The Board of Public Works, at its meeting held on January 23, 2018, approved the above
referenced agreement regarding access to City -Owned land for field surveys and
geotech.nical investigations at Boland Park, Boehm park and parcels along Boland Drive.

Access Permit - Survey Permission
I/We hereby give Indiana Michigan Power Company, a(n) Indiana corporation, a unit of American
Electric Power ("AEP"), its affiliates, agents, employees, contractors and the appropriate federal,
state and local agency's, archeologists, biologists, and/or environmental scientists, permission to
enter upon my/our premises to conduct such surveys, soundings, drillings, appraisals, and
examinations and tests necessary (including the right to drill holes for testing soil and bedrock) for
the proposed electric transmission line route.
AEP has the right to trim or cut vegetation necessary for survey purposes and agrees to pay the
prevailing market price for standing timber for any marketable trees cut down in the course of such
survey. AEP will be responsible for any damage done to such property and further agrees to
indemnify against all liability to third persons caused by negligent acts of AEP's employees while
on the property.

Attached you will find the original easements for each parcel of land effected by the rebuild transmission
project. The tract numbers are in the title of each document and can be referenced to the numbers in the top
right corner of each exhibit A. Also, attached is an updated AEP survey permit for the Board of Public Works. If
