This Change Order to the Contract document shall supplement, amend, and shall become a
part of, and be incorporated as an Addendum to the Contract.
The following is a summary and description of the revisions to the Contract by this Change
Order:

Net Change Order amount $3,174.00
1) Light Storage: Two lights are unable to be installed this year because they are on the
section of Monroe St that cannot be worked on until the Berlin Place development is complete
along that section. The lights must be stored until Fall 2018.
2) Time Extension: A final completion time extension is needed because the contractor cannot
finish work along Monroe St until the Berlin Place development is complete along that section.
Anticipated schedule for this is Fall 2018.
