The Board of Public Works, at its meeting held on. April 24, 2018, approved the above
referenced amendment for additional services regarding the Corby Blvd., Ironwood Dr., and
Rockne Dr. intersection in the amount of $24,200, for a new total of $492,825.
Enclosed please find a copy of the agreement for your records.

Contract Amendment No. 1
Project: Intersection Improvement, Roundabout
Corby Boulevard / Ironwood Drive 1 Rockne Drive

This Contract Amendment is made and entered to this Contract between the City of South Bend Board of
Public Works (hereinafter referred to as "LPA") and Lawson -Fisher Associates P.C. (hereinafter referred to
as "CONSULTANT").
WHEREAS, the LPA and CONSULTANT did April 26, 2016 enter into a Contract for project
development services for the above reference project.
WHEREAS, the LPA desires the CONSULTANT provide additional services to visually verify and
horizontally and vertically map existing underground utilities at critical proposed storm sewer crossing
locations as indicated in the attached Subsurface Utility Engineering Locations figures; and
WHEREAS, in order to provide for completion of the work, it is necessary to amend and supplement
the contract in the amount of $24,200.00
NOW, THEREFORE, it is agreed by and between the parties that the following Appendices are
amended:
Appendix "A" is amended as follows:
1. ADD Task 7 — The CONSULTANT shall provide visual verification along with horizontal and
vertical mapping of existing underground utilities at critical proposed storm crossing locations.
Appendix "C" is amended as follows:
All work by the CONSULTANT under this Contract shall be completed and delivered to the LPA for review
and approval within the approximate time periods shown in the following submission schedule:

A. 1. The consultant shall receive payment for the work performed under this contract the total
amount not to exceed is $468,625 $492,825 (Section 2 - $386,700; Section 3 - $61,925;
Section 4 - $20,090 $44,200) unless an amendment is executed by the parties which
increases the maximum amount payable.

Modify Paragraph 1:
Project Description: This project consists of intersection improvement and traffic
signal modernization with installation of new curbs, storm drainage, pavement, approaches, incidental
resurfacing, and other pertinent work. Road construction will be begins approximately 650 feet south of the
intersection at the north Country Club Lane intersection and continues north approximately 650 feet to east
leg of Rockne Drive. Approximately 600 feet of improvements will occur to the west along Rockne Drive
and Corby Boulevard and approximately 350 feet east along Corby Boulevard. Sidewalks will be
reconstructed where they currently exist and upgraded for ADA compliance in accordance with INDOT
policies and standards. Collected storm water will be piped below grade 3 Sin^ either infiltration, detention
and,lerviath overflow infe existing GOrn bin P-MA9,r—in existing right of way to a new retention facility at a
location to be determined.
Modify Paragraph 4:
The proposed typical section will consist, at a minimum, of two 12 feet 11-foot travel lanes with
sheulders/parking and straight face curb for the full project length to existing widths. Pavement width
variations are expected at the roundabout intersection to the extent necessary based on INDOT design
criteria. A five (5) foot sidewalk is proposed where present with grassed parkways where possible. Existing
roadway vertical profile shall be reused to the extent possible for project areas exceeding INDOT minimum
vertical grade requirements.
Modify Paragraph 6:
Public road approaches and private drives shall be reconstructed consistent with the new curb construction
and vertical profile along the project length. No signal Signal modernization{s"} is expected to be required
as part of this project.
Modify Part B. Project Design Development:

h-. The consultant will perform landscape design and plan development services for theGeRter
area surrounding the proposed retention basin,
including fencing as required. Three conceptual planting plans fGF th9 GeAter Island will be
prepared. Conceptual planting plans will be presented in a fully rendered two dimensional
format and will illustrate design intent, color palettes, plant type and quantities. Irrigation
design services will be provided for the nontor iglan.d. not be required.
Except as herein modified, changed and amended, all terms of the original contract dated April 26, 2016
shall continue in full force and effect.
