The City of South Bend Board of Public Works ("City") is soliciting Proposals on behalf of the
South Bend Police Department ("SBPD"), from qualified vendors to supply and install equipment
for the following project:
SBPD Body Cameral In Car Video Camera Project
Sealed Proposals shall be received no later than 10:30 a.m., Local Time, on September 7, 2017,
in the office of the Board of Public Works, County City Building, 227 W. Jefferson Blvd., Ste.
1316, South. Bend, Indiana 46601,.
General Requirements:
Proposals must comply with the following requirements:
1. Submit Proposals in a single sealed envelope. Contents roust be clearly marred "SBPD
Body Camera/ In Car Video Camera Project Proposals".
2. Proposals shall not exceed 15 pages.

4. Only proposals received prior to the above time and date will be given consideration.
5. All the proposals shall remain firm and open for a period of ninety (90) days from the date
of opening. The SBPD reserves the right to further negotiate after proposals are opened
with any potential consultants if deemed necessary.
NOTE: The City encourages proposals frown Minority Business Enterprises (MBE's) and Woman
Business Enterprises (WBE's) for this contract. The City will not discriminate against any
individual or company on the grounds of race, color, sex, physical disabilities or challenges or
national origin in the process section.

A selection committee from SBPD, City Purchasing, and City IT will review all of the proposals

There is no charge for the specifications. The specifications are also available for review only
during regular working hours in the Department of Public Works, Ste. 1316 County -City Building,

Proposals must include the City's Non -Collusion Affidavit, Non -Debarment Affidavit,
Employment Eligibility Verification, and Non -Discrimination Commitment Form. Bid bond
or check is required to be submitted with the proposal.

The purpose of this Request for Proposal ("RFP") by the City is to solicit seated proposals fi-om
qualified vendors to provide a Body Camera/ In -car video system, inclusive of all hardware,
software licenses, data storage/ retrieval, warranty, installation/ training, support and multi- year
discounts for a period of five (5) years for +/- 170 officers and +/- 170 vehicles.

A. Requirements (Meeting Specifications)
I. Body camera has the ability to offload video immediately to the In -Car Video Recorder via
a secure WiFi connection between the Officer and Vehicle, whenever the Body Camera is
within Wil7i proximity to the vehicle, to prevent the need to return to the station to

C. Warranty and Maintenance ,Service
I . The video system includes a parts and labor warranty against any defects in materials or
workmanship on all System software and hardware components for the duration of the
contract. Please see attached warranty terms
2. Software updates will be provided as part of ongoing maintenance at no additional cost. PRO-VtsION Meals or Exceeds
3. Departmental users have access to a 24/7 Help Desk Service Rhone Number for technical

111. PROPOSAL REQUIREMENTS
The proposal shall provide information necessary for the SBPD to evaluate the qualifications,
experience, and expertise of the vendor and sub consultants to perform the work required or supply
the equipment in the Scope of Work.
The Vendor is to make a written proposal which presents an understanding of the work to be
performed. The proposal should demonstrate and provide evidence that the Vendor has the
capabilities, professional expertise, and experience to provide the necessary equipment and/or
services as described in this RFP. The Vendor shall ensure that all information required herein
is submitted with the proposal. All information provided should be verifiable by documentation
requested by the SBPD. Failure to provide all information, inaccuracy or misstatement may be
sufficient cause for rejection of the proposal.

&ILLUAILM UE ILIL LQbIIRA!Cl; The City may terminate any agreement resulting
from this solicitation at any time, for any reason or for no reason, upon thirty days advance
written notice to the Vendor. In the event of such termination the Vendor shall be compensated
for services and work performed prior to termination. If the City does elect to terminate this
agreement for any reason, the Vendor shall repurchase any unused product from the City at the
invoiced price which the City paid.
TAXES: Sales to the City of South Bend are exempt from State sales tax. State sales and use tax
certificates of exemption, Form ST-12, will be issued upon request.
USE OF BRAND DJAMES: Unless otherwise provided in this solicitation, the name of a certain
brand, make or manufacturer does not restrict vendors to the specific brand, make or manufacturer
named, but conveys the general style, type, character, and quality of the article desired. Any
article which the City, in its sole discretion, determines to be the equal of that specified,
considering quality, workmanship, economy of operation, and suitability for the purpose intended,
shall be accepted. The vendor is responsible to clearly and specifically identify the product being
offered and to provide sufficient descriptive literature, catalog cuts and technical detail to enable
the City to determine if the product offered meets the requirements of the solicitation. This is
required even if offering the exact brand, make or manufacturer specified. Normally in
competitive sealed bidding only the information furnished with the bid will be considered in the
evaluation. Failure to furnish adequate data for evaluation purposes may result in declaring a bid
nonresponsive. Unless the vendor clearly indicates in its proposal that the product offered is an
"equal" product, such proposal will be considered to offer the brand name product referenced in
the solicitation.
IV. CONTRACT TERM
It is the intent to award a contract no later than September 26, 2017.
Prices shall remain firm for the term of the contract.
V. AWARD OF CONTRACT
The award documentation will subsequently be followed by a contract incorporating by reference all the
requirements, terms and conditions of the solicitation and the Offeror's proposal as negotiated

When the prospective Contractor is unable to certify to any of the statements below, it shall attach an explanation to this Affidavit.

to be bid by anyone at such letting nor to prevent any person from bidding nor to induce anyone to refrain from bidding,

4. Contractor does not knowingly employ or contract with an unauthorized alien, nor retain any employee or contract
with a person that the Contractor subsequently learns is an unauthorized alien. Contractor agrees that he/she/it
shall enroll in and verify the work eligibility status of all of Contractor's newly hired employees through the E-
Verify Program as defined by I.C. 22-5-1.7-3. Contractor's documentation of enrollment and participation in the
E-Verify Program is included and attached as part of this bid/quote; and
5. Contractor shall require his/her/its subcontractors performing work under this public contract to certify that the
subcontractors do not knowingly employ or contract with an unauthorized alien, nor retain any employee or contract with a
person that the subcontractor subsequently learns is an unauthorized alien, and that the subcontractor has enrolled in and is

fails to cure a breach of this provision no later than thirty (30) days after being notified by the City.
6. Persons, partnerships, corporations, associations, or joint ventures awarded a contract by the City of South Bend
through its agencies, boards, or commissions shall not discriminate against any employee or applicant for employment in
the performance of a City contract with respect to hire, tenure, terms, conditions, or privileges of employment, or any
matter directly or indirectly related to employment because of race, sex, religion, color, national origin, ancestry, age, or
disability that does not affect that pers on's ability to perform the work.
In awarding contracts for the purchase of work, labor, services, supplies, equipment, materials, or any combination of the
foregoing including, but not limited to, public works contracts awarded under public bidding laws or other contracts in
which public bids are not required by law, the City, its agencies, boards, or commissions may consider the Contractor's
good faith efforts to obtain participation by those Contractors certified by the State of Indiana as a Minority Business
("MBE") or as a Women's Business Enterprise ("WBE") as a factor in determining the lowest, responsible, responsive
bidder.
In no event shall persons or entities seeking the award of a City contract be required to award a Subcontract to an
MBE/WBE; however, it may not unlawfully discriminate against said WBE/MBE. A finding of a discriminatory practice
by the City's MBE/WBE Utilization Board shall prohibit that person or entity from being awarded a City contract for a
period of one (1) year from the date of such determination, and such determination may also be grounds for terminating the
contact for which the discriminatory practice or noncompliance pertains.

which it may henceforth enter into with the City of South Bend, Indiana or any of its agencies, boards or commissions.

awarded. I understand I have an affirmative duty to notify the City in my bid that my proposal does not include the use of
steel products or foundry products made in the United States, I understand it is my sole obligation and responsibility to
provide a justification to the City, subject to review and approval, why the cost of United States made steel or foundry
products is unreasonable. Prior to award and upon submission of bid which does not use steel products or foundry products
made in the United States, the City, through its director of public works, shall make a determination if the price of United
States made steel or foundry is unreasonable. I understand that violations hereunder may result in forfeiture of contractual
payments.

60 MONTH LIMITED WARRANTY
PRO-VISIONO Products
This warranty only applies to PRO -VISION" products originally purchased from, PRO -VISION, Inc. (the company) after March,
25, 2014. For all other PIRO-VISIONO products, only the warranty in effect at the fime the product was originally purchased from
the company applies. This warranty does not apply to IBODYCAMO brand products.
The company warrants that should any PRO-VISIONO product (except RODYCAMO brand products), under normal use and
conditions, be proven defective in material or workmanship within 60 months after the date of original purchase of the product
from the company (lifetime on video cables, lifetime on sd cards, and 12 months on batteries), such defect(s) will be repaired by
the company or the product replaced by the company with anew or reconditioned product (at the company's option) without
charge for parts or repair labor by the company. The repaired or replaced product will be returned by the company freight pre-
paid via "ground" service,
To obtain repair or replacement within the terms of this warranty, the product must be delivered, transportation pre -paid by the
warranty claimant, to the company's main office shown above, with written proof of warranty coverage (e.g.: company dated bill
of sale, company approved dated bill) of sale or company approved Vehicle In -Service Document), serial number on product and
written specification of defect(s). In addition, a Return Merchandise Authorization Number (RMA number) must be clearly
marked on the exterior of the shipping container. A RMA number may be obtained by calling 800.576.11126, Failure to obtain a
IRMA number and to clearly mark that number on the exterior of the shipping container may cause your shipment to be refused
by the company, In the event written proof of warranty coverage is not provided to company by claimant, warranty coverage will
be determined by the company based on the (product's serial number.
