The Board of Public Works, at its meeting held on May 9, 2018, approved the above
referenced proposal to provide and install new conduit for Western Avenue in the amount of
$3,500.

Install fiber to two traffic cabinets and splice to existing City WiFi ring
We hereby propose to furnish material and labor in complete accordance with the above
specifications for the sum not to exceed $ 3,500 (Three Thousand Five Hundred Dollars).
All materials are guaranteed to be as specified. All work to be completed in a satisfactory
manner according to standard practices, Any alteration or deviation from the above
specifications or involving extra costs will be executed only upon written orders and will become
an extra chairge over and above this proposal. All agreements are contingent upon strikes,
accidents, or delays beyond our control. The Owner will carry fire, tornado, and other necessary
insurance. Our workers are fully covered by Worker's Compensation Insurance, This proposal is
valid for 30 days from issue date.
Conflict of Interest: The Organization hereby certifies and agrees that no member, officer, or
employee of the City, or its designees or agents, and no member of the governing body of the
City of South Bend or the Organization (and no one with whom there is a family or business tie)
who exercises any functions or responsibilities with respect to the receipt and use of City funds
during his or her tenure or for one year thereafter, shall have any financial benefit, direct or
indirect, in any contract or subcontract, or the proceeds thereof, for workto be performed in
connection with the Program, To the extent the Program involves the awarding of a contract or
grant, the Organization shall not award the same to a person or entity that creates a conflict of
interest described in Indiana Code § 5-16-11-5.5
ACCEPTANCE OF PROPOSAL
The above prices, specifications, and conditions are satisfactory and are hereby accepted, SWIM, Inc. is
authorized to do the work as specified. Payment will be made as stated above.
The undersigned person executing this Proposal represents and certifies that he/she is duly authorized
officer with authority to execute this Proposal and has the full legal rights, power and authority to enter
into this Proposal and to grant the rights and perform the obligation herein; that no third party consent or

SUBJECT: MetroNet Zing Digital Inclusion Initiative — Expanding Public Wi-Fi

Linda and Members of the Board,
Please see attached quote/contract for MetroNet dark fiber aimed to expand
public Wi-Fi as part of the City's wider digital inclusion initiative. The expansion
will encompass parts of Western Avenue. This effort is a facet of a broader City
initiative aimed towards addressing the digital divide in South Bend, working
towards digital inclusion. The Office of Innovation and Technology team has
reviewed the quote and gives favorable recommendation as well.
The not to exceed cost of $3,500.00 is to provide and install conduits and fiber on
Western Avenue between South Kaley Street and Dundee street as well as
several blocks expanding outwards from these streets.
