The Board of Public Works, at its meeting held on March 13, 201, 8, approved the above
referenced amendment to increase costs to repair damaged large brass valves in the amount
of $1,400, for a new total of $11,805,

FIRST AMENDMENT
CULLIGAN OF MISHAWAKA AGREEMENT
This First Amendment to the Equipment and Services Agreement between Culligan of
Mishawaka, having its offices at 56861 Ferrettie Ct. Mishawaka, IN 46545 ("Culligan"), and City of
South Bend, Indiana acting by and through its Board of Public Works ("City) is made effective March
13,2018.
WHEREAS, Culligan and City are parties to an Equipment and Services Agreement dated
January 23,d , 2018 ("Agreement"), incorporated by reference and attachment;
WHEREAS, the parties wish to modify certain terms and conditions of the Agreement; and,
NOW THEREFORE, in consideration of the mutual promises and obligations set forth herein,
the parties make this First Amendment to Agreement as follows,
1. Equipment and services to be provided by Culligan is updated to include the supply and
installation of two 2" brass valves to replace existing ones damaged in the removal of the old
units.
2. Compensation will increase by $1,400.00 for anew total of $11,805.00,
Except for the provisions herein contained, said Agreement, will remain in full force and effect
as previously made.
IN WITNESS WHEREOF, the parties hereto have caused this Amendment to be executed as
of the day and year first above written.
