Martin, Clerk Board of Public Works FROM: Tara Weigand, Project Engineer SUBJECT: Award Project No. 118-048A 2018 Tree and Stump Removal DATE: June 5, 2018 On May 8, 2018, the Board of Public Works opened and read quotes for the above project. have reviewed them and found them correct. The quoters ranked as follows: Quoter Total Quote Amount 1. Kachur Tree Service, LLC $ 12,675.00 2. K & R Tree Service $ 15,625.00 Upon review, it was discovered that Kachur Tree Service, LLC provided a signed Non -Collusion Affidavit that was not notarized. After being notified of this omission, the Contractor provided a signed and notarized affidavit. I recommend that the Board waive this irregularity as it was immaterial to the quote and has been corrected. I recommend award of a contract for _2018 Tree and Stump Removal to the lowest responsive I responsible quoter, Kachur Tree Service, LLC in the amount of $ 12,675.00.
