AMENDMENT TO OWNER -ENGINEER AGREEMENT Amendment No. 2 1. Background Data: a. Effective Date of Owner -Engineer Agreement: July 12, 2016 b. Owner: Board of Public Works, City of South Bend, Indiana c. Engineer: Lawson -Fisher Associates P.C. d. Project: South Bend Dam Underwater Inspection and Bathymetric Survey City Project No. 116-071 2. Description of Modifications: a. Engineer shall perform or furnish the following Additional Services: In addition to the contracted Scope of Work for the South Bend Dam Underwater Inspection and Bathymetric Survey and subsequent amendments, LFA will prepare the following: 1. Prepare a Public Safety Plan (PSP) for the project to be submitted to the Federal Energy Regulatory Conu-nission (FERC). This fan will be prepared per the FERC requirements. 2. Prepare an emergency contact list per the requirements noted in the June 9, 2017 FERC letter. 3. Prepare a memorandum discussing the status and plan for the West Race Headgate Project that is currently in progress. This memorandum will be prepared so that it can be submitted to the FERC to provide an update for the project. b. For the Additional Services or the modifications to services set forth above, Owner shall pay Engineer the following additional or modified compensation: $4,000.00 C. The schedule for rendering services is modified as follows: 0 Completion of the above items and submittal to the City by December 8, 2017. 3.
