this past year as a towing company. We would like to renew this contract for another year, and

Asap towing & Recovery is pleased to submit this proposal for services to support The South Bend City
Police Department in achieving its goals for improving efficiency, customer service, and satisfaction by
providing removal, towing, and storage services. ASAP is committed to improving the customer
experience through convenience, accuracy of orders, professional interactions, and timely work. ASAP

EXECUTIVE SUMMARY
The City of South Bend Police Department is in need of one properly licensed towing company to provide twenty-four
hour a day, seven days a week service for the removal, towing, and storage impoundment of certain motor vehicles
within the city of South Bend. Additionally, the city has deemed the need for a secure storage facility within five miles of
the City of South Bend, and that that the towing entity named will provide all billing services for vehicles removed
and/or stored.
The City of South Bend Police Department is in need of one properly licensed towing company to provide twenty-four
hour a day, seven days a week service for the removal, towing, and storage impoundment of certain motor vehicles
within the city.
The goals are:
• One properly licensed towing company to provide twenty four hour a day, seven days a week removal and
storage services.
a One company that shall maintain and have exclusive control of three recovery vehicles that are DOT registered.
o Storage facilities that are within five miles of South Bend City limits, that are both secure and operate a
minimum of six days per week.
m Proper billing, storing, release of vehicles, and recording of charges for vehicle owners and services provided.
® Fair and just cost for vehicle owners and indemnity of the city of South Bend for any costs associated with the
outlines of this proposal.
ASAP Towing & Recovery {hereinafter referred to as "ASAP/The Company" is fully licensed by the State of Indiana and
the City of South Bend to own, operate, and provide removal, towing, and storage services for the City of South Bend
and the surrounding area. The company owns and operates five vehicles, twenty-four hours a day, seven days a week,
that are capable of passenger vehicle and light truck removals. The company provides fully secured storage, within the
City of South Bend, and has the capacity to house two hundred vehicles at any given time.
We will provide:
o One properly licensed towing company to provide twenty four hour a day, seven days a week removal and
storage services.
• Five recovery vehicles that are DOT registered and licensed with the state of Indiana.
• Our secure storage facility located within the city limits and the ability to house 200+ vehicles.
® Hours of operation that comply with timeframes outlined in RFP.
• Proper billing, storing, release of vehicles, and recording of charges for vehicle owners and services provided.
s Fair and just cost for vehicle owners and indemnity of the City of South Bend for any costs associated with the
outlines of this proposal.

o ASAP Towing & Recovery has a rolling land line at the base station office (2010 S. Olive Street) and
communicates with each of the five drivers via company cell lines. During evening hours when the office is not
manned, calls are dispatched by the office answering service. Calls to the mainline land line office phone are
forwarded to the answering service and calls are then dispatched to the drivers via company cell phones.
® ASAP Towing & Recovery holds indemnity and liability insurance under the Korson, Inc., the parent company, of
ASAP Towing & Recovery. Liability insurance covers up to 1,000,000.00 and is held by Arrowhead —certificate
41688980991, policy #327799.

Per resolution requirements, ASAP, owns, maintains, and has exclusive use and control of the vehicles listed below. All
vehicles named have the capacity to tow passenger, light truck, have a one ton capacity, and rear dual wheels. Per state
regulations, each company vehicle prominently displays the name, logo, phone number and DOT number on both sides
of the vehicle so it can be easily identified. Vehicle markings are in accordance with Indiana Department of Motor
Carrier Services Division 49 CFR 390,21.

In an attempt to provide cleaner and more efficient services, ASAP Towing & Recovery utilizes diesel exhaust fluid (DEF),
in the vehicles listed above. DEF allows us to avoid the disadvantages associated with increasing EGR to the extreme
levels necessary to meet the EPA requirement of near -zero NOx emissions (0.2g/bhp-hr).
DEF is an aqueous solution of 32.5% urea and 67.5% de -ionized water. It is clear, non -toxic, biodegradable and non-
flammable. The biggest advantage of DEF is maximized fuel efficiency. The higher oxygen levels DEF provides offer more
efficient combustion resulting in clean nitrogen and water emissions which are nontoxic and harmless to the
environment.
Additional equipment, that is not DEF compliant are:

o Per proposal request outlines, the International has a lifting capacity of eight tons.
a) Vehicle Equipment —the following is included in each company vehicle —list includes but is not limited to:
L A minimum of four LED flashing emergency lighting.
ii. Debris removal equipment, consisting of: Two oversized containers, a broom, shovel, fluid absorbent (Floor-
dri), safety chains, and emergency beacons.
iii. Company employees are provided with reflective vests/jackets that meet OSHA/ANSI requirements. Our
drivers wear uniforms with reflective stripes as well. All employees are required to wear reflective clothing
when on scene or during transport of removal vehicles.
ASAP Towing & Recovery, as well as all employees of the company agree to and shall be available for inspection should
the City make the request; and subject to random inspections to ensure quality performance.
ASAP Towing & Recovery currently operates a daily maintenance, cleaning, and safety check of all vehicles and the
storage facility. It is the responsibility of all employees to comply with safety and cleaning checks. ASAP agrees to
continue these standards to ensure the safety of both its employees and customers.
ASAP agrees to report to the City, in writing, any changes, additions, or deletions from the company's tow truck fleet
used under this contract.

ASAP Towing & Recovery owns and operates two adjacent commercial properties, In an attempt to better serve
our customers both ASAP properties are conveniently located within the city limits, on the local South Bend City
bus line, and within five miles of the South Bend Police Department where customers will pick up vehicle release
forms.
o 1906 S. Olive Street - houses the main office, indoor storage facilities, and outdoor storage.
2010 S. Olive Street — is an additional storage lot and also houses minimal room for indoor storage
ASAP Towing & Recovery can house fifteen passenger size vehicles within the indoor storage facility located at
1906 S. Olive Street.
ASAP Towing & Recovery provides twenty-four hour indoor and outdoor monitoring and surveillance of both
storage facilities using the following systems:
o Lot.and indoor surveillance are monitored via the Swann Pro DVR system -- benefits include
24 hour DVR recording
s 16 Channel 960H Digital Video Recorder with pre -installed 5TB HDD
® Remote access and smart viewing via Smartphone or computer
® 16 TVL line Cameras with H.264 recording technology and night vision capacity
o The Swann system has the capacity to hold seven days of twenty-four hour surveillance.
All recordings are transferred weekly to a USB drive and held for thirty days in case an incident is
reported.
o Additional twenty-- four hour security

o Additional security measures include:
• Outdoor lot and premises are surround by six foot security fencing with wire.
• Lot gates are electronic and can only be opened by authorized employees.
Per resolution requirements, ASAP hours for payment and release of vehicles include the following times:
0 8am — 6pm Monday through Friday
0 8am —12 pm Saturday
o Sundays, holidays, and after hours pick-up can be made by appointment and an additional fee of $50.0a
Hours of operation, as well as, after hours contact information signs are clearly posted on entry doors of both
locations.
A Procedures for payment and release of impounded vehicles is as follows:
Upon owner obtaining a release form from the police station and presenting it to ASAP, we will ask for a valid photo id.
(Driver's license, passport, or state issued id) we will compare the paperwork to the release form and registration. We
will make copies for our files of the release form, photo id and registration. The invoice will then be presented for
payment and signature of the owner. Forms of payment for a vehicle requiring a police release will be cash and or
credit/debit card. No personal checks will be accepted. After payment is received the owner will be provided a receipt
and escorted to their vehicle.

At this time ASAP has decided to remain with the fees listed in the Board of Public Safety Resolution No. 88-2011
effective December 22, 2011,
1. Initial tow of a vehicle including cleanup, hookup and transport:
Impounded vehicles not involved in an accident $ 145,00
Non -Impounded vehicles due to accident $ 165.00
2, A second tow of the vehicle arising out of the same accident or impound, even if not on the same date as a
result of the actions of the owner or driver.
2nd tow $ 50.00
3. If no ignition key has been provided by the owner or driver at the time of the tow, a key fee will be applied.
$ 10,00
4. A storage charge per calendar day at the storage yard at the rate of $25.00 per day for every calendar day the
storage yard is open for pick up for at least 3 hours. This charge may also be made for Sundays and national
holidays even if the yard is not open. However, no storage charge shall be made for the day the vehicle is
impounded.
5. Administrative fee:
a. $40M if car is unclaimed for less that fifteen (15) days after impound
b, A total of $120.00 if car is unclaimed for fifteen (15) or more days after impound,
6. If the owner requests that the car be picked up at a time outside its posted business hours, legal holidays
excepted, a special pick up fee of $50.00 will apply.
7. The office shall have authority to authorize in writing additional charges when warranted by exceptional
circumstances.
8. Vehicle owner and Tow Company are free to negotiate charges for additional services requested by the owner.
9, Additional fees will apply as follows:
Inside storage $ 35,00
Window wrap per window $ 25.00
Tarp fee $ 75.00
Hazmat disposal $ 25,00
Rollover fee $100,00
Extensive winching over 20ft $ 3.00 per foot
ASAP Towing & Recovery is licensed by the City of South Bend, to operate the business of vehicle removal and storage -

In accordance with the request for proposal, ASAP agrees to the following:
a. The company agrees not to move or attach any vehicle until instructed to do so by the investigating officer. All
company employees will complete training on policies and procedures outlined with the removal, towing, and
storage procedures for the city.
b. All company employees will cooperate with the investigating officer and comply with requests made in
conjunction with the removal of unsafe and/or illegally parked vehicles from the street, and in storing or
impounding of such vehicles.
c. The company agrees to provide clean-up and removal of all debris from accident scenes, including the removal
of hazardous materials and vehicle fluids. (See equipment list for materials used)
d. The company will follow vehicle release protocols outlined in the RFP and stated by the City of South Bend
Police Department.
® Employees will comply with investigation officer by signing and gathering of proper signatures, on scene,
for stored vehicle report,
e No vehicles will be disposed of, relinquished, or released without proof of ownership, customer
identification, and South Bend City stored automobile release form.
e. As outlined in subset d — no vehicle will be released or items relinquished without proof of identity, South Bend
City stored automobile release form, and payment for services rendered. ASAP Towing & Recovery will bill
vehicle owners for services provided — No services will be billed to any city entity,
f. ASAP Towing & Recovery agree to notify the city within five business days, in writing, of any person complaining
of missing items or damage to a vehicle authorized by the city to tow. The company will abide by South Bend
City Municipal Code w- Chapter 20, Sec. 20-13 (C) and Sec. 20-15 (C) which outlines the release of responsibility
for The City, its agents, servants or employees, shall not be responsible or liable in any manner for the damage
incurred in the towing, storage, possession or disposition of such vehicle.
g. All company employees have completed background checks and agree to provide fingerprints and a criminal
history search. Employee names will be provided upon request.
ASAP agrees to report to the City, in writing, any changes, additions, or deletions from the company's tow truck fleet
used under this contract.

Per resolution addendum — issued September 1, 2016 ASAP Towing & Recovery understands the importance of timely
service in emergency situations.
® The company agrees to give priority to all city removal requests and has dedicated, as outlined, in equipment
provisions three trucks for city removal services.
The company will respond to all city removal requests within the twenty minute time frame outlined.
ASAP Towing & Recovery agrees to clean-up any debris, spill, or hazards from accident scenes and removal
services to ensure the area is safe for continued use of the roadway at the tow site.
The company adheres to federal guidelines for clean-up and disposal of all hazardous materials.
All ASAP Towing & Recovery trucks include the following equipment to further ensure safety:
o Debris removal equipment, consisting of: Two oversized containers, a broom, shovel, fluid absorbent
(Floor-dri), safety chains, and emergency beacons.

ASAP understands the importance of good communication and agrees to the RFP communication outline and guidelines.
Under the guidelines of the RFP, the company agrees should three attempts be made to contact ASAP Towing &
Recovery without success, the City has the right to contact another towing company. To avoid this type of error ASAP
works with a rolling land line, cell phone communications, as well as a back-up answering service in case of emergencies
that could interrupt communications between itself, the City of South Bend Police Department, customers, and tow
vehicle drivers.
Continuous communication is held between the dispatch office and drivers via a rolling land line at the base
station office (2010 S. Olive Street) and communicates with each of the five drivers via company cell lines.
During evening hours when the office is not manned, calls are dispatched by the office answering service. Calls

the drivers via company cell phones
In case of emergency, if main office phone lines are down — the company has set up an automatic forward and
calls will be answered and dispatched via the company answering service.

ASAP Towing & Recovery as well as all employees of the company agree not to assign its rights, under this contract, to
any other person or entity without prior written notice and consent from the City of South Bend. The company and all of
its employees understand that any such assignment could constitute a breach of contract.
The company shall, at all times, maintain separate and accurate records in connection with the storage and removal
services it provides the City and shall make all records available to the City upon request.
Records to be maintained shall include the following:
a. Time, date, location of tow, and officer authorizing the tow;
b. Make, model, vehicle license registration, and VIN number of towed vehicles;
c, Itemized billing of basic tow charges, special charges, storage charges, and any special vehicle repair/care
charges, i.e. window wrap, key charge, etc.;
d. Signed release form by person claiming the vehicle;
e. If applicable, complaint form, signed by the vehicle owner reporting any complaints regarding charges, vehicle

South Bend Police Department for the period of one year and that subsequent contracts may be renewed under the
City's sole discretion in one year increments thereafter.

Korson Inc DBA ASAP Towing has purchased Workers Compensation insurance effective 9/9/2016 thru
9/9/2017 thru our agency. This coverage was placed thru the Assigned Risk Pool. We are working to
generate the necessary Acord certificate for the City of South Bend, We are not able to generate this
certificate until the Pool assigns a carrier and policy number for this risk which can take a day or two, Once
the carrier is assigned we will be able to generate the necessary Acord Certificate as required by the City.
