FYI ... H.B. Fuller permit includes the following:
1) H.B. Fuller shall report flow monthly.
2) H.B. Fuller Authorized Representative shall sign all reports including but not limited to
Periodic Compliance Reports (PCR), Slug Control Plans, Flow Measurement Plans, and
corrective actions in response to permit violations.
3) H.B. Fuller, LLC shall submit a "Flow Measurement Plan" within ninety (90) days of the effective
date of this permit, as described in Part Ill (I).
4) H.B. Fuller, LLC shall notify the Pretreatment Group orally within twenty-four (24) hours of
becoming aware of a permit violation, and must follow up in writing with details and corrective
action within five (5) days.
5) H.B. Fuller, LLC shall submit a Slug Control Plan within ninety (90) days of the effective date of
this permit. This plan shall be resubmitted every even numbered year, 2016, 2018, 2020... Please
see Part (III ) (F) for details.
6) Only the Authorized Representative shall be required to sign the certification statement
found in Part III (A) (3).
7) Section Part IV (B) (15) has been added in order to clarify the requirements concerning the
Authorized Representative.
8) For all after hour (after 4:30 PM) required oral notifications call: 574-277-8515 and dial
zero (0) to reach an operator.

RE: Transfer of Ownership Industrial Wastewater Discharge Permit
Dear Board Members:
The Division of Environmental Services recommends approving the Industrial Discharge Permit transfer of
ownership From Royal Adhesives and Sealants to H.B, Fuller LLC at the December 12, 2017 Board of Public
Works Meeting.
Besides the change in ownership from Royal Adhesives and Sealants to H,B, Fuller LLC, there have been no
changes to the permit,
