PROPOSAL
CUSTOMER: LaSalle Hotel DATE: January 6, 2009
JOB LOCATION: 237 N. Michigan St. South Bend, IN
JOB DESCRIPTION: Clean Up
GENERAL CONSTRUCTION REQUIREMENTS
1) Remove all trash and debris from floors one (1) through nine (9).
2) Remove all trash and debris from game room (basement)
3) Dispose of (up to) six (6) residential style refrigerators
4) Leave all stainless steel appliances and/or furniture in the kitchen area on the first floor.
5) Remove and dispose of all curtains and window treatments from all floors
6) Remove wood blocking the arch windows between the storage room and the second floor common area
7) Collect and organize all hazardous liquids (i.e.: paint, cleaners, etc) and place all materials in an identified location
8) Collect and organize all identified items and store in location specified.
Project duration is estimated at 38 working days for completion.
*Payment will be made following the Redevelopment Commission meetings.*
Notes: *All employee insurance will be can~ied by CB Richard Ellis ~ Bradley.
Excludes:
1. Any work not specifically noted above.
2. Any and all work associated with unforeseen conditions.
3. Any final connection for phone, data, or electrical.
LABOR, MATERIAL & EQUIPMENT COST
Based on the information supplied to us the total project cost is $36,975.00.
OMISSIONS
This proposal does not include: Permits, Performance Bond, Builder's risk, Premium Labor rates
PAYMENT
50% of the proposed cost will be due within seven (7) days of execution of this proposal. The remaining outstanding balance will be due upon
completion of the work. This proposal is in good standing for 30 days. We trust this agreement satisfactorily sets forth your understanding of the
terms and condition between us. If this agreement meets with your approval, please sign in the space provided and return one copy to us as our
written "Notice to Proceed" If you prefer to issue a Purchase Order, please include this proposal in your correspondence with your purchase
order. CB Richard Ellis/Bradley will upon request from the owner supply all liability insurance coverage. Thank you for your confidence in CB
