REQUEST FOR PROPOSALS
Sealed Applications shall be received no later than 9:30 a.m., EST, on July 24, 2018, in the office of the
Board of Public Works, 13th Floor, County City Building, Room 1316, South Bend, Indiana, for:
WATER WORKS UTILITY SERVICE LINE REPAIR PROGRAM
SUMMARY:
The Board of Public Works of the City of South Bend ("City"), on behalf of the Water Works Utility is
soliciting applications from qualified vendors to serve as preferred vendors for its Service Line Repair
Program.
Project description:
The City is authorized by Sections 17-49 through 17-55 of the City's Municipal Code to offer this Service
Line Repair Program to repair water service line leaks located between the house or building and the
water main line which would otherwise be the sole responsibility of the property owner. It is the purpose
of this request to establish a list of qualified vendors who will repair service line leaks at the rates
established herein.
The Board will open the applications at 9:30 a.m., EST, on July 24, 2018, in the Board of Public Works
Meeting, Ste, 1308, County -City Building, 227 W. Jefferson Blvd., South Bend, Indiana
46601. Representatives of the City will review the applications and notify vendors if they have been
accepted into the program pursuant to the terms below.
Applications mast meet the minimum specifications as provided herein. There is no scoring matrix
as all vendors whose applications meet the minimum specifications and who meet and accept the
following conditions will be accepted into the program.
Qualified Applicants must meet and accept the following conditions:
Requirements:
• 24-hour emergency availability
• Ability to work in adverse weather
• Ability to work in traffic
• Ability to locate water leaks and trace water lines
• Ability to freeze water lines
• Must follow 811 compliance
• "Three year warranty on work
• Company safety manual must be followed
• Must be fully equipped to work underground
• Must follow South Bend City Standards including materials
• Bills must be delivered to 915 S Olive by the 5th of the following month after work performed under the
program
• Must have experience working on water lines in the City of Souih Bend including new service line
installation
• Performance bond in the amount of $25,000 or cash bond in the amount of $25,000; and
• Proof of Commercial General Liability Insurance per Person in the amount of $50,000; and
• Proof of Commercial General Liability Insurance per Occurrence in the amount of $1,000,000; and
• Worker's Compensation Insurance.

Fee Rate:
Applicants will repair a basic service leak in grass (class 1, 2, 3, 4) for $1,450, For basic leaks in hard
surfaces such as concrete, the rate is $1,550, This flat rate will include labor, equipment & materials
including but not limited to, compressor, pump, freeze kit, compactor, saw, work truck, vac/backhoe,
dump truck, fill material, black dirt and seed, barricades, and arrow board. This flat rate includes all
emergency repairs made during normal working hours.
For stop box repair, a flat rate of$425 will be paid for repairs in grass, The rate in sidewalk or driveway is
$525. if the curb valve must be replaced, it is billed as a basic service leak with rate $1450 in grass and
$1550 in concrete.

Rules and Expectations:
• Emergency repairs require completion within 24 hours. Regular repairs require completion within
five working days.
• Applicant is responsible for customer contact prior to repairs.
• Paperwork Must be completed, scanned, and e-mailed to South Bend Water Works with details
about leak repair, parts used, and labor hours.
• City inspection and approval prior to payment.
• The City must receive monthly repair bills by the 5th day of the following month. All material
provided by City must be indicated on the invoices including the flat rate leak repairs.
• Twenty four (24) hour emergency response includes after-hours and weekends.
• On -call is scheduled on a weekly rotation.
• Applicant must be capable of working in adverse weather.
• Minimum requirement of two qualified persons for all excavations.
• Traffic control will be applicant's responsibility.
• Applicant is responsible to prepare the street, sidewalk, or driveway, according to City
specifications (4" side walk, G" driveway and driveway approach).
• Street cuts must match existing depth but not less than 8". Filling of cuts, either asphalt and/or
concrete, will be completed by the City.
• Perform customer contact, locates, leak repair, compaction, dirt and seed or prep for concrete.
• Three (3) year warranty on all work.
• City management will determine if a leak repair is not considered basic repair. Ifthere are special
circumstances as listed, the leak is billed for equipment, time, and materials.
• New service live installation will be paid per foot for one inch service line. Only K copper pipe is
allowed.
Special Circumstances:
• Any special circumstance requires City approval.
• Any leak that requires a portion of the street to be removed to complete repair is considered
special circumstance.
• Any extensive landscape removal & replacement in order to repair service leak will be considered
special cirCUMStance.
• Any leak repair involving the foundation of the structure will be considered special circumstance.
• Leak repair during non-scheduled work days that are emergency repairs per South Bend Water
Works are special circumstance.
The successful applicants shall comply with the City's ordinance and all other federal, state and local
laws and regulations governing nondiscrimination in employment.
The City reserves the right to accept, negotiate scope, or reject any or all applicants.

There is no charge for the specifications. The specifications are also available for review only during
regular working hours in the Department of Public Works, 1308 County -City Building, South Bend,

The City reserves the right to review and audit all documents related to this application and to request
additional information as determined necessary. Failure to comply with the City's request may be deemed
grounds for dismissing the proposal.
Applicants must include the Non -Collusion Affidavit, Non -Debarment Affidavit, Employment
Eligibility Verification, and Non -Discrimination Commitment Norm.
