RE: TOPOGRAPHIC SURVEY ZIOLKOWSKI CONSTRUCTION SITE/TREELAWN
SURVEYING, SOUTH BEND, INDIANA
Dear Bill:
Thank you for the opportunity to present our proposal for surveying for the above referenced project.
We have appreciated being able to be of service to you in the past and hope that we can assist you
with this project.
We have reviewed the documentation and project limits for this project and offer the following
proposal.
SCOPE OF SERVICES
Topographic survey shall include the following:
• Utility locates will be performed on Ziolkowski Construction site located at the Southeast
corner of Lafayette Blvd. and Garst Street.
• Utilities and topographic features between the existing edge of pavement and the right of
way lines for the following street: Lafayette Blvd. from Sample Street to Stull Street.
Limitations:
• Utility lines will be shown based on field evidence located during our field survey and per
plans and marking provided by the owner, utility companies, or their representatives. We
cannot certify that all utilities required are available.
The estimated fee for performing the topographic survey will be $2,820.00.

Be assured that all of the above will be delivered in a continuously timely manner with the timetable
of the project held foremost. As you know, Wightman Petrie, Inc. guarantees the best possible
product provided in the most efficient manner and accordingly will strive to reduce costs wherever
possible.
Our services will be billed monthly unless the amounts in any month are not significant. Our monthly
statements will include charges for some expenses that we may incur on your behalf. Should you
have any questions or comments concerning our services or charges during the course of the work,
please bring them to our attention immediately so any problem can be resolved quickly.
If the above is acceptable to you, please sign below and return one signed copy of this proposal at

Subject: Professional Service Proposal -Wightman Petrie
Topo Survey on Lafayette Blvd (Sample to Stull)
Date: August 3, 2010
Attached is a proposal from Wightman Petrie to provide survey services for the right-of-way
corridor between Sample Street and Stull Street on Lafayette Boulevard. Adjacent to the
work area are AirGas and Ziolkowski, both Commission owned and slated for Fall 2010
demolition, as well as other commercial-industrial businesses.
The purpose of the survey work is to provide detailed information about curbs, walks, trees
and drainage structures in the corridor so that design work can be done for curb/sidewalk
replacement as well as design and construction of a pedestrian pathway along the west
side of Lafayette to complement the pathway built along the Transpo site.
The proposal is in the amount of $2,820.00 and staff recommends approval of this proposal.
