DO R AIJ Page 1 February 7, 2018 SUPPLEMENTARY AMENDMENT #I TO THE AIA B101-2017 AGREEMENT BETWEEN OWNER AND ARCHITECT BY AND BETWEEN: CITY OF SOUTH BEND VENUES PARKS & ARTS and GREGORY A. KIL & ASSOCIATES, INC. (d/b/a Kit Architecture/Ptanning) Agreement Dated July 18, 2017 Page 1, Project Description, Add the additional Paragraph description under the paragraph below the project address at bottom of the page, as follows: The Schematic Design Study dated January 12, 2018 will serve as the basis of the Detailed Design for the renovation. This scope entails a "Renovation Option," of limited scope, since budget is limited. The Scope of the base and alternates are as follows: 1. New Site work: -Replace accessible wood ramp to main north entry with new more durable concrete ramp and metal railing. -Remove the west coal bin, infilI and pave -Modify grade for storm drainage at east side of building and site to direct water away from building -Address golf cart circulation: pick-up and drop-off 2. Exterior Architectural: -New Roof and internal gutter repair/rework -Exterior Painting and Siding repair -Demolish existing deck (alternate to replace with smaller deck) -Increase visibility of course from clubhouse where possible 3. Architectural Upgrades as follows: -North Portico deck upgrade to provide an accessible route to the main entry doors Main Level: -Main level to be converted to banquet/gathering/event space -Provide renovated HC Men/Women restroom -Retain Concessions at East part of this level (no major changes, space to be retained as concession, or as serving kitchen for caterers) -Alternate to open up west fireplace for sitting area for lounge seating (Gas Log Set) -Alternate to modify stair landing area at NE corner, this level, opening this access up -Alternate to provide new exterior balcony at south side, south new stair access. If not, replace south door with window. Loft Levels: -Retain as is for sitting area (West) and meeting room (East) Lower Level: -Modify east fireplace for sitting area for lounge seating (Gas Log Set) -Starter/Pro Shop and limited concession room and window relocated to lower level west side, at former boiler room. -Provide new M/W restrooms at lower level -Provide golf simulator space (as size (w x 1 x h geometry allow) -Alternate for new bar and concessions at east portion of this level. -Alternate for conference/meeting room 4. New mechanicals: -Demolish older boiler system and replace with new HVAC systems throughout -Provide all new restroom plumbing fixtures and systems 5. (d/b/a Kil Architecture/Planning) Agreement Dated July 18, 2017 Article 1.1.1, The Owner's Program for the Project: add the following: "The Sept 21, 2017 Architectural, Structural and MEP Conditions Assessment and updated Schematic Plans dated January 12, 2018 will serve as the Program for the project. Given that the Owner's funding for the project is limited to $1,000,000.00 (one million dollars) for Architectural/Engineering design and Construction Cost, the plan for implementation is to complete prioritized scope that will allow for the primary renovation to occur and to provide ease of future additional scope upgrades where possible, while working within the budget allowed." Article 1.1.3, The Owner's budget for the Cost of the Work, as defined in Section 6.1: add the following: "The Owner's funding for the project is limited to $920,000.00 (nine hundred twenty thousand dollars) for Construction." Article 1.1.4, The Owner's anticipated design and construction milestone dates: Add the following: "Design Development to be completed within 2 months of contract amendment execution Construction Documents to be completed within L5 months of approved Design Development Drawings Project to be bid in Spring/ Summer of 2018, with Construction to start in early summer of 2018. Construction is expected to require 6 to 8 months to complete." Article 2.5.1 Commercial General Liability, delete and replace with the following: "Commercial General Liability with policy limits of not less than One Million Dollars ($1,000,000.00) for each occurrence and Two Million Dollars ($2,000,000.00) in the aggregate for bodily injury and property damage." Article 2.5.2 Automobile Liability, delete and replace with the following: "Automobile Liability covering vehicles owned, and non -owned vehicles used, by the Architect with policy limits of not less than One Million Dollars ($1,000,000.00) per accident for bodily injury, death of any person, and property damage arising out of the ownership, maintenance and use of those motor vehicles, along with any other statutorily required automobile coverage." Article 2.5.5 Employers' Liability, delete and replace with the following: "Employers' Liability with policy limits not less than Five Hundred Thousand Dollars ($500,000.00) each accident, Five Hundred Thousand Dollars ($500,000.00) each employee, and hive Hundred Thousand Dollars ($500,000.00) policy limit."
