The undersigned Contractor, being duly sworn upon his/her/its oath, affirms under the penalties of perjury that:
I. Contractor has not, nor has any other member, representative, or agent of the firm,
company, corporation or partnership represented by him, entered into any combination, collusion
or agreement with any person relative to the price to be bid by anyone at such letting nor to prevent
any person from bidding nor to induce anyone to refrain from bidding, and that this bid is made
without reference to any other bid and without any agreement, understanding or combination with
any other person in reference to such bidding. Contractor further says that no person or persons,
firms, or corporation has, have or will receive directly or indirectly, any rebate, fee, gift,
commission or thing of value on account of such sale; and
2. Contractor certifies by submission of this proposal that neither contractor nor any of its
principals are presently debarred, suspended, proposed for debarment, declared ineligible, or
voluntarily excluded from participation in this transaction by any Federal department or agency;

3. Contractor has not, nor has any successor to, nor an affiliate of, Contractor, engaged in investment
activities in Iran.
a. For purposes of this Certification, "Iran" means the government of Iran and any agency or
instrumentality of Iran, or as otherwise defined at Ind. Code § 5-22-16.5-5, as amended from
time -to -time.
b. As provided by Ind. Code § 5-22-16.5-8, as amended from time -to -time, a Contractor is
engaged in investment activities in Iran if either:
i. Contractor, its successor or its affiliate, provides goods or services of twenty million
dollars ($20,000,000) or more in value in the energy sector of Iran; or
ii. Contractor, its successor or its affiliate, is a financial institution that extends twenty
million dollars ($20,000,000) or more in credit to another person for forty-five (45)
days or more, if that person will (i) use the credit to provides goods and services in
the energy sector in Iran; and (ii) at the time the financial institution extends credit, is
a person identified on list published by the Indiana Department of Administration.
4. Contractor does not knowingly employ or contract with an unauthorized alien, nor retain
any employee or contract with a person that the Contractor subsequently learns is an unauthorized

alien. Contractor agrees that he/she/it shall enroll in and verify the work eligibility status of all of
Contractor's newly hired employees through the E-Verify Program as defined by I.C. 22-5-1.7-3.
Contractor's documentation of enrollment and participation in the E-Verify Program is included
and attached as part of this bid/quote; and
5. Contractor shall require his/her/its subcontractors performing work under this public
contract to certify that the subcontractors do not knowingly employ or contract with an
unauthorized alien, nor retain any employee or contract with a person that the subcontractor
subsequently learns is an unauthorized alien, and that the subcontractor has enrolled in and is
participating in the E-Verify Program. The Contractor agrees to maintain this certification
throughout the term of the contract with the City of South Bend, and understands that the City
may terminate the contract for default if the Contractor fails to cure a breach of this provision no
later than thirty (30) days after being notified by the City.
6. Persons, partnerships, corporations, associations, or joint venturers awarded a contract by
the City of South Bend through its agencies, boards, or commissions shall not discriminate against
any employee or applicant for employment in the performance of a City contract with respect to
hire, tenure, terms, conditions, or privileges of employment, or any matter directly or indirectly
related to employment because of race, sex, religion, color, national origin, ancestry, age, gender
expression, gender identity, sexual orientation or disability that does not affect that person's ability
to perform the work.
In awarding contracts for the purchase of work, labor, services, supplies, equipment, materials, or
any combination of the foregoing including, but not limited to, public works contracts awarded
under public bidding laws or other contracts in which public bids are not required by law, the City,
its agencies, boards, or commissions may consider the Contractor's good faith efforts to obtain
participation by those Contractors certified by the State of Indiana as a Minority Business
("MBE") or as a Women's Business Enterprise ("WBE") as a factor in determining the lowest,
responsible, responsive bidder.
In no event shall persons or entities seeking the award of a City contract be required to award a
subcontract to an MBE/WBE; however, it may not unlawfully discriminate against said
WBE/MBE. A finding of a discriminatory practice by the City's MBE/WBE Utilization Board
shall prohibit that person or entity from being awarded a City contract for a period of one (1) year
from the date of such determination, and such determination may also be grounds for terminating
the contact for which the discriminatory practice or noncompliance pertains.
7. The undersigned contractor agrees that the following nondiscrimination commitment shall
be made a part of any contract which it may henceforth enter into with the City of South Bend,
Indiana or any of its agencies, boards or commissions.
Contractor agrees not to discriminate against or intimidate any employee or applicant for
employment in the performance of this contract with privileges of employment, or any matter
directly or indirectly related to employment, because of race, religion, color, sex, gender
expression, gender identity, sexual orientation, handicap, national origin or ancestry. Breach of
this provision may be regarded as material breach of contract.
I, the undersigned bidder or agent as contractor on a public works project, understand my statutory
obligations to the use of steel products or foundry products made in the United States (I.C, 5-16-

8-1). I hereby certify that I and all subcontractors employed by me for this project will use steel
products or foundry products made in the United States on this project if awarded. I understand I
have an affirmative duty to notify the City in my bid that my proposal does not include the use of
steel products or foundry products made in the United States. I understand it is my sole obligation
and responsibility to provide a justification to the City, subject to review and approval, why the
cost of United States made steel or foundry products is unreasonable. Prior to award and upon
submission of bid which does not use steel products or foundry products made in the United States,
the City, through its director of public works, shall make a determination if the price of United
States made steel or foundry is unreasonable. I understand that violations hereunder may result in
forfeiture of contractual payments.

SPECIFICATIONS FOR BECK'S LAKE OVERFLOW VALVE MATERIALS PROCUREMENT
April 13, 2018
PART 1 — GENERAL
1.1 DESCRIPTION
A. Scope:
1. Supplier shall provide domestically produced materials.
1.2 REFERENCES
A. Standards referenced in this Section shall comply with current editions of the following:
1. City of South Bend Engineering and Construction Standards.
2. ANSI/AWWA C111, Rubber -Gasket Joints for Ductile Iron Pressure Pipe and Fittings.
1.3 SUBMITTALS
A. Action Submittals: Submit the following:
1. Product Data — Ductile Iron Wall Sleeve
a. Provide manufacturer and material product data.
b. Provide Shop Drawings and data for pipe, fittings, gaskets, appurtenances, linings, and coatings.
c. Provide surface preparation and application reports and procedures as required for lining and
coating of pipe and fittings. Ductile iron pipe and fitting manufacturer and manufacturer and
applicator of lining and coating, as specified, shall mutually determine recommended surface
preparation and application methods, and provide written verification of mutually selected method
in submittals.
d. Provide U.S. Steel and Foundry Products Certifications.
e. Provide certificates signed by the manufacturer of each product that product conforms to applicable
referenced standards and the procurement documents.
f. Submit results of specified shop tests for pipe, fittings, linings, and coatings.
2. Product Data — Flanged Check Valve
a. Provide manufacturer and material product data.
1.4 QUALITY ASSURANCE
A. Supplier shall be responsible for coordinating the procurement of the specified materials.
B. Supplier shall verify that the Ductile Iron Wall Sleeve and Flanged Check Valve flanges have drilled and
tapped holes with identical flange patterns for each flanged component. Compatibility and mating of the
flanges shall be verified by the supplier prior to delivery and demonstrated at the time of delivery to insure
proper fit can be obtained in the field upon installation.
C. Manufacturers:
1. Manufacturers shall have a minimum of five years' successful experience producing ductile iron pipe
and fittings and shall be able to show evidence of at least five installation in satisfactory operation in
the United States that are similar applications to the specified service.
2. Lining and coating products shall be manufactured by a firm with a minimum of five years successful
experience in protecting pipelines exposed to the specified service conditions, and shall be able to
show evidence of at least five installations in satisfactory operation in the United States that are similar

applications to the specified service.
3. When not applied by the manufacturer, lining and coating Subcontractor shall have a minimum of five
years successful experience in the application of the specified linings and coatings for similar
applications for the specified service, and shall be able to show evidence of at least five installations in
satisfactory operation in the United States.
D. Supply and Compatibility
1. Unless otherwise approved, obtain all pipe, fittings, and appurtenances included in this Section from a
single ductile iron pipe manufacturer.
2. Ductile iron pipe manufacturer shall review and approve all Shop Drawings and other submittals for
pipe, fittings, and appurtenances furnished under this Section.
3. Pipe, fittings, and appurtenances shall be suitable for the specified service and shall be integrated into
overall piping system by ductile iron pipe manufacturer.
4. Ductile iron pipe manufacturer shall be responsible for all products and all factory -applied linings and
coatings, whether installed at pipe manufacturer's facility or at manufacturer's Supplier's facility.
E. Regulatory Requirements:
1. Pipe and fittings, including linings and coatings, that will convey potable water or water that will be
treated to become potable, shall be certified by an accredited organization in accordance with
NSF/ANSI 61 as being suitable for contact with potable water, and shall comply with requirements of
authorities having jurisdiction at Site.
1.5 PRODUCT DELIVERY STORAGE AND HANDLING
A. Supplier shall coordinate delivery of the product to the City of South Bend with the City's Project
Representative at the Wastewater Treatment Plant.
B. Materials shall be delivered and inspected at the Wastewater Treatment Plant located at 3113 Riverside
Drive, South Bend, Indiana, 46628.
C. Deliveries shall be scheduled between the hours of 8:00 A.M. — 2:00 P.M., Monday through Friday. No
deliveries will be accepted on weekends or City recognized holidays.
PART 2 — PRODUCTS
2.1 PERFORMANCE CRITERIA
A. Products provided shall meet or exceed the minimum requirements of this specifications.
2.2 MATERIALS
A. Ductile Iron Pipe — Wall Sleeve (Flanged End — Plain End)
1. Ductile iron pipe materials shall be domestic for this procurement contract.
2. Wall sleeve shall be a Plain end — Flanged End configuration.
3. Flanged pipe shall be fabricated in accordance with ANSI/AWWA C115.
4. Non -Flanged pipe shall conform to ANSI/AWWA C151 for material, pressure, dimensions, tolerances,
tests, markings, and other requirements.
5. Wall sleeve shall be 30" diameter.
6. Wall sleeve width shall be 12" end -to -end dimension.
7. Wall sleeve flange shall be drilled and tapped with holes to mate with the specified Flanged Check
Valve. Coordinate bolting pattern with Flanged Check Valve manufacturer prior to fabrication. Mating
flange shall be flat and full-faced.
8. Wall sleeve flange mating surface shall be serrated to grip the rubber flange of the Flanged Check
Valve specified. The serrations should be cut 1/16-inch deep, with 90° angle tool point. The pitch
should be 8 (eight) cuts per inch.
9. Mating Flange inside diameter must match the Flanged Check Valve sleeve inside diameter.

a. Flanged Joints shall conform to ANSIIAWWA C110 and ANSIIAWWA C111.
b. Gaskets: Wall Sleeve manufacturer shall consult Flanged Check Valve manufacturer for any
special gasket material requirements.
c. Bolts: Comply with ANSI B182.1. Service condition will be submerged. Provide ASTM A193,
Grade B8M, Class 2, Heavy hex, Type 316 stainless steel.
d. Nuts: Comply with ANSI 18.2.2. Service condition will be submerged. Provide ASTM A194, Grade
68M, Heavy hex, Type 316 stainless steel.
12. Lining, General:
a. Surface Preparation:
i. Initial Surface Inspection: Surface to be lined shall be inspected by pipe and fitting manufacturer
and applicator, if applicator is other than pipe and fitting manufacturer. Inspecting parties shall
inspect surface to be coated and mutually determine recommended surface preparation
method.
ii. Surface preparation: Prepare surface in accordance with recommended method.
iii. Finished Surface Inspection: Lining applicator shall inspect finished surface prior to application
to determine acceptability. If surface is unacceptable, repeat surface preparation as
necessary.
13. Cement -mortar lining
a. Provide bituminous seal coated cement -mortar lining in accordance with ANSIIAWWA C104.
14. Marking for Identification
a. Name or trademark of manufacturer.
b. Weight, class or nominal thickness, and casting period.
c. Country where cast. All products shall be domestic, manufactured in the United States of America.
d. Year the pipe was produced.
e. Letters "DI" or "Ductile" shall be cast or metal stamped.
f. Flange manufacturer's mark, size and letters "DI" cast or stamped on the flanges.
g. Fabricator's mark if other than flange manufacturer,
h, Length and weight.
B. Flanged Check Valve
1. Supplier shall provide flanged check valve as specified herein.
2. Flange drilling shall conform to ANSI B16.10, Class 125/150#.
3. Provide Flange with 316 Stainless Steel back-up rings or retaining rings for installation.
4. Flanged check valve shall be constructed of reinforced all -rubber duck bill check sleeve type.
5. Flanged check valve shall have an integral elastomeric flange that will allow the flange to be mounted
to a mating flange or wall thimble or directly to a concrete wall.
