The attached Professional Services Agreement is a proposal from Phifer Environmental
Services for a Phase II site assessment for 122 E. Sample St, Enyart Electric Motor Repair.
A Phase I was completed by Phifer Environmental for this property on July 06, 2010, which
revealed certain Recognized Environmental Conditions. A Phase II is recommended to
ensure the building's industrial use as an electric motor repair and service shop, have not
contaminated surrounding soil.
This proposal includes doing four Geoprobe soil borings, to a depth of up to 30 feet, for
laboratory testing. These four soil boring sites will then be used to gather ground water,
which will also be tested. This data will then be reviewed and compiled into a Phase II
report, with an opinion given whether further site assessment is needed.
Total project services cost is a not to exceed of $5,305.
Staff requests approval of this Professional Services Agreement, to allow a Phase II
environmental site assessment to be completed before we move forward with the
acquisition of this property.

Subject: Proposal for Phase II Environmental Site Assessment
Enyart Electric Motor Repair Facility
122 E. Sample Street

Dear Mr. Relos:
Phifer Environmental Services, LLC. has recently completed a Phase I Environmental
Site Assessment of the property identified as the Enyart Electric Motor Repair facility,
located at 122 E. Sample Street. The current tenant, Mr. Rich Siri, owns the business,
and leases the facility from the property owner, identified as the Patricia Enyart
Revocable Trust. Also owned by the Patricia Enyart Revocable Trust and included as
part of this Phase I Environmental Site Assessment, are adjoining former residential lots
that include, by address, 117, 119, 121 and 123 E. Ohio Street, with such lots being
vacant or serving as access drives and gravel covered parking areas. Together the
subject lots constitute approximately 0.4875 -acres of land.
As documented by the Phase I Environmental Site Assessment, the following
Recognized Environmental Conditions were identified for their potential for
environmental impact to the subsurface environmental:
1) The subject site (122 E. Sample Street) was identified as a location of a former
6,000 gallon heating oil tank by the regulatory database search. The UST, which
was removed from the ground in January 1990, was reportedly assessed for
potential releases at the time of the tank removal. However, no records of the
assessment were available through the current tenant or search of the IDEM
Virtual File Cabinet. We note that in 1990, the requirements were for retention of
records for only a period of up to three years. Therefore, in the absence of any
available information regarding the assessment of the UST at the time of removal;
the former UST system is considered to be a Historical Recognized Environmental
Condition. We note that the use of the UST was as an on -site consumptive fuel
(heating oil), and that the system would be considered "exempt" by current
regulations.

2) The facility (122 E. Sample Street) was identified as a RCRA CESQG by the
regulatory database search, with waste streams identified as Not Defined (D000),
Ignitable Hazardous Wastes (D001) and Spent Non - halogenated solvents (F003).
Such wastes are generally the result of mixed wastes generated by two parts
washing basins containing mineral spirits. Such mineral spirits, as well as waste
oils generated on site, and received from off -site, are burned within an on -site
waste oil furnace. Overall, an estimated 200 - gallons of mineral spirits and 275 -
gallons are burned annually by the waste oil furnace, with introduction by an
aspirating (air compressor) system. The waste oil /solvent mix is stored within
aboveground storage tanks that are adjoining the furnace unit. As a result of
transfer operations and blow -down from the air compressor unit (aspirator),
significant staining of the concrete flooring was identified in the area of the waste
oil furnace. Some level of subsurface contamination may have resulted as a
result of the incidental releases associated with the waste oil furnace operations
(Recognized Environmental Condition).
3) Numerous floor drains were identified throughout the workshop area of the
building. Although attempts had been made to "close" several of the drains, there
remains some doubt as to how effective such closure was in sealing off potential
entry of waste oils and other potential contaminants. Other drains remained
operational and were located in areas of the building whereby the introduction of
waste oils and potentially solvents (i.e., mineral spirits) was possible. The
discharge point for drains is thought to be the municipal sanitary sewer
(unconfirmed); however the likelihood exists that at least one of the drains
(principally installed to facilitate the removal of snow melt) may discharge to the
on -site dry well located within the rear parking area of the facility. Staining of the
concrete in areas surrounding the floor drains was visually apparent. Based on
the aforementioned factors, the floor drains and the potential introduction of waste
oils and possibly solvents to the subsurface environment throughout 45 years of
motor repair service, is considered a Recognized Environmental Condition.
It should also be noted that the immediate area of the subject site has been the location
of significant industrial activity dating to the early 1900s, and that potential sources of
contamination in the immediate area of the subject site may not be fully delineated. As
such, the potential exists that as yet "undiscovered or unreported" releases may be
identified in the future (i.e. off -site sources). We also note that the chemical and physical
characteristics of the primary Contaminant of Concern (Tetrachloroethylene) for both the
Sibley Foundry and Ziker Cleaners Corrective Action Plan activities (north of site), to that
of waste oils and mineral spirits identified as being associated with historical Enyart
Electric Motor Repair operations, are distinctly different. Thus, the potential for on -site
environmental impact as a result of material handling activities conducted at the Enyart
Electric Motor Repair /Service facility cannot be discounted based solely on the
groundwater date from two nearby, off -site well locations installed during assessment of
Tetrachloroethylene associated with the Sibley Foundry and Ziker Cleaners.
Based upon the aforementioned, PES has recommended a Phase II Environmental Site
be undertaken for the purpose of assessment the extent to which, if any, the historical
activities have potentially impacted the subsurface environment. Accordingly, the
following sections present our approach toward conducting the Phase II Environmental
Site Assessment, projected costs and a tentative schedule for completion.
PROPOSED SCOPE OF SERVICES
Soil Sampling via Geoprobe Methodology
Geoprobe methodologies (small track mounted hydraulically driven sampling probe) will
be used to collect soil samples from a total of three locations across the subject site. A
fourth soil boring location is proposed for the interior of the building, within the area that
has been identified as the location of former heating oil UST system. Because of the
interior location, a concrete coring device will likely be required in order to gain access to
undertling soils. It is anticipated that the soil sample from this location will be collected
at a depth of 10 to 12 -feet, below the bottom of the former UST system.
For each of the Geoprobe boring locations, discrete soil samples will be collected at 5-
foot intervals throughout the soil profile to a depth of 30 feet below ground surface.
Each soil sample interval will be screened for the presence of volatile organics by
placing a portion of the soil sample into a plastic bag, allowing time for equilibration, and
insertion of a photo ionization detector (PID) probe to observe a reading of "total VOCs"
in parts per million (ppm). All data associated with the field screening of VOCs will be
recorded for future reference (Soil Boring Logs).
In addition, a portion of each discrete sampling interval will also be collected for
submittal to the laboratory. PES proposes to submit one soil samples from each of the
four boring locations based on the PID field screening results and /or other indications of
the presence of potential contamination (visual or olfactory). In the event that field
screening with a PID, visual or olfactory observations do not indicate the presence of
contamination; the depth interval immediately above the soil /groundwater interface will
be submitted for laboratory analysis. Each of the soil samples selected for laboratory
analysis will be analyzed for the presence of Volatile Organic Compounds (VOCs), TPH
Extended Range Organics (TPH -ERO) and Polynuclear Aromatic Hydrocarbons (PAHs).
PES will utilize IDEM guidance Terra -Core Methods to collect soil samples from each
discrete sampling interval for the analysis of VOCs. Soil samples to be analyzed for
TPH -ERO and PAHs will be collected within pre - cleaned, laboratory provided 4 -ounce
jars. All soil samples will be labeled, packed on ice and forwarded to the laboratory
under chain -of- custody procedures.
Temporary Geoprobe Well Installation for Groundwater Sampling
PES proposes to convert each of the three Geoprobe soil borings to temporary wells
with insertion of a Geoprobe groundwater sampling device, which allows for the
extension of a stainless steel slotted screen for collection of a groundwater sample from
the point at which the saturated or water - bearing zone is encountered. For this
proposal, PES has assumed that the water - bearing zone will be encountered within 30-
feet of land surface. We propose that the three temporary wells for consist of: 1) an up
gradient location, assumed to be along the southern property boundary 2) two down
gradient locations, assumed to be the western property boundary (within the adjacent
common alleyway with Mason's Heating & A /C), and along the sidewalk area at the front
of the building.
Once installed, each temporary well would be purged until such time as the water
discharge becomes relatively free of solids or until at least three gallons of water have
been removed. PES proposes that groundwater from each of the temporary wells be
analyzed for similar constituents identified for soil samples (VOCs and PAHs). Each
groundwater sample will be appropriately labeled, packed on ice, and forwarded to the
laboratory (priority overnight delivery) under chain -of- custody procedures. PES will plug
the boreholes /wells with bentonite once soil and groundwater sampling activities have
been completed.
Reporting
Upon receipt of the laboratory analyses, data will be reviewed and tabularized.
Comparison of the data for each respective chemical constituent will be made to the
Risk Integrated System of Closure (RISC) Default Closure Values for Residential and
Industrial /Commercial Properties. Upon completion of the initial data review, a "Letter
Report" will be prepared which presents the sample collection and analytical
methodologies used during the field phase of the assessment. Copies of the sample
location maps, tabulated data and comparison to RISC Default Closure Values will also
be incorporated. Recommendations for any additional assessment activities will be
made for review and comment.
SCHEDULE
PES proposes to commence site investigation activities within one week of receipt of
notification to proceed (depending on weather, utility clearance and Geoprobe
availability). Upon mobilization, we anticipate completion of the Geoprobe sampling
requiring one day. Laboratory analyses will be completed by Pace Analytical on
standard one -week turnaround from receipt by the laboratory. Our initial discussion of
results will occur within one day of receipt of laboratory analytical results. Our written
"Summary Letter Report" will be submitted within one -week of the initial conference.
COST ESTIMATE
An ESTIMATED COST for Phase II Environmental Assessment Services with respect to
the Baseline Environmental Assessment is presented as follows:

Please note that this ESTIMATED COST should be considered a Project Maximum,
which PES will not exceed without prior approval. However, should the scope of
services require modification (i.e., additional borings) based upon field conditions and /or
circumstances that would not have otherwise been anticipated, PES will notify the client
of such "changed conditions" and resultant cost implication to the project.
LIMITATIONS /QUALIFICATIONS
The sampling and testing of soil, groundwater and /or other materials not specifically
identified in this proposal is beyond the scope of services for this project. Cost estimates
for our proposed services have been prepared based on the following assumptions:
• PES will have unlimited access to the site during daylight hours;
• Interior areas identified for borehole installation will be cleared to allow for sufficient
access for equipment
• No subsurface obstructions (buried utilities or fill debris) will interfere with our work;
• Sufficient access /clearance will be provided for exterior Geoprobe locations;
• Groundwater will be encountered within 30 -feet of ground surface;
• Inclement weather will not interfere with the completion of our field work;
• Soil cuttings and purge waters will be returned to the boring from which they were
generated, with the remainder of the boring filled with bentonite pellets to land
surface;
This Phase II Environmental Assessment should be considered as a "tool" in the
evaluation of the subject site relative to the potential presence of contaminants, and
does not necessarily fulfill Indiana Department of Environmental Management (IDEM)
guidelines for delineation of contaminants identified within soil or groundwater.
