingent temporary sampling
point installation portion of this scope of work is necessary, with permanent monitoring well
installation to be completed within one week thereafter, we anticipate all drilling and sampling
activities will be completed by November 18, 2011. This schedule will allow for completion of
the RWP by mid- to late December in accordance with the schedule you have proposed to Mr.
Corey Webb of IDEM.
COMPENSATION
A breakdown of costs is attached as Table 1. The fees have been developed based on our
estimate of hours for each labor category expected to be involved in the project. The project will
be billed on a four -week basis with payment due to Hull within thirty days after receipt of an
invoice.

ADDITIONAL WORK
Additional work beyond the Scope of Work defined herein shall not be performed until such time
as an amendment to this proposal, including the scope of the additional work and associated
costs, has been prepared in writing to address the additional work and said amendment has
been approved by the Client in writing.
Specific items not within the Scope of Work on this project include, but are not limited to the
following:
collection of more than 72 groundwater samples (including QA/QC samples) to
be analyzed for VOCs in accordance with U.S. EPA Method 8260;
2. collection of more than 36 groundwater samples (including QA/QC samples) to
be analyzed for total and dissolved arsenic and lead concentrations in
accordance with U.S. EPA Method 6010;
3. disposal of groundwater that is determined to be hazardous;
4. submittal of soil samples for laboratory analyses; and
5. communication with regulatory agencies regarding the Site.
No amendment to this proposal shall be required in situations where additional work may be
necessary, as a result of unanticipated or unidentified conditions including, but not limited to, the
presence of unidentified or mislocated buried utilities, structures, or objects and unanticipated
hazardous materials and /or hazardous or dangerous circumstances, to complete previously
identified tasks. Hull shall be compensated for the actual time spent performing this additional
work and other direct expenses and subcontractor fees at the billing rates outlined in Table 2.
STANDARD OF CARE AND LIMITATIONS
Hull shall perform its services using that degree of care and skill ordinarily exercised under
similar conditions by reputable members of its profession practicing in the same or similar
locality at the time of service. No other warranty, expressed or implied, is made or intended by
our proposal or by our oral or written reports. The work will not attempt to evaluate past or
present compliance with federal, state, or local environmental or land use laws or regulations.
Conclusions presented by Hull regarding the Site to be investigated shall be consistent with the
Scope of Work, level of effort specified, and investigative techniques employed. Reports,
opinions, letters and other documents will not evaluate the presence or absence of any
compound or parameter not specifically analyzed and reported. The presence of radiation,
radon, lead, electromagnetic fields, and indoor air pollution will not be investigated, unless
specifically stated in the scope of work. Hull makes no guarantees regarding the completeness
or accuracy of any information obtained from public or private files or information provided by
subcontractors.

preparing for such meetings, project coordination time, design activities, field and office
investigations, and travel time when job - related.
2. Hourly billing rates reflect the range of salaries for each job classification. Rates are typically
