Amendment of Agreement - 236 E Sample block building demo
Date: October 15, 2010
On September 17, 2010, the Commission approved a Professional Services Agreement
with Christopher Burke Engineering (CBE) for demo specs and environmental services for
the demolition of the cement block building at 236 E. Sample St. This building is next to Ivy
Tech's administrative offices at 250 E. Sample St.
In planning for the demolition of 236 E. Sample and the new fagade that was put on 250 E.
Sample (the ITOSS building), it has been determined that new sidewalk, curbing, and tree
lawn would fully complete the project.
The attached Professional Services Agreement Amendment from CBE will provide the
necessary topographic survey and design services to bid out this work.
Total project services cost for this additional work is $3,803.
Staff requests approval of this Professional Services Amendment, to allow for this work to
be done, to complete the transformation of this section of E. Sample St.

Dear Mr. Relos:
As you and Toy Villa requested at your meeting with Jason Durr on September 29tH
2010, we are providing this amendment for a change to the original scope of our
project for additional work. The scope changes and additional work are described
below:
1. Provide a topographic survey on the west side of Carroll Street from Ohio to
Sample and on the south side of Sample Street from the West property line
of 236 Sample to Carroll Street.
2. Provide a design for new curb, sidewalk and ADA ramps on the west side of
Carroll Street from Ohio to Sample and on the south side of Sample Street
from the west property line of 236 Sample Street to Carroll Street.
Additional Estimated Fee: $3,803.00
This amendment is subject to the General Terms and Conditions included in the
original contract.
